2.1.1. Bệnh thay đổi tối thiểu (thận hư nhiễm mỡ) Minimal change disease,
electron microscopy

This is minimal change disease (MCD) which is characterized by effacement of

By light microscopy, the glomerulus is normal with MCD.

Minimal change disease is a relatively benign disease and is the most
common cause of nephrotic syndrome in children. The ultrastructural feature of
the disease is the loss of foot processes of podocytes, while the glomeruli appear
completely normal on light microscopy. The age group most affected is 2-6
years old. The disease responds well to corticosteroid therapy.
2.1.2. Viêm cầu thận màng Membranous nephropathy microscopic
Viêm cầu thận màng hoặc viêm cầu thận ngoài màng là nguyên nhân chính của
hội chứng thận hư ở người lớn. Hầu hết các trường hợp là vô căn. Tuy nhiên,

Membranous glomerulonephritis or extramembranous glomerulonephritis is the
main cause of nephrotic syndrome in adults. Membranous GN is the most
common cause for nephrotic syndrome in adults. In most cases there is no
underlying condition present (idiopathic). However, some cases of membranous
GN can be linked to a chronic infectious disease such as hepatitis B, a
carcinoma
- Light microscopy description: Only thickened capillary walls seen in the form
of spikes and holes. No cell hyperplasia.
- Electron microscopy description: Granular deposition of IgG and C3 along the
vessel walls.

FSGS is characterized by focal (affecting some but not all glomeruli) and
segmental (< 100% glomerular tuft involvement with some residual patent
glomerular capillaries) sclerosis of glomeruli. Focal segmental
glomerulosclerosis is often accompanied by nephrotic syndrome. The disease
can be idiopathic or secondary to obesity, acquired renal atrophy, congenital
unilateral renal aplasia, sickle cell disease, recurrent congenital heart disease,
HIV infection, abuse of intravenous drugs.
2.1.4. Viêm cầu thận tăng sinh gian mạch lan tỏa –có hội chứng thận hư
Diffuse mesangial proliferative glomerulonephritis with nephrotic syndrome
Bệnh có đặc điểm là tăng sinh gian mạch lan tỏa kết hợp với hội chứng
thận hư trên lâm sàng.
The disease is characterized by diffuse mesangial proliferation associated with
clinical nephrotic syndrome.
- Hiển vi quang học: Thấy giãn rộng gian mạch, ít hoặc không có tổn
thương lòng mạch. Các khoang gian mạch chứa các tế bào đơn nhân (tế bào gian
mạch và bạch cầu đơn nhân) với 4 tế bào hoặc nhiều hơn trong một khoang gian
mạch. Tuy nhiên cũng có trường hợp tăng sinh khu trú hoặc cục bộ.
- Huỳnh quang miễn dịch: Viêm cầu thận tăng sinh gian mạch có hội

Light microscopy: Dilated mesangial spaces with little or no vascular
lumen damages. The mesangial spaces contain mononuclear cells (mesangial
cells and monocytes) with four or more cells in one mesangial space. However,
localized hyperplasia can be seen in some cases.
Immunofluorescence: Proliferative glomerulonephritis with nephrotic
syndrome often has IgM deposition with or without C3 deposition.

viêm họng, viêm da do nhiễm liên cầu. Ngoài ra, các loại vi khuẩn khác có thể
gặp gồm: Tụ cầu, phế cầu,...
- Cơ chế: Kháng thể gắn lên các kháng nguyên vi khuẩn bị bắt giữ bên
dưới tế bào nội mô cầu thận.
Acute post-infectious glomerulonephritis is often a diffusely proliferative
glomerulonephritis following an infection. Most commonly associated with
pharyngitis and dermatitis, caused by Streptococcus. Besdides, other bacteria
may include: Staphylococcus, Pneumococcus,...
Mechanism: Antibodies bind to bacterial antigens trapped beneath the
glomerular endothelial cells.
2.2.2 Viêm cầu thận cấp (tăng sinh nội mao mạch) sau nhiễm liên cầu.
Post streptococcal or acute glomerulonephritis
Hình thái học/Morphology:
- Kính hiển vi quang học: Tất cả các cầu thận đều bị tổn thương, hình
ảnh kinh điển là cầu thận giãn rộng và tăng sinh tế bào cầu thận gồm tăng
sinh cả cả tế bào nội mô, tế bào biểu mô và tế bào gian mao mạch.

- Light microscopy: All glomeruli are involved. Typically is dilated
glomeruli and glomerular cell proliferation (including endothelial cells,
epithelial cells, and mesangial cells).
- Swelling of endothelial cells (might obstruct capillary lumina) and
presence of neutrophils and mononuclear leucocytes. Slight infiltrate of
inflammatory cells and edema in the interstitium.

The hypercellularity of post-infectious glomerulonephritis is due to
increased numbers of epithelial, endothelial, and mesangial cells as well as
neutrophils in and around the glomerular capillary loops.

gặp ở người lớn.

Clinical relevance: Common in children aged 6 to 10 years old, less
likely in adults.
Occurs 1 to 4 weeks after strep infection of pharynx or skin.
Symptoms: Fatigue, fever, nausea, oliguria, hematuria, or dark urine.
- Facial edema (especially periorbital region), mild hypertension.

