MONTH-END ACCOMPLISHMENT REPORT


SEPTEMBER 2022

For this month of September, the following activities and programs have been performed by
the School Health Services Office:

• Updated student health records by securing the health and developmental history for
all K, 1st graders, and new students and update returning student’s records and special
health plans.
• Prepared file with emergency information for students and staff.
• Notified teachers of known student health problems and any special procedures
required. Provided general staff education on cleanliness.
• Scheduled visits to classroom regarding health services and how to access care.
• Arranged meeting with advisers on classroom presentations on topics for their
homeroom such as:
o Hand washing
o Colds and flu
o Dental
o Sore throats
o Positive health practices

• Monitored students who are at risk of psychological/emotional abuse and make


referrals to the guidance counselor.
• Provided assessments and interventions for students with mental health concerns.
• Recommended provisions for a healthy school environment conducive to learning.
• Release 2nd health bulletin regarding Obesity prevention Week.
 Attended the Basic Life Support with AED Training by the Philippine Red Cross on
September 1-2, 2022.
 Prepared first aid kit for the Science Laboratory care of Teacher Whitnez Ebersole.
 Reminded class advisers to sanitize their respective classrooms every afternoon after
their last period.
 Ensured that Fridays are regular disinfection time in classrooms and offices.
