Dear Hospital and Laboratory Administrators/Pathologists/Water Testing facilities:

We are happy to announce that PAMET-Davao is now accepting training applicants for Water Microbiological Analyst,
Water Sampler and Drug Test Analyst to be conducted by the East Avenue Medical Center - National Reference
Laboratory with the following details

1. Proficiency Training Course on Microbiological Analysis of Drinking Water

Date: June 22-25, 2020 (Monday - Thursday)
Venue: San Pedro College, Guzman St., Davao City
Total registration fee: Php 8,000.00
Reservation fee: Php 3,000.00 (deductible from total registration fee)
Please bring lab coat/gown during the training.
Wearing of slippers, sleeveless, tattered pants & miniskirt are not allowed inside the campus.

2. Analyst Training Course on Manual of Operation for Screening Drug Testing Laboratories
Date: June 24-26, 2020 (Wednesday - Friday)
Venue: San Pedro College, Guzman St., Davao City
Total registration fee: Php 6,500.00
Reservation fee: Php 1,500.00 (deductible from total registration fee)
Wearing of slippers, sleeveless, tattered pants & miniskirt are not allowed inside the campus.

3. Training Course on Drinking Water Sample Collection (Water Sampler)

Date: June 26, 2020 (Friday)
Venue: San Pedro College, Guzman St., Davao City
Total registration fee: Php 2,200.00
Reservation fee: Php 1,000.00 (deductible from total registration fee)
Wearing of slippers, sleeveless, tattered pants & miniskirt are not allowed inside the campus.

Registration fee is inclusive of training manuals, lab materials, reagents, lunch and AM, PM snacks. Cancellation of
participation 30 days before the training or failure to appear during the training date forfeits payment made due to
preparations made and the decline of other interested participants in favor of the confirmed participant. Please note
that training slots are limited thus training applicants are required to pre-register with corresponding reservation fee

1. Inquire from secretariat for availability of slot. Secretariat will send you the Registration form

2. Fill-out the Registration form (fillable PDF file) and attach required document (i.e. PRC ID, PAM ID);
3. Pay total training fee or reservation fee and write participant’s name in front of the bank deposit slip;
4. Email the completely filled out Registration form, PRC ID/PAM ID, and deposit slip to the following secretariat
per training type:

Kindly bring the following on the 1 day of the training:
a. Original completely filled-out registration form;
b. Original copy of the deposit slip (with your name written on it)
c. Updated PRC ID or PAM ID

Please note that we only issue 1 official receipt per bank deposit slip. If you intend to get official receipt for your own
payment made, do not aggregate your payment with other participants in one deposit slip. We encourage you to
register as soon as possible as training slots are easily filled-out. For other inquiries, please reach the respective
contact person per training type. Deadline for registration is May 20, 2020 or earlier if slots are already filled out.
