Create its organizational chart. Then, explain the type of organizational system and structure
that you will be employing. Include also the name, logo, location and explain why you chose it.

ORGANIZATIONAL CHART

For the organization that I had made, it is runned by a rational system for the reason that
it is focused on formal structures as an organization who have individuals who aim together with
specific goals. Even though that organization has too many branches, yet it is focused on
attaining a certain goal which is providing a quality service together with the most optimal health
our patient could ever have after suffering from a serious injury and any health related
complications. Having this many branches provides us the efficiency along with the
effectiveness of our goal for the reason that having this many branches, we can take part in our
own responsibilities for the organization to be fully functional. The division of responsibilities
were evenly distributed and followed under each hierarchy in order to fulfill the job according to
the organization's goal. While the structural system it has the hierarchical structure due to the
division of the organization. On each level of the organization it suggests a certain specific
reporting relationship. It also has a certain degree of interdependence along with a level of
responsibility that needs to be performed.

For the logo that I have come up with, I have chosen this for the reason that it simply
represents the organization goal and objectives for its clients. The silhouette of a muscular man
represents the toughness you could attain in our organization along with the help and support of
our staff by encouraging you to do more, be more and have the optimal health you could ever
have. Along with the hearth logo and a cross sign symbol, it portrays the care our organization
offers with every health situation our clients have, we as professionals want to give the best
healthcare you could ever have.

For the location or our hospital, we are going to build it up in a rural area specifically here
at Taal Batangas for the reason that with this modern day, yet the citizens here still believe in
superstitions which serves as a barrier to them to have a more radical understanding about
health conditions. By this location we are able to break the stigma of this belief and be more
realistic in providing healthcare assistance in a broader community.
