    hereby solemnly affirm and declare as follows : -

    (1) That I am a bonafide Indian citizen by birth and have a permanent residence at
    Ward No.5, P.O.- Barpeta Road, District - Barpeta, Assam.

    (2) That my commercial address is “Ashoka Enterprise”, L. N. Complex, Main


    Road, Ward No.5, Barpeta Road, Barpeta – 781315, Assam, India. My
    residence and commercial shop is situated in the same building .
