JOINT AFFIDAVIT OF SPOUSES

(Legitimation through Subsequent Marriage)

We, ROLANDO AMANGAO and MARY JUNE U. WACAS, both of legal age,
husband and wife respectively, Filipinos and residents of Bulanao, Tabuk, Kalinga, after
having been duly sworn to in accordance with law, hereby depose and state:

1. That we are the legitimate parents of MA. THERESA SALOME W.

AMANGAO who was born on October 17, 2000 at Purok 1, Bulanao, Tabuk,
Kalinga;

2. That at the time the aforementioned child was conceived and born, we were
not married before the church or civilly but we were married in accordance
with the native custom on marriage;

3. That we are executing this affidavit for the purpose of legitimizing the status
of our child and it is our desire that said child be registered with the Office of
the Local Civil Registrar of Tabuk, Kalinga, and for this purpose, we are
submitting this document to the same office for their appropriate action;
