1.0 INTRODUCTION

1.1 Project Background

PETRONAS intends to appoint a competent CONTRACTOR to undertake Engineering,


Procurement, Construction and Commissioning (EPCC) of Effluent Treatment Plant (ETP) Service
Water Header Modification, henceforth referred as WORK.

1.2 Definition

PETRONAS means PRPC UF, PePSB, PETRONAS Technical Services, IRSO Southern, its
representatives and permitted assigns.

CONTRACTOR means a person or company who has been accepted by PETRONAS to undertake
the WORK and shall include their employees, representatives, and appointed subcontractors.

WORK means engineering design, procurement and supplies, manufacturing, construction, testing,
commissioning, and services as specified in the scope of works to be furnished, supplied, or
performed by CONTRACTOR for this project.

FACILITIY/FACILITIES means the equipment and its associated set up as specified in the scope of
works.

1.3 General Requirement

The WORK has to be executed in live plant and as consequence all WORKS shall be governed by
the PETRONAS Permit to Work system, details of which will be provided to the CONTRACTOR by
the PETRONAS or its representative(s). CONTRACTOR shall attend safety induction conducted by
PETRONAS Safety Officer.

CONTRACTOR shall, at any time wear basic Personnel Protective Equipment (PPE) while doing
work in the premises. Basic PPE outlined by PETRONAS HSE governance are as follows:

 Fire Retardant Coverall


 Safety Helmet
 Safety Boot
 Safety goggles/glasses
 Earmuffs / Earplugs

CONTRACTOR shall adhere to PIC park rules and entry requirement imposed by the PETRONAS
in relation to Covid-19 Standard Operating Procedure, required entry document, HSE induction,
vehicle entry rules, equipment inspection, etc.

Contractor shall ensure competent personnel for working at height and all personnel have attended
the WAH training.

Contractor shall ensure all personnel working at height are wearing full WAH PPE. All PPE and
lanyard shall be inspected by relevant authority at site.
Contractor shall ensure certified/competent crane operator, rigger, lifting supervisor and signal man
for lifting activities and ensure crane and lifting accessories are inspected by relevant authority at
site.

Contractor shall ensure competent fire watcher for hot work activity and ensure enough fire
extinguisher and fire blanket provided to complete the work.

If required to work extended hours or at night, Contractor shall ensure enough manpower for rotation
and sufficient lighting provided at night.

Contractor shall ensure competent driver for lorry, crane, forklift, trailer and other machineries and
ensure all machineries are inspected by relevant authority at site.

previous similar scale and magnitude EPCC project.

3. Project Control  5 years experiences in the role in oil/gas industry.


Engineer/Schedule  At least 3-years’ experience using Primavera P6
 Has been assigned as Project Control Engineer in at least
1 previous similar scale and magnitude EPCC project.

4. QA/QC Engineer  5 years experiences in the role in oil/gas industry.


 Has been assigned as QA Engineer in at least 1 previous
similar scale and magnitude EPCC project.
5. Piping/Welding Inspector  3 years experiences in quality control and inspection
experience for construction industries out of which 3 years
have been gained in Oil and Gas projects of similar scale
and magnitude to the EPCC project.
 Qualified Welding Inspector 3.1 CSWIP/ PCN Level II.

6. Welder  3 years experiences in welding for construction industries


out of which 3 years have been gained in Oil and Gas
projects of similar scale and magnitude to the EPCC project.
 Passed Welder Qualification Test (WQT) for above
materials.

