    O Duly accomplished Accreditation Application Form


    O Scanned/ Photocopy of SEC Registration with of Articles of Incorporation or
    O Scanned/ photocopy BIR Certificate of Registration (Form 2303) or received BIR
    Application (Form 1901) if the former is still in process.
    O Photocopy of Official Receipt or BIR Application for Authority to Print Receipts and
    Invoices (Form 1906) if the former is still in process.
    Lead Broker's:
    O Recent 1x1 photo
    O Scanned/ photocopy of Real Estate Service License w/ certificate
    O Scanned/ photocopy of additional Government-issued ID
    O Scanned/ photocopy of notarized Secretary Certificate
    Corporate Secretary’s:
    O Recent 1x1 photo
    O Scanned/ photocopy of two Government-issued ID's

    Single Proprietorship
