AFFIDAVIT

I, Smt. Tilaka Ghosh, W/o Sri. Sajal Ghosh, aged about 45 years, by faith – Hindu, by
occupation – Self-employed, by Nationality – Indian, residing at 420, Taltala Road,
Motilal Colony, P.O. – Airport, P.S. – Baguihati, District North 24 Parganas, South Dum
Dum (M), West Bengal, Kolkata:- 700012, do hereby solemnly affirm and declare as
follows:-

1. That I now work for/in Naba Diganta Aya Center, at Sodepur, Indira Nagar, Block
– 3, Kolkata:- 700110.

2. Here I will be assigned as a maid or house maid for people.

3. My monthly salary will be Rs. 13,000.00/- (Rupees Thirteen Thousand Only)

which will be paid to Naba Diganta Aya Center and from there my salary will be
credited to me, either in cash or via any suitable mode of online payment within
the 5th of every succeeding months.

4. I have also completed my Police Verification from my regional Police Station which
is Airport Police Station, where there are no criminal or police records found against
me.

5. I will be liable to work here as a maid or house maid by abiding all the laws of the
State and Country, along with all the terms and conditions of Naba Diganta Aya
Center.

6. I will not be able to claim any extra amount of money either from Naba Diganta Aya
Center, or from the owner or his/her family members for whom I will be working, in
case of any kind of violation of that I will be responsible to return the very same to
the respective person.

7. I will also be liable to allow the owners and his/her family members to check my bags
while leaving their respective houses and to return them if any of their belongings
were found in my bags.

8. If I do violate any of these above mentioned conditions/clauses then my employment

in Naba Diganta Aya Center, will be subjected to immediate termination and no
objections will be entertained.

All the statements made above are true to the best of my knowledge and belief.
