1.0 BACKGROUND OF MINISTRY

The Ministry of Health is a vital ministry in Malaysia. Khairy Jamaluddin Abu Bakar is the
minister who served in the ministry of health. The Ministry of Health began prior to
independence and continued to grow until it became a model for neighbouring countries. The
Ministry of Health's function is to support individuals in obtaining and maintaining a specific
degree of health status to further enable them to lead a productive lifestyle, socially and
economically. Each ministry has its own distinct mission and vision. The mission of the ministry
of health is to engage and collaborate to assist and help individuals succeed in health,
appreciating wellbeing as a precious asset and accepting responsibility and effective change for
their wellbeing. Additionally, to guarantee a high-quality healthcare system that is customer-
focused, inclusive, affordable, efficient, technologically suitable, adaptive to changing
environmental conditions, and innovative, Simultaneously, the vision would be for the nation to
strive collaboratively toward improved health (Ministry of Health, 2021).

In 2020, the Ministry of Health would get an allocation of RM30.6 billion from Finance
Minister Tengku Datuk Seri Zafrul Tengku Abdul Aziz. The operational budget is RM27.94
billion, whereas the development budget is RM2.67 billion (Official Portal of Ministry of Finance
Malaysia, 2020). The Ministry of Health has launched the Know Your Medicine Program to
guarantee that consumers take medications rationally and that the correct information is given
to consumers. The second programme is Clinical Pharmacy Services, which ensures patient
stability by counselling patients on the wards and ensuring proper and adequate
prescription use.

Budget 2021 has been allocated a total of RM31.9 billion. It surpasses the RM30.6
billion budgeted for 2020. RM27.2 billion for operational and RM4.7 billion for development
budgets (Official Portal of Ministry of Finance Malaysia, 2020). The allocations made in this
budget proposal report for the Ministry of Health's operational and development budgets are
consistent with those made in Budget 2021. Additionally, for budget 2021, the programmes that
we proposed include CLINIC4U, and Motherhood Guides as well as mot Test Kids, which were
developed by our group members.

2.0 DESCRIPTION OF THE PROGRAMME

2.1 Programme 1: CLINIC4U

Clinic4U is a three-year programme, an alternative offered by the government to the


people, particularly the Malaysian Ministry of Health (MOH), in order to expand the health clinic
sector's services to hard-to-reach places. As is well known, the medical industry is one of the
most important drivers of economic development, encompassing various groups and individuals
regardless of race. Meanwhile, Clinic4U will focus its operations in rural areas. As we have
noted, this can help many rural residents get essential health services and medicines as well as
other assistance such as advice from health professionals. However, we will also provide free
COVID-19 swab testing services, and even medical treatment services from doctors are also
available at an affordable price. We must restrict our movement because of the COVID-19
epidemic (Ministry of Health, 2021). Therefore, the MOH will set up clinics in every district
throughout the country so that the rural population does not have to go to the city just to get
COVID-19 swab testing services and other medical services.

In essence, as previously stated, the operations will be carried out over a three-year
period in accordance with the phases that have been outlined. 220 branches will be opened for
3 years. Number of clinics launched according to the size of the particular states in Malaysia.
For example, in Pahang, it will be 22 branches there because the big area of the state and the
relatively large population meanwhile in a state which with a small area and a low population
such as Perlis, will open only 3 CLINIC4U branches. The Ministry of Health will assure that
Clinic4U facilities are available in all districts throughout the state of Malaysia to ensure the
public's health and wellbeing. Therefore, with the existence of Clinic4U in rural communities, the
health ministry may be more convincing to locals with the presence of health professionals.

2.1.1 Activity of the programme

 COVID-19 Swab Test

One of the Clinic4U programme's activities is the COVID-19 Swab Test. The goal of this
endeavour is to identify and combat the COVID-19 outbreak. In addition to counselling with a
health expert, Clinic4U offers a COVID-19 swab test so that people living in rural and remote
locations can obtain it if they show early signs and symptoms of COVID-19. Naturally, the public
no need to worry, as the physicians assigned to each clinic will explain in detail regarding the
techniques of the application process to ensure no abuse or wastage occurs. At the same time,

outside households are not forced to go to the city just to get this COVID-19 swab test but they
only need to go to the nearby Clinic4U branch to get the COVID-19 swab test services.

            Additionally, the Ministry of Rural Development is assisting in the success of this
initiative. As seen by its participation, it demonstrates its care for the advancement and comfort
of rural residents. However, COVID-19 is one of the world's most deadly pandemics, having
already killed over 250 million people. Thus, by disclosing this information, Clinic4U will assist
local communities in obtaining the requirements and services of clinics in order to break the
COVID-19 cycle through physicians’ advice and treatment of health professionals and the
COVID-19 swab test services provided. For example, when an individual has symptoms of
COVID-19, he needs to immediately go to a health centre such as Clinic4U to make a swab test
by the doctor so that he may know whether he was infected with COVID-19 virus or not. Also,
the symptoms can be curbed thereby preventing the COVID-19 virus from spreading to others.
As a result, these people's activities can alleviate the strain on a variety of parties, including
families, close contacts, casual acquaintances, and the ministry of health.

