Risks and Safety Culture

1. Overview of Safety Culture (Process & Occupational)

2. The correlation between Risk, Risk control & Safety Culture

VU MINH HUNG

Definition and Elements

Culture is the characteristics and knowledge of a particular group of people, encompassing
language, religion, cuisine, social habits, music and arts.
- Culture encompasses religion, food, what we wear, how we wear it, our language, marriage, music, what we believe is right
or wrong, how we sit at the table, how we greet visitors, how we behave with loved ones, and a million other things"

The United Kingdom Health and Safety Executive defines safety culture as “... the product of
the individual and group values, attitudes, competencies and patterns of behavior that
determine the commitment to, and the style and proficiency of, an organization's health
and safety programs”.

Safety culture is the collection of the beliefs, perceptions and values that employees share in
relation to risks within an organization, such as a workplace or community. Safety culture is a part of
organizational culture, and has been described by the phrase "the way we do things around here".

Process Safety vs. Occupation/(Personal) Safety

Process safety includes Occupational Safety (or personal safety)
the prevention of generally refers to classic safety, health,
unintentional releases of and welfare of people at their workplace. It
chemicals, energy or other involved lower-level risk that are likely to

trips, slips and falls)
Process safety looks at
the bigger picture when it Occupational safety injuries come from a

injury prevention, rather and/or equipment – often seen as easily
than slips and trips, preventable with training and reminders
manual handling accidents people in the form of posters. It does not
or personal injuries from go beyond the workplace and deals with
chemicals or electrical things such as PPEs, noise exposure,
equipment. vehicle accidents, slips, trips, and falls.

Process Risks vs Occupational Risks

Process Risks Occupational Risks
• Low frequency but huge consequences • High frequency but limited consequence
• Related to the system • Related to individual
• Limited control by individual • Controlled by individual
• May affect employee, contractors & public • May affect employee and contractors
• May cause catastrophic property and • Not normally cause huge property and
environment damage environment damage

The above comparison demonstrates the major tendency within the operation of different
industries but not absolutely right in some specific cases.

Expectations toward Safety Culture

Know Your ü Be familiar with the potential hazards in your workplace
Area ü Follow all established safety requirements and procedures

ü If we see a safety issue, don’t assume that someone else will fix it
See It - Own It ü Escalate the issue to the proper owner to ensure the issue is corrected

Coach and Be ü If we see something that looks unsafe, ask questions and provide feedback
Coached ü Welcome coaching and feedback from others if it prevents an incident or injury

Pause and ü For new, unplanned or non-routine tasks

Plan ü Create a plan to identify hazards and the controls to reduce the risk

Stop the Job ü We are all empowered to stop the job when something changes or if you feel unsafe

Concept of Risk Control

Possible

Unsafe Unsafe Poor/weak

condition Act control process

- No rule or not - Safety awareness - No Std.

follow rules - Don’t care Safety - No re-enforcement
- No 5S, No RA - No safety rules or - No inspection/audit
- No clear process not follow rules - No disciplines

HUMAN ERROR

Todd Conklin: Human are “error-prone”, the organization can’t make their workplace safer and get to zero injuries and
fatalities simply by “fixing the worker” but “designing work” & corrective actions follow the “Hierarchy of Control”

Critical Safety Programs

Logout - Tagout
Work must not commence until all necessary control of hazardous energy has occurred
(LOTO)

Lifting & Hoisting Inspect vehicles, secure the load and drive per the speed limit

Working at Heights When working at heights, always use the fall protection devices and handrails

Electrical Safety Work must only be done by Qualified Personnel with proper PPE, tools and equipment

Confined Space Do not enter a confined space unless you have been trained and authorized

Equipment Safety Never by-pass, remove or modify safety devices or machine guards without proper authorization

Road Safety NO Drink & Drive. Always buckle up when driving & do not use your cell phone until safely parked

BP’s Refinery explosion and fire

Texas city, Texas, March 23, 2005 (15 killed, 180 injured)

1. BP Texas City lacked a reporting and learning culture. Reporting bad news was not
encouraged, and often Texas City managers did not effectively investigate incidents or
take appropriate corrective action.
2. BP Group lacked focus on controlling major hazard risk. BP management paid attention to,
measured, and rewarded personal safety rather than process safety.
3. BP Group and Texas City managers provided ineffective leadership and oversight. BP
management did not implement adequate safety oversight, provide needed human and
economic resources, or consistently model adherence to safety rules and procedures.
4. BP Group and Texas City did not effectively evaluate the safety implications of major
organizational, personnel, and policy changes.

SAFETY CULTURE creates a SAFE working environment. It
needs TIMES to build and needs EFFORTS from ALL people…

• Risk Management is a base of • PSM is a base of Process Safety

safe working environment (Tools: Bowtie, QRA, HAZOP, etc.)
• Risk Assessment is necessary • Right Process design is expected

RISKS are always created existing

Bow-Tie Method
The Bow-Tie method is a risk evaluation method that can be used to analyze and demonstrate causal
relationships in high risk scenarios. ...
A Bow-Tie diagram is a form of risk assessment to identify these potential hazards. It is a visualization of
the path a hazard may take “to cause a severe consequence and the combination of preventative and
mitigative barriers … required to reduce the process safety risk” (Vaughen & Bloch, 2016).

2. Risk Control Self-Assessment (RCSA) - to identify and assess the risks;
identify, map and assess the control
3. Scenario Analysis – mapping the scenario and assessing the impact
4. Key Risk Indicators (KRIs) – to identify and assess the quality of KRIs
5. Compliance – understand why compliance breaches occur and what
their impact could be
