hereby solemnly affirm and declare as under:
1. That my name as per the records my name is ANTHONY
2. By virtue of this affidavit, I state that the name Raj is my suffix name and the
same is given in my Family Smart Card.
3. I submit that my name is Anthony and my suffix name Raj
4. I submit that my Aadhar card is annexed herewith for the proof of the same.
5. Also, I state that Anthony and Raj is the name of the same person, and that is
myself.
I hereby state that whatever is stated hereinabove is true to the best of my