 1 Week 1 Medicine (1W1M)

1W1M activities are designed to make it simpler for the community to use medications
and equipment offered in the clinic, regardless of the location of the Clinic4U branch. It is
because there are instances of medication misuse where people have a limited understanding
of a medication and are unaware of the additional side effects that might lead to death, damage,
or other undesirable effects.

Clinic4U will use flyers to educate the nearby populace about the medication each week
through 1W1M for three-years period. To supplement other approaches to spreading
information about pharmaceuticals and equipment supplied in clinics, Clinic4U staffs will be
using social media. In this era of globalisation, the majority of people are familiar with the
technical advancements occurring in both large cities and minor towns. Therefore, by spreading
from one social media platform to another, they are able to boost awareness of the benefits and
drawbacks of items supplied in clinics. The final approach is through advertising. For individuals
without a smartphone or who are unable to read, advertising can be viewed in the same way as
it does on television.

2.1.2 Function

The primary goal of Clinic4U is to enable individuals in rural areas to gain access to
medical services, such as obtaining COVID-19 swab testing services, physician treatment, and
advice from health professionals. The Malaysian Ministry of Health plans to develop 220
CLINIC4U branches within the three years the program is underway, following changes in state
regulations. In addition, the Clinic4U operations are limited to rural regions like Ulu Tembeling,
Pahang.

            Furthermore, to enhance rural households' awareness as to what is provided at


the clinic, the ministry of health will publicise on social platforms and share risks associated with
uses, benefits, cons, guarantees, side effects, and ways of usage, among other things. For
instance, the public might aware the functions and the suppose limit of usage of that medicine
on every age level. Consequently, the establishment of the Clinic4U Programme ensures that
individuals living in rural regions are not neglected and their health is not jeopardised, as well
as work together to prevent and control the spread of this pandemic.

2.1.3 Target group

Clinic4U has its own intended audience, which consists mainly of people living in rural
areas throughout Malaysia. These individuals should have access to the very same
infrastructure as the citizens of the city. According to Datuk Nazri Ngah, Pahang's Exco for
Felda, Cooperatives, Entrepreneurs, and Consumer Affairs, the Jerantut UMNO Task Force
Team is equipped to embark on hazardous journeys using four-wheel drive vehicles and a boat
to assist residents in remote parts of Ulu Tembeling (Rosedi, 2020). Undoubtedly, their
situations are challenging, much more so during this pandemic.

Consequently, the initiative, developed in partnership with the Ministry of Rural


Development, is anticipated to ease the burden on citizens who live in rural areas and struggle
to obtain adequate supplies. As a government institution, we owe it to the people to
accommodate them, as they represent the country's future. As a result, we cannot look at these
individuals' fates with one eye closed. As previously stated, rural areas of Sabah continue to
confront infrastructure and facility challenges. In comparison to the peninsula, in which
everything is self-contained.

            As a response, it is vital that we prepare as fully as possible at the national health
division to guarantee that the people's health and education are not jeopardised. Indeed, this

programme has the potential to increase their understanding of the importance of COVID-19
swab testing using appropriate methodologies to ensure that it does not spread to others.

2.2 Programme 2: Motherhood Guides

The Motherhood Guides programme would be a first for the government. This initiative is
designed to target pregnant women throughout the country, which includes every state in
Malaysia. As it is commonly recognised that pregnant women require attention to their
substances and feelings, one method that the government may use to assist them is to develop
a mechanism like this. As a result, this programme will be focused on them since all aspects of
their health must be coordinated. Finally, but certainly not least, this initiative will be
collaborating with the Ministry of Women, Family, and Community Development (KPWKM),
which, was founded with the goal of assisting women and families.

2.2.1 Activity of the programme

They will be concentrating on activities such as yoga and parenting classes in this
programme. This is because, as previously said, we will be emphasising the pregnant mothers'
and parents’ emotional and physical health as they cope with their kid. Additionally, many
individuals are unaware of the benefits of exercising during pregnancy and are unconcerned
with their child's feelings and care.

 Yoga Class

To begin, pregnant mothers are included in yoga sessions that will average roughly 50
people per week and will be taught by a specialist in yoga practise. There will be two expert
instructors and ten employees who will serve as assistants to the instructors or as observers to
the participants. The programme runs from 10am to 11am, or approximately one hour every
week. The benefit is that it will support them in their pregnancy journey. Every technique will be
shown effectively by the professionals so that they may practise it at home as well.

