APPLICABLE TO FAMILY TAKAFUL NEW PROPOSAL PAYMENT

ACCIDENTAL DEATH COVER

When the Company receives a proposal together with deposit payment equivalent to the first
contribution, the Company will provide accidental death cover equivalent to the Sum Covered for
a maximum period of 90 days from the date of receipt and up to a maximum sum of RM500,000
for each person to be covered. The benefit is paid provided also that the proposal is acceptable by
us on standard rates.

If payment is made by cheque, the cheque must be honoured before such cover is given.

This coverage is temporary and will stop once the Company accepts or declines your proposal or if
the first contribution payment is refunded by the Company before 90 days from the date of
receipt. Where payment of benefit is made, the first contribution payment will not be refunded.

TERPAKAI UNTUK BAYARAN CADANGAN BARU KE ATAS TAKAFUL KELUARGA

PERLINDUNGAN KE ATAS KEMATIAN OLEH SEBAB KEMALANGAN

Apabila Syarikat menerima cadangan bersama-sama dengan bayaran deposit yang bersamaan
dengan caruman pertama, Syarikat akan memberikan perlindungan ke atas kematian oleh sebab
kemalangan bersamaan dengan Jumlah Dilindungi bagi tempoh maksimum 90 hari dari tarikh
resit, tertakluk pada jumlah maksimum RM500,000 bagi setiap orang yang akan dilindungi.
Manfaat ini akan dibayar sekiranya cadangan yang dibuat boleh diterima dengan kadar standard.

Sekiranya bayaran dibuat melalui cek, cek tersebut mestilah diluluskan sebelum perlindungan
tersebut diberikan.

Ini merupakan perlindungan sementara dan akan tamat sebaik sahaja Syarikat menerima atau
menolak cadangan tuan/puan atau sekiranya caruman pertama yang dibayar dipulangkan oleh
Syarikat dalam masa 90 hari dari tarikh resit. Sekiranya bayaran manfaat telah dibuat, caruman
pertama tidak akan dipulangkan.
