RESOLUTION Resorting to the canvassing without PHILGEPS of Semi-expendable Medical Machinery equipment and
declaring the Lowest Bid for the Delivery of Medical Machinery Equipment (Thermal Scanner with Alcohol Dispenser)

BAC RESOLUTION NO. OO8, S. 2022

WHEREAS, Salvador C. Resol Nationasl High School needs of Semi-expendable Medical Machinery equipment
(Thermal Scanner with Alcohol Dispenser) for learners and staff use.

WHEREAS, Salvador C. Resol Nationasl High School agreed to canvass without Philgeps for the procurement of
Medical Machinery equipment (Thermal Scanner with Alcohol Dispenser) for learners and staff use.

WHEREAS, request for quotations were sent to Prospective and eligible bidders last November 23, 2022 and
appropriate documents for the conduct of bid were submitted to BAC.

WHEREAS, the BAC RECEIVED from 3 prospective and eligible bidders in response to the invitation after all the bids
were ranked and the BAC identified the Lowest Bid to be the supplier for the said project.

WHEREAS, upon evaluation of submitted documents of the lowest bidder namely, ROSEJIM’S PHARMACY from
Candoni Negros Occidental is legally capable to deliver the Medical Machinery equipment (Thermal Scanner with
Alcohol Dispenser) for learners and staff use.

NOW, THEREFORE, WE, the Bids and Awards Committee, hereby RESOLVED, as it is hereby RESOLVED, that the
amount of 4,500.00 will be spent for the Medical Machinery equipment (Thermal Scanner with Alcohol Dispenser)
for learners and staff use.

RESOLVED, at Salvador C. Resol National High School District of Candoni, Brgy. Gatuslao, Candoni Negros Occidental,
this 1ST day of December 2022.