 Parenting Class

Parenting lessons are critical for everyone who requires motivation and instruction on
how to handle their children. The training is open to both new and seasoned parents. Several
topics will be covered during the program, including how to effectively communicate with
children, comprehend them in any way, and many others. Moreover, this initiative will be
supported by the Ministry of Women, Family, and Community Development (KPWKM). For each
state, the ministry of health will appoint two parental coaches to provide a lecture on the
parenting class, as well as ten helpers. The lecture lasts for two hours. Thus, the primary

objective of the activity is to assist them in gaining greater education on how to advocate for
their children.

2.2.2 Function

There are several purposes for developing the programme, one of which is to assist
pregnant mothers in maintaining a healthy lifestyle. Many expectant moms assume that
pregnant women cannot exercise because they are concerned about the baby's condition.
However, exercise is beneficial for them during childbirth later. For example, yoga is one of the
greatest exercises they can do to support themselves. There are several yoga postures, such
as mountain pose, chair pose, and triangle pose, but since this programme is oriented at
expecting moms, Prenatal Yoga is the ideal option (Greatist, 2019). Furthermore, prenatal yoga
is a type of yoga that is designed exclusively for pregnant women. Yoga is intended to
harmonise the emotional, mental, physical, and spiritual aspects of one's being. Also, prenatal
yoga focuses on soothing the body and emphasising safe practises and postures during
pregnancy in order to assist you in preparing for childbirth. Thus, it is recommended that
pregnant women exercise for at least 30 minutes. Yoga is a low-impact fitness activity that may
help you improve your mood and sleep, strengthen and stretch your muscles, and alleviate
lower back stiffness and other pregnancy symptoms (WebMD, 2021).

Next, the purpose of parenting classes is to educate parents on their children's growth,
various methods of parenting and discipline, efficient communication with their children, as well
as how to cope with their child's shifting moods. Raising a kid is not a simple task for anyone,
which is why this programme was established to assist individuals with families in learning a
little bit about parenting. However, these programmes will provide you with the materials
necessary to develop and enhance your abilities, as well as educate you on how to
communicate effectively with your child at home. Additionally, participating in these programmes
enables you to develop fundamental parenting skills, such as how to employ constructive
discipline with children, and to ask questions concerning raising children in a secure
environment. Finally, these programmes provide an excellent opportunity for parents to meet
other parents who are struggling with similar challenges and may share their parenting
experiences (Mrunal, 2018)

2.2.3 Target group

To commence, pregnant women from all states in Malaysia, including Sabah and
Sarawak, are prioritised for yoga lessons. This programme will be held weekly (week 1 and

week 3) on Wednesdays from 10am to 11am. This is to ensure that their health is indeed taken
care of during pregnancy and to provide a possibility for everyone to join the programme without
having to pay for anything. Then again, the majority of pregnant women are predisposed to
mental health conditions such as depression and emotional issues as a result of a range of
internal and external causes, including physical, psychological, and socioeconomic concerns.
However, according to psychiatrist and MedTweetMY Member Dr Ahmad Rostam Md Zin,
pregnant women who encounter over than two life situations and pressure have a three to
fourfold increased risk of mental health problems compared to pregnant women who live a
normal life, making them more susceptible to problems such as postpartum psychosis or
insanity (KASSIM, 2019). Thus, by performing this activity weekly, they may use their leisure
time and indirectly alleviate their mental tension.

Following that, the target population for parental classes is parents who are currently
dealing with challenges related to child rearing but want to learn more about family. The
programme will be held weekly (week 2 and week 4) on Wednesdays from 10am to 12pm. The
programme's purpose is to educate parents as to how to approach, interact effectively, and
cope with their child's shifting attitudes. This is because the government has recorded 3,875
instances of child abuse, maltreatment, and neglect up to September 2020. Alternatively,
compared to 2019, the incidence rate decreased to 6,061 from 5,578 in 2018 (HAIROM, 2020).
However, such matters should not be treated lightly, since the case may resurface at any time,
for instance, in 2021. In the four months from January to April of this year, the Social Welfare
Department (JKM) documented a total of 2,040 instances of child abuse (Jalil, 2021). As a
result, constructing this programme has the potential to prevent and further reduce exploitation
each year. There is a reason why parents continue to abuse and neglect their children, which
could be due to an increase in the average age of first marriage, urbanisation, lifestyle changes,
economic status, and greater use of family planning (Thesundaily, 2021). Thus, by developing
this programme, they will be able to control the situation without committing any blunders.

