MONTH-END ACCOMPLISHMENT REPORT


APRIL 2023

For this month of April, the following activities and programs have been performed by the
School Health Services Office:

• Continuous update of student health records by securing the health and developmental
history for all K, 1st graders, and new students and update returning student’s records
and special health plans.
 Monitored immunization compliance among learners and staff.
 Monitored levels of absenteeism related to illness.
 Reviewed individual student health records for completeness
 Coordinated school/community health activities and served as liaison between school,
home, community, and health care providers.
 Initiated an information campaign on November with the DOH Drug Abuse Prevention
and Control on the third week.
 Continued promotion of the optimal health status of children.
 Released 9th Health Bulletin regarding Heat-Related Illnesses in School.
 Submitted quarterly narrative report to principal regarding services rendered.
 Sent April newsletter for parents regarding the surge of heat-related symptoms in
school.
 Met with the advisers and reminded them to vigilantly monitor learners with flu-like
symptoms and encouraged them to stay at home to prevent cross-infection among
learners
