care items or other hazardous items shall be inaccessible to children.
CONCLUSION SUMMARY
The allegation that on May 14, 2019, a teacher unplugged a bottle warmer that was on top of a 1/2 wall and that Child A then
pulled on the cord of the bottle warmer, pulled the bottle warmer down on himself, and received second degree burns on Child
A's left arm is being substantiated.

The following rule was violated:

Rule 19 CSR 30-62.082 (1)(I) which states: "all flammable liquids, matches, cleaning supplies, poisonous materials, medicines,
alcoholic beverages, hazardous personal care items or other hazardous items shall be inaccessible to children."

After Child Care Facility Specialist (CCFS) Marla Chrisco conducted interviews with daycare staff , Parent A and observed the
video footage of the incident, it was determined that on May 14, 2019, staff Bela Norris-Weathersby unplugged the bottle warmer
that was sitting on the half wall between the infant and toddler rooms, leaving the cord hanging down accessible to the children
in the toddler room. Child A then went over and grabbed the cord causing the bottle warmer to fall on Child A . The hot water
caused second degree burns on Child A's right forearm and a spot on the back of Child A's head.
