    A near miss incident occurred at a construction site in Taloja MIDC, Navi-Mumbai on June 25, 2020 at 12:45 pm. A transit mixer truck became stuck and tilted to one side when its wheel sank into uncompacted ground during concrete pouring work. The area was barricaded and the concrete was removed from the tilted truck. The soil and boulders under the wheel were cleared and the truck was driven in reverse to become unstuck. The root causes were identified as an undefined vehicle movement path, non-compaction of the ground, and lack of supervision. Immediate actions were taken to define vehicle paths and properly compact areas in the future.

    OBSERVATION / INITIAL CAUSE


    The UG tank slab concreting work was going on. There was a movement of transit miller for concrete pouring. One of the
    TM (MH 46 - BM-2216) was stuck and inclined one side due to wheel of the TM go down into the ground.

    IMMEDIATE ACTION
    1. Barricaded the area and movement of person immediate stopped
    2. Concrete from TM started get down with the help of tough rider till the empty of TM.
    3. Clear the soil and boulders beneath the wheel.
    4. TM was driven in reverse position and get free from stuck