2.3 Programme 3: COVID Test Kids

COVID Test Kids is a programme implemented by the Ministry of Health to help reduce
COVID-19 cases among school students. It was inspired by a developed country that always
puts all its citizens first, regardless of age. The objective of this programme is to reduce and
decrease the number of COVID-19 cases, especially among students. This programme will run
for three years only. This programme emphasises the health of schoolchildren because the
number of schoolchildren in Malaysia has almost reached 4 million. Hence, this programme is
designed specifically for the well-being of school children. Thus, this programme focuses on
nine states that have become red zone areas, COVID-19. The nine states are Penang,
Selangor, Melaka, Kuala Lumpur, Putrajaya, Labuan, Johor, Kelantan, and Sabah. It is because
the number of COVID-19 cases in Malaysia is increasing day by day until four figures were
recorded for several weeks, and more worryingly, on July 25, 2021, the total number of cases
recorded was 1,013,438. To curb the problem and reduce COVID-19 cases in red zone areas,
the Ministry of Health is implementing this COVID Test Kids programme.

           Thus, the COVID Test Kids programme is a collaboration programme with the Minister of
Education (MOE). This collaboration can help this programme go further and facilitate this
programme running smoothly because they have details and info on all schools in Malaysia.
Therefore, it helps in the process of giving COVID test kits stocks to every school in red zone
areas.

2.3.1 Activity of the programme

 Free COVID Test Kits

COVID test kits will be given to all schools located in red zone areas. There are 5,082
schools located in red zone areas, so the Ministry of Health will order about 10,162 nurses to
send stocks of COVID test kits to all schools safely. Two nurses will work in each school in the
process of providing the COVID test kit. The provision of this COVID test kit is given in stages in
each school so it can be carried out systematically and smoothly. Each symptomatic student will
receive a set of COVID test kits to achieve the objectives of the program. This programme will
facilitate the smooth process of schooling without thinking about the problem of the COVID-19
epidemic. However, standard operating procedures (SOPs) and social incarceration in schools

The Ministry of Health will send one doctor to each school to explain and brief students
on the importance and applications of COVID test kits. It is to give a clear view to all students
about the importance of COVID test kits nowadays. Other than that, it is to teach students to be
clever in using the COVID test kit. In fact, this learning can also be used for generations, which
means students can train their younger siblings how to use COVID test kits correctly. In short, it
can have many positive effects and benefits if the knowledge is used properly.

2.3.2 Function

The central function of this programme is to reduce and lower the number of COVID-19,
especially in the red zone area of Selangor, in line with the objectives of this program. This
programme emphasises the state of red zone areas because the cases there are getting higher
day by day. Consequently, the Ministry of Health decided to act by creating this kind of
programme. In addition, the superb cooperation of the Ministry of Education further facilitated
the work of implementing this programme. Moreover, parents who have children in red zone
areas can also feel calm when they know their children are safe and healthy to go to school. So,
indirectly, this programme can ease the burden borne by the people in red zone areas because
there is no reason to send their children to school. But the cooperation of all parties, including
parents and students, is also needed to run this programme smoothly.

            Other than that, the function of this programme is to gain more knowledge about the
uses and importance of COVID test kits. This knowledge can also be shared with others as it is
easy to apply in our daily lives. The COVID test kit is important nowadays as our country,
Malaysia, is being hit by the COVID-19 disaster right now. Thus, it can also be a good starter
programme that can have a positive impact on the country's economy and society. It can help in
reviving the country’s economy since COVID-19 cases are declining. When cases are lower and
more stable, various activities and premises can be reopened. So, this COVID Test Kids
programme can not only benefit red zone area students but all citizens as well, because it can
restore the country's economy with a decrease in COVID-19 cases. So, there are many benefits
and advantages that students will get from this COVID Test Kids program.

2.3.3 Target group

The COVID Test Kids programme was launched for the purpose of reducing COVID-19
cases among school students, but only focused on symptomatic students in red zone areas. It is
because those areas recorded the highest number of COVID-19 cases among all states in
Malaysia. Apart from that, the total number of schools located in red zone areas also almost
reached a thousand schools, which is 5,082 schools ( Ministry of Education Malaysia (MOE),
2021). Hence, the estimated number of school students in red zone areas is more than one
million. That is 10,164,000 students (List of All Secondary Schools in Each States in Malaysia,
2020). Therefore, the Ministry of Health is taking action to establish such a programme to
reduce the increasing number of COVID-19 cases.

4.0 JUSTIFICATION ON THE PROGRAMMES PLANNED

4.1 PRINCIPLES OUTCOME BASED BUDGETING SYSTEM (OBBS)

The Outcome-Based Budgeting System (OBBS) has eight types of principles. The first
principle is that planning and constructing the budget are based on a programme-based
approach. The first programme is the Clinic4U programme. The Ministry of Health needs to
identify and recognise places where it is difficult to obtain clinical supplies, mainly in rural areas.
In addition, we will provide affordable prices to needed people, such as rural residents. Its
purpose is to make them feel better about taking care of their own health. It can also assist us in
achieving our program's aims, which are to provide residents in rural areas with access to
clinical services, such as the COVID-19 swab test service, and to educate the public about
clinical supplies. The second programme is the Motherhood Guides programme. As for this
programme, we focus more on health issues for pregnant mothers. We will focus on all aspects
of the health that pregnant mothers experience naturally, including health and mental problems.
We know that pregnant mothers will face stress and emotional instability, so we strive to devise
a programme to alleviate their emotional difficulties. Furthermore, everything related to the
program's success is done with care. The third programme is the COVID Test Kids programme.
To launch this programme, we need to provide a very systematic framework so that everything
runs smoothly without any problems. Every entity involved in this programme is also taken
seriously as it is something new for our country. The Ministry of Health proposed giving a
focused emphasis to school students as the number of schools has almost reached a million.
So, every plan, such as the distribution of stock to each school and carried out at that stage, is
the best strategy to ensure the success of this programme.

