16.01.2023 through RTI online portal. In this context reply of your query is
appended below:

Query 1: Certified eligibility criterion released by the bank to apply for the
UCO Bank Rupay Select Debit Card.

Reply 1: The cards can be issued in all UCO Bank saving accounts where age
of the customer is greater than 18 years.

Query 2: Certified Sfeps-cum-procedure (online and offline) to apply for fhe

UCO Bank Rupay Select Debit Card.

Reply 2: Customer con apply online for Rupay select Debit card through UCO
Mbanking app. In offline mode customer has to visit the branch and submit
offline request through Debit card request form for Rupay select Debit card.

Query 3: Information on Account Type eligible for UCO Bank Rupay Select
Debit Card.

Reply 3: The card can be issued in all UCO Bank saving accounts where age
of the customer is greater than 18 years and selected current account.

Query 4: Information on Minimum Balance requirements needed to get a

UCO Bank Rupay Select Debit Card.

Reply 4: There is no such minimum balance required to avail Rupay select

Debit card, however issuance/reissuance charges is applicable for the card
while request.

Query 5: Information on Joining Fees and Annual Card Renewal fees for UCO
Bonk Rupay Selecf Debit Card.

Reply 5: The applicable issuance/reissuance charges for Rupay select Debit

cards is Rs. 1000/- plus GST and Annual charges for the card is Rs. 900/- plus
GST

Query 6: Information on if any application format-cum-desired format is

available in the branch to apply for UCO Bank Rupay Select Debit Card.

Reply 6: There is standard debit card request form available in all UCO Bank
branches for the card request.

As such, your RT! application is disposed off.

However, in case you are not satisfied with the reply as above, you may file
an appeal before the l^t Appellate Authority of the Bank at the following
address within 30 days from the date of receipt of this letter.
