II. Description of action taken by the school or division to alleviate the risk
factors of the CAR:
To alleviate the risk factors of the CAR, the school implements quarterly assessments
among students through an interview among their parents/guardians. This was done
every report card day. So far, no cases of children at-risk has been reported in the
school for the SY 2021-2022.
