Thank you for choosing Prudential BSN Takaful Berhad (PruBSN) as your preferred health and medical
protection solution provider. As we strive to provide our customers with the best possible service, we would
like to share an update on your Total Pandemic Protection coverage eligibility.

We regret to inform you that we did not receive your consent to remove the exclusion clause “any
communicable disease requiring quarantine by law” for your medical plan(s) before 30 November 2022.
Consequently, you will not be eligible for Total Pandemic Protection coverage and the exclusion will remain
as an exclusion to your medical plan.

If you would like to receive Total Pandemic Protection coverage now, you can consult your servicing agent
or bank representative. You will need to complete a Supplementary Proposal Form (endorsement application
form)* which may be subject to underwriting assessment and a possible increase in contributions adjustment
(if applicable).

dan perubatan pilihan anda. Dalam usaha kami untuk menyediakan perkhidmatan terbaik kepada pelanggan
kami, kami ingin berkongsi maklumat terkini mengenai kelayakan untuk Perlindungan Pandemik Menyeluruh
anda.

Dukacita dimaklumkan bahawa kami tidak menerima persetujuan anda untuk mengecualikan klausa
pengecualian “sebarang penyakit berjangkit yang memerlukan kuarantin di sisi undang-undang” pada pelan
perubatan anda sebelum 30 November 2022. Oleh itu, pengecualian tersebut kekal pada pelan perubatan
anda dan anda tidak dapat manfaat Perlindungan Pandemik Menyeluruh.

Jika anda ingin mendapatkan Perlindungan Pandemik Menyeluruh sekarang, anda boleh merujuk kepada
ejen atau wakil bank anda. Anda perlu melengkapkan Borang Cadangan Tambahan (borang permohonan
pengendorsan)* yang mungkin tertakluk kepada penilaian pengunderaitan dan perubahan sumbangan (jika
ada).

1. What is Total Pandemic Protection about?

Effective from 22 January 2022, the eligible medical plans provide additional coverage of Total Pandemic
Protection by removing the exclusion clause “any communicable diseases requiring quarantine by law”
from product documents. For the existing certificate holders who own the eligible medical plans prior to
22 January 2022, the Total Pandemic Protection is only provided if you have issued your consent to
remove the exclusion on “any communicable disease requiring quarantine by law” from your medical
plan before 30 November 2022.

2. What is “communicable diseases requiring quarantine by law”?

Any communicable diseases requiring quarantine by law refers to infectious diseases or transmissible
diseases that are declared as requiring quarantine under the Prevention and Control of Infectious
Diseases Act 1988, such as COVID-19 mentioned under the Prevention and Control of Infectious Diseases
(Amendment of First Schedule) Order 2020. This is an exclusion in your medical plan.

3. What are the medical plans that are eligible for Total Pandemic Protection?

The Total Pandemic Protection coverage is offered to the existing certificate holders for the following
medical plans:
(a) HealthProtector
(b) Medic Protector
(c) Medic Booster
(d) Takaful Health
(e) Annual Limit Waiver
(f) Takaful Health2
(g) TH Annual Limit Waiver
(h) TH Overseas
(i) TH Auto Upgrade
(j) TH Retirement
(k) HealthEnrich
(l) HealthEnrich+
(m) Medic Essential

4. Can I opt in for the Total Pandemic Protection coverage now?

No. Any submission after 30 November 2022 will not be accepted. However, we provide Total Pandemic
Protection coverage for certain endorsement applications. Please consult your servicing agent or bank
representative for endorsement application which may be subject to underwriting assessment and
possible contribution adjustment (if applicable).

5. What type of endorsement will be provided with Total Pandemic Protection?

Any of the following endorsement applications will be provided with Total Pandemic Protection:
(a) Inclusion of Medic Protector, Medic Booster or HealthProtector rider;
(b) Upgrade of Medic Protector, Medic Booster or HealthProtector (Increase Room & Board or Smart
Value Point / decrease of SmartSaver or deductible amount);
(c) Downgrade of Medic Protector, Medic Booster or HealthProtector (Decrease Room & Board or Smart
Value Point / increase of SmartSaver or deductible amount).

1. Apakah Perlindungan Pandemik Menyeluruh?

Berkuat kuasa dari 22 Januari 2022, pelan perubatan yang layak akan menyediakan perlindungan
tambahan melalui Perlindungan Pandemik Menyeluruh di mana klausa pengecualian "sebarang penyakit
berjangkit yang memerlukan kuarantin di sisi undang-undang" dimansuhkan daripada dokumen produk.
Bagi pemegang sijil sedia ada yang memiliki pelan perubatan yang layak sebelum 22 Januari 2022,
Perlindungan Pandemik Menyeluruh hanya disediakan jika anda telah memberikan persetujuan anda
untuk klausa pengecualian "sebarang penyakit berjangkit yang memerlukan kuarantin di sisi undang-
undang" dimansuhkan dari pelan perubatan anda sebelum 30 November 2022.

2. Apakah “sebarang penyakit berjangkit yang memerlukan kuarantin di sisi undang-undang”?

Sebarang penyakit berjangkit yang memerlukan kuarantin di sisi undang-undang merujuk kepada
penyakit berjangkit yang diisytiharkan memerlukan kuarantin di bawah Akta Pencegahan dan
Pengawalan Penyakit Berjangkit 1988, seperti COVID-19 yang dinyatakan di bawah Perintah Pencegahan
dan Pengawalan Penyakit Berjangkit (Pindaan Jadual Pertama) 2020. Ini adalah pengecualian di dalam
pelan perubatan anda.

3. Apakah pelan perubatan yang layak untuk Pelindungan Pandemik Menyeluruh?

Perlindungan Pandemik Menyeluruh ditawarkan kepada pemegang sijil sedia ada bagi pelan perubatan
berikut:
(a) HealthProtector
(b) Medic Protector
(c) Medic Booster
(d) Takaful Health
(e) Annual Limit Waiver
(f) Takaful Health2
(g) TH Annual Limit Waiver
(h) TH Overseas
(i) TH Auto Upgrade

4. Adakah saya masih boleh menyertai Perlindungan Pandemik Menyeluruh sekarang?

Tidak. Sebarang penyerahan selepas 30 November 2022 tidak akan diterima. Walau bagaimanapun, kami
menyediakan Perlindungan Pandemik Menyeluruh berdasarkan permohonan pengendorsan yang
tertentu. Sila rujuk dengan ejen atau wakil bank anda untuk permohonan pengendorsan yang tertakluk
kepada penilaian pengunderaitan dan perubahan sumbangan (jika berkaitan).

5. Apakah jenis pengendorsan yang perlu saya mohon untuk layak mendapatkan perlindungan
tambahan melalui Perlindungan Pandemik Menyeluruh?

Sebarang pengendorsan berikut akan disediakan dengan Perlindungan Pandemik Menyeluruh:


(a) Penambahan rider Medic Protector, Medic Booster atau HealthProtector;
(b) Peningkatan manfaat Medic Protector, Medic Booster atau HealthProtector (Peningkatan Bilik dan
Penginapan or Smart Value Point / pengurangan SmartSaver atau jumlah Deductible);
(c) Pengurangan manfaat Medic Protector, Medic Booster atau HealthProtector (Pengurangan Kadar
Bilik dan Penginapan or Smart Value Point / peningkatan SmartSaver atau jumlah Deductible).
