We regret to inform you MR. VINCENT V. SOLANO that you are under preventive suspension
for fifteen (15) consecutive days from July 20, 2017 – August 4, 2017 due to your violation of our
Company Policy Article No XI Disciplinary Action for Violation Section 7 Offenses against
Management and Supervisory Personnel referring to:
In line with this, you cannot use your vacation leave credits in replacement to your suspension and in
every violation /offenses you make it will affect your periodic evaluation performance appraisal and shall
be reflected in your 201 files.
After the period of suspension you are hereby instructed to report to HR Department on August 5, 2017 to
submit your medical clearance for the continuity of your assignment.
