KNOW ALL MEN BY THESE PRESENTS:

I, TENIE R FLORES, of legal age, single and residing at Balagbag Cuenca Batangas,
after having been duly sworn to in accordance with law, hereby depose and state that:

1.I was employed as a Trailer driver of A.Macalindong Logistics Corporation.

2.That during my last work in the company I was asks for my schedule of activities until a
heated argument happened that causes my prolonged absences.

3.That the Company has sent me a notice to explain but I refused to submit my explanation
and that I waived my rights to be heard by the management which lead them to make a decision.

4.That my unauthorized Absent without Leave (AWOL) constitutes gross and habitual
neglect of duties.

5.That I hereby further acknowledge that I have been paid all my salaries and legal benefits
during my employment.

6.That I hereby acknowledged to my full and complete satisfaction, do hereby release and
forever discharge said Company, its officers and stockholders from any and all claims arising out
of and in connection with my Absent without leave (AWOL).
