1.0 INTRODUCTION
1.1 Purpose
This plan specifies steps to be taken by Tombim to safely carryout lifting and
positioning of 6no transformer. The lifting shall be done by a 30T crane with
SAW 17.5T and will be carried out one after the other.

2.0 MANPOWER AND EQUIPMENT LIST

2.1 Roles & Responsibility
a) DPICL – Assess, plan and oversee the safe execution of the lifting
operation
b) Crane Operator – Control the Crane Operations as per approved lift plan
c) Riggers – Inspect, install, remove rigging gear and support during lift
operation per approved lift plan
d) Banks-man - Co-ordinate the lifting movements and maintains radio
and/or visual communication with crane operator
e) Safety Officer – Shall ensure involved personnel observe all safety
precautions
2.2 Equipment List
The following are the list of equipment and have passed required pre-mob
inspection/ certification. Each lift shall be drawn from the under listed
equipment as required. All equipment shall be kept within valid certificated
pre-mob status at all times to minimize potential delays when required.

3.0 SAFETY PRECAUTION

3.1 Pre-lift Activities
The DPICL must ensure the following activities are carried out prior to lift.
a) Conduct risk assessment for tagline usage prior to lifting operation.
b) The rigging configuration set up checklist must be completed and signed
off prior to lifting activities.(DPICL)
c) The crane operator shall carry out crane checks with the mobile crane
pre-use inspection checklist, with oversight check from the Designated
Person In charge of Lift. (DPICL).
d) Ensure all certifications for crane, personnel; equipment and tools are
present and current. All rigging equipment must be inspected, colour-
coded and certified by an approved third party vendor before use.(DPICL)
e) Pre use inspection/ certification verification shall be carried out by
specialized personnel.
f) Ensure crane footprints are clear and free from obstructions (including
access/egress).
g) Identify and confirm to all parties access and egress path of trucks with
transformers.
h) Confirm crane operator knows where to position the crane, where loads
will be located prior to lifting, as well as the location to lift/set the loads.
i) Pre-lift meeting/walkthrough must be conducted with truck driver to
determine how truck will approach lift area
j) Confirm the correct weight and size of the load (length, width, height).
k) Confirm center of gravity of the load.
l) Confirm pad eyes on transformer are fit for use
m) Identify appropriate lifting and rigging materials (Crane, hook block, slings,
shackles, etc.).
n) Confirm working radius for pick up and final location radii is free from
obstruction
o) Confirm transformer plinth is capable of carrying load i.e., verify strength
from result of “crush test”.
p) Ensure weather conditions and wind speed are within working parameters

q) Inspect ground stability/ condition to determine the load bearing capability

of the ground with fully extended outriggers, including potential
obstructions to outriggers.
r) Site access control during lifting operation to be in place. Barriers to be in
position after cranes/truck positioned and prior to lifting.
s) Hold JSA review and Toolbox talk with all persons involved with task.
Banks-man to be identified to all personnel involved in the lifting
operations
3.2 Lift Guides
The documents listed here must be in place to facilitate a safe lift.
a) Permit to work
b) Job Safety Analysis
c) Approved Diagram/Sketches of operation (appendix 1)
d) Approved Method statement
e) Crane Charts (appendix 2)
f) Third party certification of the lifting equipment and accessories (appendix
3)
3.3 Lift & Installation Steps
The transformers to be lifted are as seen in the table 2. During lift, the crane
shall be placed at approximately 12m away from the load and the following
steps shall be followed.
a) Check transformer for any damage (do not lift where substantial damage is
observed)
b) Position lowbed truck at minimum distance away from crane
c) Connect sling with shackle to each pad eye of the tranformer (the slinge
shall be 4 legged and connected together by master link).
d) Ensure all 4 lift points are positively locked
e) Connect master link to crane hook (30T crane)
f) Ensure that the crane hook is approximatly centered parpendicularly to the
top of the transformer. Tension the sling to verify so.
g) Lift few inchs away from the ground to confirm stability and the capability
of lifting aids. If any abnormality is observed; terminate lift, resolve
anomally and repeat everything fresh
h) Else, lift, swing and drop while the load is guided by 2 riggers with 2 marine
ropes ties to either ends of the base of the tranformer
i) While lowering, install anchor bolts as follows;
i. Suspend the lifted transformer at 0.8m away from the plinth
ii. Insert anchor bolts in the 4 no. transformer hold down plates
iii. Guide transformer such that the installed bolts penetrates the plinth
provided anchor holes
j) Lower transformer gradually to position while ensuring that the installed
bolts are centered
k) Verify that the tranformer is centered properly
l) Sag the 4 legged sling connection, remove shackle and transfer the 4
legged slings to laydown area

4.0 LIFTING EQUIPMENT DATABASE/REGISTER

A lifting equipment database/register will be maintained on the site with details of
all lifting equipment. The lifting equipment database / register will be compiled and
maintained by the DPICL and will show both the identification and certification status
of the equipment. Files shall also be maintained of all current certification for lifting
gear on the site.
5.0 EMERGENCIES
All lifting operations must cease immediately in the event of any emergency alarm
signals. The Operators shall make the Side Boom/Crane and any load safe, with
loads lowered into a safe area.
During thunderstorms, operations involving crane must cease immediately and the
equipment powered down.