The second principle is the roles and responsibilities of the Ministry of Finance
(MOF), the Economic Planning Unit (EPU), and the Public Service Department (PSD) to
examine the ministry budget. The budget given to each minister will be carefully examined by
the Ministry of Finance (MOF). Meanwhile, the Economic Planning Unit (EPU) and the Public
Service Department (PSD) are to ensure that all ministers receive an adequate budget based
on the programmes and plans they produce. The budget must be thoroughly examined and
scrutinised in order to avoid undesirable outcomes such as waste due to an excessive budget
amount or a lack of budget and insufficient funds. If something undesirable happens, it will not
only affect the implementation of the programme, but it will also affect the economy of our

country. The funds provided to the Ministry of Health will be put to good use with execution that
has a beneficial impact on the population, particularly during this COVID19 season. The funding
employed is not only focused on the COVID-19 victims but also all Malaysians, such as
expectant women, schoolchildren, and so on. Clinic4U is one of the programmes implemented
to provide many facilities to the people, especially to the rural population. It is because the
Clinic4U programme focuses on the health welfare of the rural population. As we know, rural
people face problems getting medicine from clinics, so we help them solve their problems by
building clinics close to their areas. This program's implementation has a lot of positive effects
on the rural population because it helps them get health supplies competently and conveniently.
Next, the Motherhood Guides programme is also a programme established by the Ministry of
Health to assist all pregnant mothers in maintaining their health, emotions, etc. This programme
is the first established in Malaysia and it will have a huge impact on pregnant mothers as it has
a variety of healthy activities that they can follow. Not only that, but this programme can also
help pregnant mothers learn new techniques for keeping their bodies healthy. In fact, it also
makes it easier for all pregnant mothers to give birth smoothly. Apart from that, the programme
also aims to teach pregnant mothers a healthy lifestyle and the right way to educate their
children in the future. So, it not only focuses on the current period of pregnancy but also gives
guidance to all the mothers regarding childcare. The third programme is the COVID Test Kids
programme. This programme is very helpful in raising the country's economy. It is because the
programme focuses on schoolchildren who are in the red zone COVID-19 area. So, the purpose
of this programme is to reduce COVID-19 cases among schoolchildren. As we know, the
number of schoolchildren is almost a thousand, so the Health Ministry will supply COVID test
kits to all schools, particularly in the red zone. So, this programme can help in various aspects,
namely economic and social aspects.

The third principle is that the ministry’s programme must be aligned with the National
Result Framework (NRF). All programmes created by the Ministry of Health must be
contributed efficiently to the NRF. All programmes must make a positive contribution and impact
on the people and the country. The first program, Clinic4U, was created to help rural people get
access to medical sources. It also saves time and energy for the rural population in the process
of sourcing medicine from the clinic, as the health ministry will set up clinics in every district
across the country so that the rural population does not have to travel to the city just to get
medical services. In addition, the programme also helps COVID-19 victims from rural areas with
the best COVID-19 swab services and medical treatment services from doctors. The second
programme, the Motherhood Guides programme, also helps in overcoming the social

problems experienced by pregnant mothers. It is because this programme prioritises all aspects
pertaining to pregnant mothers. As we know, the emotions of pregnant mothers are very
sensitive. Therefore, the existence of this programme can help pregnant mothers control their
emotions well and keep their bodies healthy. If this programme is successful, the number of
cases involving expectant mothers will be reduced, and even social problems in our country will
be reduced. The third programme, which is the COVID Test Kids programme, was also an
initiative created by the Ministry of Health to rebuild the country's economy. This is because the
goal of this programme is to minimise the number of COVID-19 cases, particularly among
schoolchildren. If COVID-19 cases are reduced, various premises can be opened as usual, as
can many infrastructures. Indirectly, the country's economy will also develop due to the
existence of flexible currency exchange, a favourable labour market, and so on. Therefore, this
programme not only has a positive impact on schoolchildren but can also develop the country's
economy.

