Nurse: "Good morning Ms. Elsa, I am Ns. Eprilia would like to inform you that there is a new
patient Mrs. Parami with a diagnosis of post closed fracture left neck humerus sinistra from the
emergency room to the inpatient room, Cambodia room. I have received the patient and the
patient is currently in the inpatient room. Next we will carry out drug centralization"
Karu: "Well, thank you for the information, Sus. Is the medication administration format
complete?"

Nurse: "Already ma'am, this is the format"

Karu: "Yes, the drug administration format is complete. The medicine storage shelves are
available and if there are any problems with centralization facilities, please report to me."

Nurse: "Yes, thank you, then we will continue our work"

Karu:"Please sus"

Pre-interaction Phase

Orientation Phase

Morning at 7:30 am.

Nurse: "Good morning mom..." (smiling)

Patient: "Yes, good morning, ma'am." (smiling)


Nurse:"Let me introduce my self first, my name is Eprillia, I am the nurse who’ll take care of
you this morning, starting this morning I’ll take care of you from 7:00 am to 2:00 pm. May i
know what’s your name , and what you like to be called, Mrs? ".

Patient: "Yes, my name is Siti Khotijah, you can call me Siti".

Nurse: "Well Mrs. Siti, how’re you feeling now? What do you feel?”

Patient: "Since the accident yesterday, the wound on my knee is still a little bit bumpy."
(touching her knee and contemplating).

Nurse: (nodding her head) "Yes, mom, that is indeed the effect of the wound you have because
your wound has an inflammatory response."

Patient: "Is it dangerous?" (worried)

Nurse: "No Mrs, inflammation is a beneficial symptom and is a defense of the body that works
to neutralize and destroy injurious agents in preparation for wound healing. So you doesn't need
to be so worried." (explaining)

Patient: "Umm..I see." (slightly relieved)

Nurse: "Yes Mrs, well I'll excuse myself, please let her rest, I'll come back around 10:00 am to
do wound care, it won't take long mom about 5 minutes and we can do it here, is Mrs. Siti
willing?".

Patient "Yes." (nodding her head)

Nurse: "If you need my help, please call me, good morning." (smiling)

Patient: "Yes, good morning." (smiling)

Work Phase
Noon at 10:00 am.

Nurse: "Good afternoon Mrs. Siti." (smiling)

Patient: "Good afternoon." (smiling)

Nurse: "Mrs, according to the agreement that has been agreed upon earlier now I will carry out
wound care actions, are you willing?"

Patient: "Yes"

Nurse: "Okay, I will prepare the tools first." (goes outside the patient care room)

After the wound care process...

Nurse: "Mrs. Siti, I have finished taking care of your health. I wish you a speedy recovery."
(smiling)

Patient: "Yes, thank you." (smiling)

Nurse: "You're welcome, good afternoon."

After performing wound care, the nurse cleaned the tools and washed her hands.

The next day, at 07.00

Nurse: "Good morning mom." (smiling)


Patient: "Yes, good morning." (smiling)

Nurse: "How did you sleep last night, Mrs. Siti?"

Patient: "Alhamdulillah, last night I was able to sleep well, unlike the previous nights."

Nurse:: (nodding her head) "Maybe now your condition is better than the previous days here, so
it also has a good impact on your sleep pattern last night, is that right, Mrs. Siti?"

Patient: "Yes, I feel better now." (a little Patient smiles)

Nurse: "So what about the pain that you felt before?can you tell me about it?"

Patient: "Alhamdulillah, after the wound care that has been done for 2 days, my wound has
lightened a little, so I can sleep well without feeling pain in my knee." (trying to explain)

Nurse: (smiling) "Yes, it is also because of the help of Mrs. Siti who is always eager to heal,
and I hope that your current condition can also have a good impact on the healing of the wound
that Mrs. Siti is experiencing."

Patient: "Aamiin"

Nurse: "Oh yes, did you take a shower this morning?"

Patient: "Not yet." (shakes head)

Nurse: "Well, since you haven't bathed this morning, and it seems that you are not in a condition
to bathe yourself, I will bathe Mrs. Siti this morning, so that you feel more refreshed and you
can recover quickly. Let's do it here, it won't take long, about 20 minutes. How about it, are you
willing?"
Patient: "Yes."

Nurse: "Okay, I will prepare the tools first." (leaving patient's room)

After the bathing process....

Nurse: "How did you feel after the bath this morning? What do you feel Mrs?

Patient: (looks fresh, hair, and clothes look neat.) "i feels fresh and my body feels very clean."

Nurse: "Well Mrs. Siti, I have finished bathing you, for later in the afternoon or tomorrow
morning if you want to bathe you can do it like I did earlier, by asking for help from your
family, do you understand?" (with a questioning face)

Patient: "Yes."

Nurse: "Mrs, after this then I will do wound care to clean your wound again, are you willing?"

Patient: "Yes."

Nurse: "Okay mom, I will prepare the tools first." (goes out of the patient's room and back again
to do the treatment lulca)

Noon at 14:00

Termination Phase
Nurse: "Good afternoon Mrs. Siti." (smiling)

Patient: "Yes, good afternoon." (smiling)

Nurse: "How’re you Mrs?"

Patient: "Alhamdulillah, i feel better." (smiles slightly and took a seat)

Nurse: "Okay Mrs, because your condition is now improving and the wound that wound is in
the healing stage only, the doctor said today you can go home." (smiles)

Patient: (smiling) "Yes, ma'am."

Nurse: "Is there any celebration mom?"

Patient: "No, I just asked you for help to contact my family to pick me up this afternoon."

Nurse: "Alright, Mrs.siti I'll do it, is there anything else Mrs?"

Patient: "Oh yeah, thank you for your care so far."

Nurse:"Oo." (a little laugh) "You're welcome Mrs. Siti, this has become my obligation as a
nurse to treat and serve you as best as I can."

Patient: (smiling)

Nurse: "Goodbye Mrs. Siti, I'll excuse myself, hopefully at home Mrs. Siti can be back to health
and can return to her activities, good afternoon." (smiles and leaves the patient room)
Patient: "Yes, good afternoon."

The nurse meets the patient's family and informs them that the patient has been discharged.

Nurse: "Yes sir, the examination of your wife has been completed, now you can take the
medicine in the medicine room."

Family: "Okay, thank you, Sus"

After receiving directions from the nurse, the patient's family immediately walked to the
medicine room.

Family: "Excuse me"

Pharmacist: "Good morning, how may I help you?"

Family: "I would like to exchange this prescription"

Pharmacist: "Yes, sir, wait a moment, sir."

Then the pharmacist immediately prepared the medicine according to the prescription given

Pharmacist: "Excuse me, this is the medicine, sir"

Family: "Thank you, ma'am

The family headed back to the nurse station

Family: "Excuse me, this is the medicine that I have redeemed earlier"
Nurse: "Yes sir, I received the medicine, I'll check it first"

After a while PP checked the medicine....

Nurse: "Yes sir, I accept this medicine, sir, then you can sign this drug handover statement

Family: "I'll sign here, Sus"

Nurse: "Yes, sir"

The patient's family signs the medication handover statement format

Family: "This is sus, have I signed it?"

the patient's medication.

Nurse: "Nurse. Eprilia, please keep Mrs. Siti's medicine stored and organized in the patient's
medicine skirt and for now please give the medicine for the afternoon."
