    Good Day! We, the Kaysuyo Farmers Association were organized Last September 9, 2018 and
    made its first election by July 8, 2019. And we got registered at DOLE last October 10, 2019 with the
    DOLE Registry No. RO4A-CPO-WA-39-10-10-19.

    In lieu with this, we would like to request your office for an accreditation certificate that we will
    be using for the application of Sangguniang Bayan Accreditation and other legal purposes.