8. Painting Inspector  3 years experiences in quality control and inspection
experience for construction industries out of which 3 years
have been gained in Oil and Gas projects of similar scale
and magnitude to the EPCC project.
 Qualified min IMM Level II or equivalent.
9. Instrument Inspector  3 years experiences in quality control and inspection
experience for construction industries out of which 3 years
have been gained in Oil and Gas projects of similar scale
and magnitude to the EPCC project.
 Qualified minimum diploma in related discipline.
10. Discipline  5 years experiences in the role in oil/gas industry.
Engineer (Process)  Has been assigned as Engineer in at least 5 previous
similar scale and magnitude EPCC project.
11. Discipline Engineer  5 years experiences in the role in oil/gas industry.
(Mechanical/Piping/Piping  Has been assigned as Engineer in at least 5 previous
Stress) similar scale and magnitude EPCC project.
12. Discipline Engineer  5 years experiences in the role in oil/gas industry.
(Instrument)  Has been assigned as Engineer in at least 5 previous
similar scale and magnitude EPCC project.
13. HSE Supervisor  3 years of HSE experience in major Onshore / Oil & Gas
Projects.
 The Department of Safety and Health (“DOSH”), Safety &
Health Officer (“SHO”) certification and registration, as
required under the Occupational Safety and Health (Safety
and Health Officer) Regulation of 1997 (or equivalent/
higher local or international HSE certification approved by
COMPANY).
14. Site Supervisor  3 years of Site Supervision experience in major Onshore /
Oil & Gas Projects.

CONTRACTOR shall hire third party Engineering Consultant or if utilizes in-house


engineering proficiency and competency, it shall be in condition having good reputation
that comes along with project track record with minimum of 10 years experiences
working in PETRONAS live plant (brown field) environment.

CONTRACTOR shall submit the name of Engineering Consultant during Technical

Evaluation. The selection of Engineering Consultant shall meet all the stipulated

requirements above. It shall be brought up to PETRONAS for approval prior any

commencement of engineering works.

2.0 CODES, STANDARD AND REGULATIONS

The design shall comply with PETRONAS Technical Standard (PTS) and the national, international,
and recognised industry standard and codes where applicable. For any deviations/conflicts from the
applicable rules, code and standards, the prevailing priorities will be according to the following
sequence:

1. National or local regulatory standards where the WORK is being performed and/or where
the FACILITIES are to be installed.
2. PETRONAS Technical Standards (PTS)
3. International Codes and Standards (API, ANSI, ISO etc.)

In case of conflict between the requirements and the referenced specifications or codes and
standards, CONTRACTOR shall bring the matter to PETRONAS attention for resolution and
approval in writing.

All deviation from the requirements, its attachment and the referenced specification, codes and
standards shall be stated in the proposal. In the absence of such a statement, full compliance will
be assumed.

Compliance by the CONTRACTOR with provisions of this document does not relieve the
responsibility to furnish equipment of a correct design for the specified service conditions and/or to
meet the requirement of local government.

The list of applicable PETRONAS Technical Standards and Code & Standards for this project are
as listed in Table 2.1 but not limited to.

3.0 PROJECT MANAGEMENT

CONTRACTOR shall supply adequate and competent labor, supervision, tools, construction
equipment, installed and consumable materials, services, testing devices and each item of expense
necessary for the engineering, supply, fabrication, handling, installation, construction, assembly,
testing and commissioning, and quality assurance of the WORK.

CONTRACTOR shall prepare Project Schedule with detail Work Breakdown Structure (WBS) as well
as CONTRACTOR organization chart within seven (7) days after receive Purchase Order (PO) /
Letter Of Award (LOA). CONTRACTOR shall follow clause 10.0 Schedule for all planning and
project control matters.

The WORK is to be executed in live plant, and all work shall be governed by PETRONAS Permit to
Work system. CONTRACTOR to attend safety induction conducted by PETRONAS Safety Officer
prior to commencing work. CONTRACTOR shall manage its personnel, safety, welfare and ensure
adherence to PETRONAS HSE regulations and requirement.

All materials and equipment supplied shall meet the design and environmental conditions as stated
in this document. All materials shall be new, sound, free of defects and of the size and quality
specified. CONTRACTOR shall note that all accessories should be supplied as complete assembled
unit.

