Application Guidelines for

PRC Licensure Examination for Psychometrician

with CAV-CHED Authentication
Reminders:

• Lead time for processing 4-5 weeks upon confirmation of payment by the Treasury
Department
• Follow the instructions strictly to avoid inconvenience and delay
• If applicant would like to get an additional copy of documents listed here, kindly make a
separate application since this follows a specific procedure.
• If you have any questions or clarifications, you may email our personnel in charge Mr.

Procedures:

1. Open Office of the Registrar official website (https://registrar.ust.edu.ph)

2. Under services, click “request now”
3. Logon using your credential during your stay in the University.
4. Under document list, select the following:

a. Transcript of Records for PRC Licensure Examination - Applicant must email

photo with white background (in .jpg format)
b. CHED AUTHENTICATION (For graduate who is applying for CHED
Authentication only WITHOUT DFA)

5. Enter Shipping Details (where you want your documents to be delivered.)

6. Download summary of request (OSRA)
7. Pay the total amount stipulated on the website.
8. (Very Important) Send the following to registrar-online@ust.edu.ph,
treasury@ust.edu.ph, and rico.dalisay@ust.edu.ph
a. Scanned copy of proof of payment
b. OSRA
c. Scanned copy of 2X2 picture with white background in .jpg format
d. Scanned copy of valid ID
9. Wait for the verified OSRA payment number that will served as your tracking number.
10. Wait until documents are delivered to the address you provided.
