Information of Incident :

Humyra sat alone in the corner of the class.

Explaination :

4 Magica were divided into two groups for dramatic play.Suddenly, Humyra didn’t participate the activity
and was seen sitting alone in the corner of the class.

Suggested Solution :

I went to Humyra and sat down next to her. We talked for awhile and she said she was shy and is not brave
enough to join the dramatic play. I encouraged and reassured her then give her some time until she felt ready
to participate the activity.

Name of Child : Haziq bin Hafizan

Age of Child : 3 years old

Information of Incident :

Haziq vomited during physical activity.

Explaination :

Today’s physical activity was dancing. The 1 Mutiara children were divided into four groups. In the
beginning, all children participated in the activity. Suddenly, Haziq from group 2 vomited after dancing. He
started to cry and he was warm indicating he has slight fever.

Suggested Solution :

I assisted Haziq to the toilet to clean up. He was advised to stay in the sickbay room while waiting for his
parents to pick him up. I contacted his mother through phone call with guidance from the principal.

Information of Incident :

Mohd Hafiz hit his toe on chair.

Explaination :

During lunch time, everyone was rushing outside for their lunch then suddenly Mohd Hafiz accidentally hit
his toe on chair while running because he didn’t saw it coming.

Suggested Solution :

I pick Mohd Hafiz up and immediately went to sickbay. I lay him down on the bed then rub ointment on his
toes then let him rest a bit.

Dhamira cried during physical activity.

Explaination :

During physical activity outside, everyone was participating in running activity except for Dhamira. She was
crying because she’s not feeling well.

Suggested Solution :

I bring Dhamira to the sickbay then let her rest a bit. I called her parents to pick her up.

Explaination :

Today’s language activity, children were being asked to sing to ‘If you’re happy and you know it clap your
hands’ song. Unfortunately, Muhd Farish doesn’t want to participate and was being silent the entire time.

Suggested Solution :

I went to Muhd Farish then asked him to dance instead. He then agreed and make some movements to the
song.

Ameera complained her friend stole her colour.

Explaination :

Today’s activity was art and creativity which children are bing asked to colour fruits. Ameera said her colour
is missing. She then complained her friend stole her colour.

Suggested Solution :

I went to her friend’s table and asked her if she had stolen Ameera’s colour and she said Ameera misplaced
the colour on her table. Ameera then felt calm after found her colour.

Nurul Ain forgot to bring her book.

Explaination :

Today’s language development activity was for children to write ABC. I asked the children to take out the
book I gave last time. Then I found out Nurul Ain didn’t bring her book.

Suggested Solution :

I gave Nurul Ain a piece of paper instead and asked her to write down ABC on the paper and bring them
home. Then I asked her to put the paper inside the book that has been given.

Explaination :

Today’s dramatic play Qhairah had a fight with her classmate because the wants to wear the costume that her
classmate had picked.

Suggested Solution :

I went to Qhairah and calm her down. Then, I asked her to take turns wearing the costume.

Information of Incident :
Mohd Nazhriem tripped on the floor.

Explaination :

We were in the bathroom washing hands after eating breakfast. Then suddenly Mohd Nazhrien played water
with his friends and the floor was slippery then he tripped on the floor.

Suggested Solution :

I pick him up and lay him down on the sick bay. I checked his body to see if there’s any bruises and found
out one on his ankle. I rub some ointment then called his parents to pick him up.

Iryani slapped her friend.


Explaination :

Today’s class, everyone was divided into three groups to create building blocks. Then, while playing blocks,
Iryani’s friend accidentally hit her with blocks. Iryani slapped her friend and they both got into an argument.

Suggested Solution :

I break up the fight and bring both of them outside the class. I talked to Iryani that the incident happened was
just a misunderstanding and her friend doesn’t mean to hit her. I asked both parties to apologise then they
reconciled.

Observation

I saw that Elyana was having some trouble shredding the papers with her fingers for the fruits collage during
the art and craft exercise. In addition, she made several attempts at sticking the torn papers onto the fruit
collage.

Action

Throughout the process, I helped Elyana tear and adhere the papers to the collage.
I offered Elyana to use during free play and demonstrated how to form it into a circle for her. Elyana was able
to form some playdough into a few different forms.

Observation

During storytelling activity, I saw Arif was having a hard time reading a book whereas his friends could read
them fluently.

Action

I helped Arif to read the book. We read together during storytelling.

During free play, I teach Arif personally on how to pronounce certain letters and we spell it together then
practice by reading books afterwards.

Observation

During playing blocks, I saw Maryam wasn’t communicating with her friends while building the blocks in
groups.
Action

I went to Maryam and encouraged her to speak up rather than keeping quiet. I asked some of the children to
interact with Maryam more so she doesn’t feel left out.
