LEAVING NIGERIA
1. Get your plane ticket finalised

2. Get information on the updated rules in your country : example how many hours maximum should you
do the PCR test before your departure from Lagos, is there other documents you need ?

3. Based on the date/time of your flight, set the date/time for the PCR test.
Example the PCR test must now be done maximum 48 hours before departure. Say you fly from
Lagos on the 5th of February at 1 pm. You must do the test on the 3rd of February any time after 1
pm

4. Register online and fill the form on : https://lagosepid.com


a. Create an account if not yet done.
b. Click on “Request to be tested” and fill in the format.
c. Register your document and print

 Your LGA is : Badagry
 Next of kin : your wife or mother or father

5. Contact Mahesh and ask him to pay for your test.

6. Print the proof of payment

7. The day of the test at Lagos, bring your proof of payment, a copy of the Lagosepid form filled, a biro,
your passport

8. Print your PCR test result before leaving ASL

9. Read the specific requirements from your country as other documents may be required. For example,
for Indonesians in February 2022, get your EHAC card by filling a health delaration form, which cannot
be filled out manually. Passengers have 2 options to obtain the eHAC:
 The online form available on the Indonesian Ministry of Health website
 Downloading the eHAC Indonesia app on their phone, available for Android and iOS
Print the declaration or keep in your phone
COMING BACK TO NIGERIA
1. Read the Guidelines on the ncd nigerian international travel portal https://nitp.ncdc.gov.ng

2. Do a PCR test within the time (48 hours before departure)

3. When you get the negative PCR result, register yourself on the nigerian international travel portal ncdc

a) Load your PCR test at the end of the questionnaire
b) On the portal, when prompted to choose a lab, select « Lagos area » for your 2nd test. And V-
Care in Lagos.
c) Contact Mahesh by mail (msharma@primstar.com) or by whatsapp (+234 906 298 1065) ask
him to pay for your 2nd test. This test must be taken the day after arrival for all travellers
whether vaccinated or not.
d) Mahesh will then send to you a mail with a button to click on to get your QR code. You need
this to get on the plane.
e) Print the PCR test result and the QR code
