The researcher is a third-year student of Sultan Kudarat State University

Kalamansig Campus with a course of Bachelor of Secondary Education Major in

General Science. As a requirement for the researcher’s course, the researcher is

conducting a survey about ‘BLENDED LEARNING: KNOWLEDGE AND SKILLS

AMIDST POST COVID-19 PANDEMIC ‘over questionnaires- checklist.

The researcher is blessed to include you as a suitable partner to the research

problem which this study wishes to resolve by providing answer to the questions, with

your related knowledge and experiences. Rest assured that all data gathered will be

treated with utmost confidentiality and will be used for academic purposes only. It will