The fourth principles are medium-term perspectives in planning and budgeting. This
principle needs to be considered so that it corresponds to the time allotted to achieve the
outcome of the programme successfully. It marks the direction of the policy and any changes in
the funding of a programme. It also helps identify possible budgets allocated for new and
existing programmes ( Government at a Glance 2009, 2009). For the first programme, Clinic4U,
the Ministry of Health has done a bit of research on rural areas and has considered building
clinics in every district in rural areas before starting the programme. The initial research
conducted by the Ministry of Health is intended as a precautionary and wise measure because it
can prevent the occurrence of problems such as a lack of budget or excessive budget. The
Ministry of Health has also set a target for the number of clinics to be built to draught the budget
process efficiently. All these steps can help in achieving the goals and objectives of the
programme easily. Next, for the Motherhood Guides programme, the health ministry has set a
limit for the number of people who can follow the programme properly. It is because it can help
in the budget process that will be used in realising this programme well. Apart from that, the
programme can also run smoothly because every detail and information have been taken into
account by the Ministry of Health. This action also aims to provide the best service to pregnant
mothers who follow this programme. At the same time, they managed to achieve the objective
of the programme, which was to alleviate the stress they bore. Finally, as for the COVID Test
Kids programme, the Ministry of Health has supplied 1000 COVID Test Kids only to each
school in the COVID-19 red zone. The health ministry has also instructed 2 nurses to work in
the process of giving COVID tests to symptomatic students. In this way, the COVID Test Kids

can run smoothly, and all procedures will run smoothly. The effectiveness of this programme
can also contribute to the national economy and not just to school students. So, such systematic
measures need to be taken care of by all parties, including teachers, parents, and
schoolchildren.

Next, the fifth principle of OBBS is strengthened performance management, which


means when the organisation has strengthened their performance management, then it is easy
for some programmes to decide based on valid evidence. Firstly, the programme Clinic4U is a
programme that has been recreated to focus more on rural areas. For this program, each
selected area will be accompanied by a sufficient number of skilled and sufficient staff to carry
out their duties there. This is because the programme will be focusing more on giving service on
the swab test and exposure to the medications and other equipment supplied. This item is
necessary to focus on rural areas because there are still those who do not know about some
things, such as where to get the service. In fact, there are areas far enough from the city for
them to get the service. Therefore, the services offered by us are very helpful to them in dealing
with health issues, especially COVID-19. So, the strengthened performance management for
this programme is to make sure that we can provide the best service for them so that it will help
those who find it difficult to get this service, like other people living in urban areas. The second
programme is Motherhood Guides, which is improved performance management. This is when
we can make sure that all the activities in the programme are well organized. For instance, all
essential items required by the programme are completed before the activity takes place. This is
because we want to avoid any problems that may arise that would slow down an activity. Thus,
we also need to make sure that every activity carried out should be appropriate for the
circumstances of the pregnant mother and the parents present there. The third programme is
called COVID Test Kids, where the management needs to focus more on the kids who are their
main target because it involves children who are still in school. They usually know less about
health. Therefore, we need to be careful in providing information and test kits to school
students. We need to ensure that the items will be given according to the set plan. This is
because we do not want any students to miss out on giving treatment because of a lack of
proper equipment.

In addition, the sixth principle of OBBS is the empowering of the controlling officer,
which is the power the controlling officer must have over each of the officers in the programme.
They have the right and responsibility to decide what is best for the programme, as it still
benefits the whole programme. However, the three programmes that were mentioned before

have an officer that controls the programme to make sure that all the programmes run as
planned. Even the allocated budget should be used according to the settings that have been
set. This is because we don’t want problems that can slow down any programme from running
at the right time. Furthermore, they need to be wise in managing the money that has been
entrusted to them because they want to avoid any problems such as wastage of insignificant
resources and so on. In fact, all those who are directly or indirectly involved must also be
responsible for their duties so that the programme can be successful until the end of the
programme.

Last but not least, the seventh principle of OBBS is incentives for results, which
means, as we all know, for someone to keep their performance level every day, they need to be
at least appreciated by the officers that hire them. Firstly, in Clinic4U, the doctor is being paid at
least RM10,000 a month, which is the logic for someone who is in a professional position to
have that salary, and the nurse and clinical assistant are being paid RM5000 and RM3000 a
month. Besides, in Motherhood Guides, they are also paid, which is for yoga instructors
RM10,000, parental coaches RM5000, and the staff for both programmes, each of them gets at
least RM2,000. With the salary given to each of these programmes, it can keep an employee
from continuing in this job. This is because it is worth the job they take on. However, we will
make sure that their salaries are given in accordance with the stipulated agreement and on time
to avoid unintended problems arising. Besides, during the programme, the staff will get food
allowance as a sign of appreciation to them for helping during the programme to ensure the
success of the programme until the end. This may help them be more motivated to work for the
day to come. Finally, the COVID Test Kids programme also provides safety equipment for their
staff, i.e., they provide PPE for each of the staff. It is important for them to always be provided
with safety equipment because it motivates them to work hard as they know the government will
protect their employees. Besides, the staff who are involved in this programme will get an
allowance of RM200 each, and RM150 for the delivery staff.

