AFFIDAVIT OF SOLO PARENT

I, Arkiana Dimailig, a Filipino citizen of legal age, single, residing at Boac, Marinduque,
after being sworn to in accordance with law, depose and say that:

1. That I am a solo parent of three (3) minor children, namely: Wally Bayola, Allan Kei
Marasigan, and Angelina Jolie Dimailig and one (1) twenty-four (24) year old with a
physical disability, named Jose Manalo who are living with me and my parents at
our residential home in Antipolo City, and that;

2. I and the biological father of Jose Manalo was never married and had gone missing
for five (5) years and left no trace of his existence;

3. At twenty-five years old, I gave birth again with Wally Bayola on February 28, 2006.
I and Wally’s biological father was also never married due to an unfortunate
accident that led to his death on March 12, 2012.

4. On March 5, 2015, I gave birth to my son, Allan Kei Marasigan, at Dumantay,

Batangas and then again, I and Allan’s biological father never got married. He
suffered from acute liver disease that caused his death on September 1, 2018.

5. On February 26, 2023, I gave birth to Angelina Jolie Dimailig in Antipolo City. Due to
poverty and life difficulties, I was forced to resort to sexual activity in exchange for
payment, hence, I had multiple intercourses with several men, therefore, the
biological father of Angelina is yet to be accurately determined.

6. That due to the said events, I was left alone with the responsibility of providing for
the needs and support of my children.
 7. I am executing this affidavit to attest to the foregoing facts and for the purpose of
securing a Solo Parent ID and to receive the benefits under the Solo Parents’ Welfare
Act of 2000.

IN WITNESS WHEREOF, I have here unto set my hand this ____ day of ____________ in

9. securing a Solo Parent ID and to receive the benefits under the Solo Parents’ Welfare
10. Act of 2000

That due to the said circumstance, she was left alone with the
responsibility of providing for the needs and support of their child.
