    REQUEST LETTER

    Dear Sir/Madam,

    I Victor John B. Laderas, currently employed with SSS no. 35-017-6682-4


    would like to request to your good office to transfer my contribution payment to my
    father Victor Laderas with SSS no. 03-7036707-5. The contribution money was not
    belongs to me.

    It is an honest mistake that my SSS Number 35-0176682-4 was paid on


    October 18, 2022 with Ref. No. C18040704170067 at SSS Valenzuela City, instead
    of my father’s voluntary contribution for the month of October-December 2022.

    We are truly sorry for this trouble that we make. Please guide us more.
