WAIVER OF RIGHTS

I, JEAN RACA ESQUEJO, of legal age, resident of Marietta Village, Brgy. Taloc, Bago City and wife
of the late BYRON B. ESQUEJO, who passed away on April 5, 2023, hereby endorse his
body/remains to Santuario De Bacolod for cremation service to be held on April 16, 2023, do
hereby acknowledge to give my full cooperation and waiver to the said company and states the
following:

1. I hereby certify that I am fully aware and have a full understanding that the facility’s
documents pertinent to the renewal of its licenses are still on process and that I waive
any legal responsibility and any form of accountability thereof should untoward
circumstances arise.

2. I hereby release and discharge Santuario de Bacolod and its stakeholders, officers,
directors, agents or employees from any action, damages, claims and demand
whatsoever, which in law or in equity , I ever had, now have, or which I, my heirs,
successors and assigns hereafter may have upon or by reasons of any matter, cause or
things whatsoever up to the time of this presents, the intention hereof being completely
and absolutely release the company, its successor- in-interest, stockholders, officers,
agents or employees from all liabilities arising wholly or partially directly from herewith.

3. I finally declare with full knowledge of my rights under the law, these documents and/or
waiver hereby given have been read by me and that I fully understand its contents and
imports and that I willingly and voluntarily executed the same.
