Gratings Thematic Focus: background


Following the thematic focus no. 12 on accidents involving gratings recorded in all Eni Business Units in the period 2019-2022
and considering the LOST TIME INJURY recorded on the EDC48 on last March 2020 in Agiba, a cascade among contractors and
Agiba personnel has been arranged in order to ensure a continuous attention on the risks relevant to the gratings.
The most recurrent situations in Agiba are the following:
1) corrosion; 2) not proper fixing, or 3)open holes not provided with proper barriers.

Introduction

Electro welded metal grating, normally made of carbon steel,


galvanized steel, aluminum or stainless steel, is an indispensable tool in
a variety of industries including Oil & Gas. Used for covering trenches,
landings, worktops and industrial or safety ladders, it is used to reduce
the risk of slips and falls and can be used anywhere indoors or
outdoors. Its design allows for the drainage of liquids, mud or dirt and
increases grip in the event of water, ice, snow, oily substances or
detergents. Inaccurate installation or inadequate maintenance of
walkway or access gratings and their supports and locking hooks is a
major cause of a worker slipping and falling with the possibility of
serious or fatal injury. Falling can expose workers to other hazards
including drowning, noxious atmospheres and hazardous materials
(confined spaces, drains). In addition, there is the risk of an element
coming loose and hitting a worker.

Areas for improvement

Work direction and execution of the activity


It is forbidden to remove gratings without prior authorization; the removed gratings must be reassembled
and anchored at the end of the work in accordance with the regulations. During the period in which the
work is being carried out, the areas concerned must be protected and forbidden to pass through with
suitable fencing, appropriately marked.
Work environment
Assess the conditions of the working environment before starting the activity, also taking care that the
Gratingsis not a source of tripping or slipping

Inspection, controls and maintenance


In addition to the periodic inspections foreseen, sensitise personnel to pay more attention to and report any
anomalies found, even during routine rounds, such as positioning/alignment/deformation of gratings,
corrosion, holes, integrity of supports and clamping systems, missing or worn fastening clips on and
between panels. Checks should be carried out not only on metal structural elements but also on concrete
structures/supports that may be subject to oxidative deterioration or flaking

Design
In hostile environments, consider installing gratings with superior anti-slip properties to increase grip and
make them safer to use. Provide any gratings, which must be removed to access equipment/instruments,
with handles for easy and safe handling

Safety Alert BSEE 427 Gratingselement failure
On 10 January 2022, the US Bureau of Safety and Environmental
Enforcement also issued a Safety Notice in response to an
increase in accidents involving gratings.
Accidents are mainly the result of an incorrect assessment of
the state of integrity of gratings affected by wear and tear Company Technical Standard
(altered physical and chemical characteristics, corroded).
In 2020-21, there was an increase in near misses and accidents
involving gratings, including one fatal one. Civil works flooring and tiling