Pathogenesis: Associated with nephritogenic strains of Streptococcus
pyogenes (beta hemolytic Strep group A) with more than 90% of cases being
confirmed as types 1, 4, and 12. Post-streptococcal glomerulonephritis is an
immune-mediated disease. The latent time between infection and onset of renal
disease is consistent with the time required for antibody formation. Furthermore,
the granular immune deposits in the glomeruli demonstrate an immune complex-
mediated pathogenic mechanism, in which streptococcal antigen is responsible
for this complex.
2.2.3. Viêm cầu thận tiến triển nhanh ( viêm cầu thận liềm tế bào) Rapidly
progressive glomerulonephritis.
Seen here within three glomeruli are crescents ( ) composed of proliferating
epithelial cells. Crescentic GN is known as rapidly progres - sive
glomerulonephritis because this disease has a fulminant course. RPGN may be
idiopathic or may result from anti–glomerular basement membrane (anti-GBM)
antibody disease, such as Goodpasture syndrome (type 1); from im - mune
complex deposition with diseases such as systemic lupus erythematosus (SLE)
or postin - fectious GN (type 2); and from various types of vasculitis (type 3),
often “pauci-immune” forms. In the lower left glomerulus, the capillary loops
are markedly thickened (the so-called wire-loop lesion of lupus nephritis)

+ Hiển vi quang học: tuỳ thuộc vào nguyên nhân gây bệnh, tổn thương có
thể là hoại tử ổ, tăng sinh tế bào gian mạch và nội mô lan toả hoặc khu trú. Tuy
nhiên, hình ảnh mô học điển hình là những liềm tế bào (do tăng sinh tế bào biểu

+ Light microscopy: focal necrosis/diffuse or localized mesangial cell and
endothelial cell proliferation, depending on particular causes. The hallmark
feature is a cellular crescent (due to proliferation of parietal epithelial cells and
infiltration of monocytes and macrophages) that often obstructs Bowman's space
and compresses glomerular capillary loops. Over time, most cellular crescents
result in fibrosis.

2.2.5. Bệnh thận IgA IgA nephropathy
Đại cương/Overview
Bệnh thận IgA là viêm cầu thận gây ra bởi tích luỹ phức hợp miễn dịch
trong đó lắng đọng IgA chiếm ưu thế ở các vùng gian mạch. Bệnh chỉ chẩn đoán
xác định được bằng hoá mô miễn dịch và huỳnh quang miễn dịch
bệnh thận IgA chiếm khoảng 1/3 tổng số chẩn đoán sinh thiết thận và gần
một nửa của tất cả các bệnh cầu thận nguyên phát được chẩn đoán bằng sinh
thiết thận ở một số nước châu Á, bao gồm Nhật Bản, Trung Quốc và Singapore.
Ở Châu Âu và Úc, bệnh thận IgA chiếm 10% đến 20% chẩn đoán sinh thiết thận
và 20% đến 30% các bệnh cầu thận nguyên phát. Bệnh thận IgA đã được tìm

IgA nephropathy is glomerulonephritis caused by immune complex
deposition, in which IgA predominates in the mesangial regions. The disease
can merely be confirmed by immunohistochemistry and immunofluorescence.
IgA nephropathy accounts for approximately one-third of all renal biopsy
diagnoses and nearly half of all primary glomerular diseases diagnosed by renal
biopsy in several Asian countries including Japan, China, and Singapore. In
Europe and Australia, IgA nephropathy makes up 10% to 20% of renal biopsy
diagnoses and 20% to 30% of primary glomerular diseases. IgA nephropathy is
the most common primary glomerular disease in the United States among adults
aged 20 to 39 years, accounting for 14% of renal biopsy diagnoses in this age
group.
In some Asian countries, especially Japan, there are well-established
screening programs for the detection of urinary abnormalities in children and
young adults, with renal biopsies now being performed in people with persistent
microhematuria and/or persistent mild proteinuria.
Hình thái học/Morphology
In IgA nephropathy, there is abnormally glyco - sylated IgA1. Antiglycan
antibodies form and lead to immune complexes deposited within the mesangium
of the glomeruli. These complexes attach to fibronectin or type IV collagen in
the ex - tracellular matrix and activate mesangial cells to produce extracellular
matrix, leading to mesan - gial hypercellularity ( ), segmental glomeruloscle -
rosis or adhesion, tubular atrophy, and interstitial fibrosis. Some viruses and
bacteria express N-acetylgalactosamine on their cell surfaces so that infection
may promote antiglycan antibody formation; IgA nephropathy may initially
appear in association with an upper respiratory or gastro - intestinal infection.
IgA nephropathy has now become the most common form of GN

- Light microscopy: The glomeruli might be normal-appearing or with
dilated mesangial spaces and localized proliferation in some glomeruli (focal
proliferative glomerulonephritis). When cured, it can lead to focal sclerosis.
- Immunofluorescence: Typically is diffuse and very strong mesangial
deposition of IgA. Rarely, complement components such as C3, C1q, and C4
can be seen. In addition, capillary walls may also show IgA deposition.

2. Viêm cầu thận mạn Chronic glomerulonephritis, gross
Here is the default answer to the question regarding the cause of many cases
of CRF: Without a clear history or specific morphologic findings, call it
“chronic GN.” Seen here are atrophic kidneys with thin cortices from a
patient with CRF. About one third to one half of patients with CRF slowly
reach end-stage renal disease without significant signs or symptoms along
the way, and at the end stage, there are no diagnostic features, so there is no
point in performing a renal biopsy. Steadily increasing serum creatinine and
urea nitrogen are clues to this progression. Most patients also have
hypertension. Some incidental simple cysts (◀) are also seen here.