Lastly, the principles of OBBS are continuous development of ability. This principle is
very important to ensure that the programme will continue based on the plans that have been
set. However, for the Clinic4U programme, the maintenance building and renovation need to be
always monitored. This is because provisions have also been made to ensure the comfort of all
parties is at a high level. Therefore, it is important for us to ensure that the maintenance is done
and monitored as per agreement. Besides, the tax on medicine also needs to be focused on
and ensured that the tax on medicine is paid as mentioned before, to contribute to social

security and Medicare. Next, for the second programme, the Motherhood Guides, is a
programme whose development is to continue the food allowance and rental building for the
programme. This programme has been allocated a specific budget to ensure that all the money
is used as planned. Besides, the importance of these two things is that they will help the
programme run more smoothly while using the rental building. The ministry of health will also
ensure that each participant present will be given the treatment and comfort they deserve. This
is because the original purpose was to make them comfortable and happy after the end of the
program. Thus, for the last programme, which is the COVID Test Kids programme, the ministry
will ensure that all the priority equipment will be given as planned in the programme, for
example, PPE for the staff who will be dealing with kids to ensure both parties don’t get sick.
Besides, the ministry will ensure that all staff get their benefits, for instance, allowance, as a
sign of how much we appreciate them. As a result, the ministry will ensure that all the
programmes listed above are carried out as planned and that all parties involved benefit.

4.2 PROCESS OF OUTCOME BASED BUDGETING SYSTEM (OBBS)

There are four processes to implement outcome-based budgeting system OBBS which
are outcome-based planning, budget preparation, monitoring and evaluation and the last
process is reporting of results. To complete this budget report, there are only two processes
which are outcome-based planning and budget preparation. First process in this report is
outcome-based planning. Outcome based planning is a strategy or framework that sets inputs
and outputs to obtain that outcome. Then a combination of the agreed aspects can form the
Ministry Result Framework (MRF). Other than that, outcome-based planning makes it necessary
for the ministry to determine what activities that can be done that benefit the people. Through
outcome-based planning, the ministry can plan well to avoid wastage of financial and human
resources. This clearly shows that outcome-based planning is very concerned with the results to
be produced. Usually this is a 5 year plan (Portal Pekeliling Perbendaharaan Kementerian
Kewangan Malaysia, 2021).

The first programme that the Ministry of Health can implement is Clinic4U. The opening
of Clinic4U is more focused on rural areas. In other words, the purpose of opening Clinic4U is to
help rural people get the same health services that urban people get. Because of that, this
programme will collaborate with the Ministry of Rural Development. In line with the Clinic4U
concept, the Ministry of Health makes it easier for the public to buy medicines, and due to
COVID-19, the Ministry of Health is also focused on giving COVID-19 swab tests to rural
people. The Clinic4U programme will run for 3 years. There are 220 branches that will be
opened under Clinic4U programme. Then, during these 3 years, there are two activities that will
be conducted under this program, namely COVID-19 swab tests and 1 Week 1 Medicine
(1W1M).

Through the first activity, which is the COVID-19 swab test, the clinic will provide COVID-
19 test kits to any resident who feels symptoms of COVID-19. How to use the COVID test kit will
also be explained by the on-duty clinic staff. The main purpose of these activities is to detect
and break the COVID-19 chain in rural areas. This also lessens their burden without having to
go to urban areas. The second activity under the Clinic4U programme is 1 Week 1 Medicine
(1W1M). The main function of these activities is to increase understanding and prevent the
misuse of medicines sold in clinics. The clinic will use pamphlets, social media, and advertising
as alternatives to disseminate information about medicines that are in the clinic to the local

population. As a result of these two activities, we are able to reduce the COVID-19 chain and
maintain the rural population's health.

The second programme that the Ministry of Health can carry out is Motherhood
Guides. This is the first initiative programme by the government that focuses on the mental and
physical health of pregnant women state-wide. The Ministry of Health tries to raise awareness of
women's rights and needs. In addition, this programme will collaborate with the Ministry of
Women, Family and Community Development (KPWKM) due to its objectives which assist
women and families. This programme will run for 3 years. Then there will be two activities under
this programme, which are yoga class and parenting class. Through yoga classes, pregnant
mothers can relax their bodies and minds. Moreover, they can consult directly with the coach.
Classes held in the first and third weeks can also have a positive effect on the birth process. In
other words, yoga therapy does not burden pregnant women, and the results of this activity can
produce good results. Meanwhile, the parenting class aims to improve parents' knowledge and
skills in their children's education. Class that run for two hours in the second and fourth weeks
will be conducted by two certified coaches with ten assistants. Every week there will be another
coaching, so anyone who joins will benefit later and become a parent who understands the
child's future.

