Currently, I am working with Piramal Swasthya Foundation (ADT) project, as a Block

Transformation Officer under“Integrated Child Development Scheme and Maternal
Health” department in Sonbhadra District.

Job Responsibility

 Lead frontline workers at Block Level ANM, ASHA and AWW.

 Supporting Supervision and sensitization of frontline worker at Block Level.
 Ensure smooth data collection, data collation at Block Level.
 Submit collated data and reports on monthly basis.

Job Responsibility

 Extend on field advisorship for the Swasthya Bharat Prerak upto District, Block and
Village level.
 Create performance and progress reports of the scheme as required.
 Design and define procedures for the proper implementation of the Poshan Abhiyaan
in consultation with the India Nutrition Initiative.
  Day to day monitoring and coordination of the Poshan Abhiyaan. periodic review of
the work of the District, reporting the same to States.
