AFFIDAVIT OF UNDERTAKING

WE, the members of the Board of Directors

_______________________________, a corporation duly organized and
existing under the laws of the Republic of the Philippines, with office address at
No. 78, Concepcion Street, Morning Breeze Subd., Caloocan City, after having
been sworn to in accordance with law hereby depose and state;

That by authority of the Board, we have applied for Registration as a

Job/Service Contractor/Sub-Contractor with the Department of Labor and
Employment, National Capital Region (DOLE-NCR), and for this purpose, we
hereby undertake that our company:

“Shall be primarily liable and responsible for all the salaries,

wages and benefits provided for by law as we further undertake to
comply with all the applicable laws and regulations of the
Department of Labor and Employment (DOLE) and all other
relevant regulations imposed by the Social Security System (SSS),
Bureau of Internal Revenue (BIR) and all other government
agencies having regulatory powers over Job/Service
Contractors/Sub-Contractors, etc.”

That in the event our company failed to comply with this undertaking, we
hereby acknowledge the rights of the Department of Labor and Employment
(DOLE) and other offices to impose the necessary sanctions against us,
including outright disqualification as a Job/Service Contractor/Sub-Contractor,
etc;