The third programme that the Ministry of Health can do is COVID Test Kids. The
Ministry of Health wants to reduce the number of COVID-19 cases among students. However, in
this programme, the Ministry of Health only focuses on students who are in the red zone. Due to
the focus of the programme being school students, this programme will be in collaboration with
the Ministry of Education (MOE). Then, the programme will run for 3 years. There are two
activities under this programme, which are the free COVID test for kids and a consultation with a
doctor. The aim of Free COVID Test Kids is to give COVID test kits to all students in the red
zone that have symptoms. The Ministry of Health will put two nurses in each school to regulate
the supply of COVID test kits. Those students who have symptoms will receive one set of
COVID test kits. Through this, we were able to break the COVID-19 chain in schools. The
second activity is a consultation with a doctor. The purpose of this activity is to provide
information as well as guidelines on COVID test kits. The Ministry of Health expects that
everything they learn in school will be applied, so the knowledge they gain about COVID test
kits will be shared either with family or friends. The Ministry of Health is sure that the outcome of
this programme will have a good effect for the good of all.

The second process in this report is budget preparation. Budget preparation can be
defined as a document that consists of a plan that collects and spends money over a period of
time. The beginning is with the Ministry of Finance publishing guidelines for preparing the year’s
planned expenditures estimated. Then, the policy, programme, or activity to be carried out must
be taken into consideration while estimating. In addition, budget preparation also considers
demand or wants as well as the cost of the products that must be purchased to carry out an
activity (Reed & Swain, 1997). Budget preparation is also to ensure that each allocation and
manpower are sufficient to make each programme achieve its objectives. In simple words, every
minister must prepare a proper budget plan to get the expected results. There are two methods
in budget preparation, which are the operational budget and the development budget.

Budget preparation for this programme is also the same as for other programmes. The
first programme is Clinic4U. The target of this programme is rural areas throughout Malaysia.
Due to that, the Ministry of Health must have proper budget planning. The operational budget
for this programme is RM22, 512,360,640.00 and the development budget is RM34,
078,000.00. The second programme is Motherhood Guides. The estimated operational budget
is RM27, 918,864.00 and the development budget is RM4, 147,000.00. There are fixed charges
under the operational budget and development budget. Both have fixed charges for food
allowance and rental buildings, which are RM1, 053,000.00 for the operational budget and
RM468, 000.00 for the development budget. The last programme is COVID Test Kids. This
budget allocation for this programme is from schools that are in the red zone, which includes
Penang, Selangor, Melaka, Kuala Lumpur, Putrajaya, Labuan, Johor, Kelantan and Sabah. The
operational budget is RM3, 557,196,720.00 and the development budget is RM4,
573,800,000.00. They also have fixed charges under the operational budget and development
budget. For the operational budget, there is a fixed charge for nurse allowance and an
allowance for COVID test kit delivery staff, which costs RM42, 688,800.00. The Ministry of
Health should properly prepare all the budget for the programme. With proper planning, it can
enable the Ministry of Finance to allocate resources for the programme. The Ministry of Health
refers to the financial and human resources aspects of implementing this programme.

5.0 CONCLUSION
In conclusion, these are all the programmes that the Ministry of Health prepares for 5-
year planning. All that is presented here is a plan that has been long thought of by the Ministry
of Health. In simple words, the budget that the Ministry of Health has presented above is
specific and detailed. All the programmes and activities will be beneficial to all the people in
these countries. The Ministry of Health is confident that the slogan "Teguh Kita, Menang
Bersama" can be used (Berita Harian Online, 2020). The Ministry of Health collaborates with
other ministers to further facilitate the programme. Not to mention the civil servants who are
eager to participate in any programme that is created. Following the desire for "no one left
behind," the Ministry of Health has made plans to hold a programme called Clinic4U. Through
this programme, people who are in rural areas will get benefits. Then 220 clinics will be opened
and operated for three years of programme and many staff will be placed. This programme
makes it easier for rural residents to get health services without having to travel to urban areas.

Furthermore, to uplift women in the country, the Ministry of Health also carries out a
programme called Motherhood Guides. The Ministry of Health provides yoga classes and
parenting classes for anyone who is pregnant. Through this service, anyone who attends the
programme can consult with the coach. Other than that, as COVID-19 cases are on the rise and
schools have opened, the Ministry of Health carries out a programme called COVID Test Kids.
Through this programme, the government can break the COVID-19 chain among school
students who are in the red zone. Cooperation with the Ministry of Education facilities all matters
related to the provision of information and the provision of COVID test kits to students. Last but
not least, referring to this aforementioned programme, we can see that the Ministry of Health
emphasises manpower and finance while making this budget. In short, the Ministry of Health
tries to avoid wastage of resources. Then, the Ministry of Health ensures that planning is done
carefully so that the results and outcome of the planning are as expected.
