Dear Gov. Padilla,

Greetings!

I am Rachelle D. San Miguel, 34 years old, from Brgy. Homestead, Bambang, Nueva Vizcaya.

I would like to draw your attention to the fact that it has been a really enriching experience to work in Nueva
Vizcaya Provincial Hospital and to be part of Nurse I (COS) employee. It has been a fruitful journey
throughout and I have learned from each of my assignments and from my teammates.

At this point, I would like to express my humble request to apply for a Casual Position as Nurse I at Nueva
Vizcaya Provincial Hospital.

I am a graduate of Bachelor of Science in Nursing at Saint Mary’s University and a Registered Nurse. I
have been serving in the hospital for 8 years and designated in different wards like Medical ward and Ob-
Nicu ward. At this time, I have been recognized as a dedicated, ambitious and reliable person who has the
ability to work without supervision. I’ve also equipped with my competitive skills that would allow me to
blend your mission and vision, thus, would propel to a new height of success.

I can assure you that I will do full justice to this designation as I truly understand the responsibilities that
come with this promotion.
