Purpose & Scope: Philippines Staff can use this document to self-declare their COVID-19 vaccination status
(vaccinated or unvaccinated), exemption, as well as proof of vaccination, if any.

Step A: Indicating if Vaccinated or Not
Step Action to Take Screen Reference
1. From your Workday, click on
“View Profile”

2. Once inside your Profile,

click “Overview”

3. Once inside Overview,

go to
More > Additional Data

4. Click on Edit Button to
provide information

5. Are you vaccinated with any

of the COVID-19 doses?
Yes - Continue to next step

No - Go to step 9

6. Select YES option

7. Select the Vaccine

Manufacturer from the
dropdown menu for each
dose you want to declare
Select the Dose Vaccination
Date for each of them

8. Click on OK button to
submit the information

9. Select NO option if you are

Not Vaccinated (declaring
an exemption)
10. Select the Exemption

11. Click OK button to submit

Step B: Providing Documentation based on country requirements (can be done separately)
12. From your Workday Profile
menu, Click on Personal
13. Go to Documents Tab

Click on Add

14. Click on Select files to

attach your proof of
Vaccination document
OR: Exemption Document
(ex: Medical Certificate or
MedPass stating Medical
Reason for Vaccine ineligibility)

15. System will show you that

the document has been
attached

16. (If Vaccinated) Select from

Document Category one of
these options:
- COVID-19 First Dose
- COVID-19 Second Dose
- COVID-19 Third
Dose/Booster
Note: Upload photo of Vacc
document PER dose as proof
(even if it’s the same page)

17. Click OK button to submit.

Repeat steps for each dose.