All suggested changes to this document to improve the installation, operation or to affect economy
of fabrication at no sacrifice to the quality and safety of operation of the equipment shall be
considered. Where cost effective alternatives are offered, they shall be fully described and must be
accompanied by all pertinent engineering information to facilitate evaluation of alternative. It shall be
the responsibility of CONTRACTOR to inform or clarify with PETRONAS of any errors, ambiguities,
inconsistencies, or discrepancies which may be found to exist in any documents, specification or
drawings given by PETRONAS.

CONTRACTOR to use deviation form attached in Appendix 1 and seek PETRONAS approval.
Alternatively, Technical Clarification/ Query Form could be used to address alternative proposal
deem suitable by the CONTRACTOR. If there is no list of deviations submitted by the
CONTRACTOR along with the bid or during technical clarifications, it shall be assumed that the offer
is in full compliance with this document and its attachments.

The selection of materials, methods of fabrication, consumables and accessories not specifically
covered by this document but are necessary to complete the WORK shall be the responsibility of
the CONTRACTOR and shall be in accordance with good engineering practice.

The supply of equipment and other accessories as specified in the scope of works shall be off-the-
shelf and shall be avoiding any special or customized design. No prototype shall be offered, and the
equipment must have been field proven of operation worldwide and in this region with proven
performance record and relevant PETRONAS recommendation.

In case of delay caused by either CONTRACTOR or PETRONAS, remedial plan comprises of


revised project schedule and execution plan shall be provided.

Upon completion of project, CONTRACTOR shall submit Job Completion Certificate (JCC) for
approval by PETRONAS.

CONTRACTOR shall warrant and guarantee the design and construction to comply with
requirements stipulated by Petronas.

4.0 ENGINEERING SCOPE OF WORK

CONTRACTOR shall prepare all engineering deliverables required for proper and efficient
installation and operation of the FACILITY. CONTRACTOR shall submit Master Deliverable Register
(MDR) for PETRONAS’s review and approval.

The main scope of work shall cover the following but not limited to:

i. Process Scope of Work – refer Appendix 2


ii. Piping Scope of Work – refer to Appendix 3
iii. Instrument Scope of Work – refer to Appendix 4
iv. Civil / Structure Scope of Work – refer to Appendix 5

CONTRACTOR shall prepare a comprehensive set of drawings, calculation, data sheets,


specifications and reports and submitted to PETRONAS for review, comment, and approval.
PETRONAS will review and approve the documents/drawings based on the following code:

 Code 1: Rejected
 Code 2: Returned with Comment
 Code 3: Accepted without Comment
 Code 4: Accepted with Comment

CONTRACTOR shall produce Material Take Off (MTO) during engineering stage. All bulk material
and consumables deem required to complete the WORK shall be under CONTRACTOR scope of
supply.

CONTRACTOR shall develop Project Specific Procedure (PSP), which comprises among others,
Project Quality Plan (PQP), Project Execution Plan (PEP), Inspection Test Plan (ITP), Non-
Destructive Test Procedure (NDTP), Welding Procedure (WP), Leak Test Procedure (LTP), and
other procedure deem required to complete the WORK.

CONTRACTOR shall inform relevant engineering discipline upon submission of deliverables. When
requested, CONTRACTOR shall conduct document review session after submission of engineering
deliverables. COMPANY shall review and respond to documents and drawings submitted by
CONTRACTOR in a timely manner.

Prior to Final Acceptance (FA) of the WORK, CONTRACTOR shall amend and update all drawings
associated with the modification work to show the details of the FACILITY as it was constructed.
CONTRACTOR shall utilize Computer Aided Drafting (CAD) System for generating its drawings.
CONTRACTOR shall issue the As-Built Drawings in native file and PDF format as part of final
document submission.

CONTRACTOR shall ensure all engineering drawings and related engineering simulation/analysis,
results and reports to be approved by Professional Engineer with Practising Certificates (PEPC)
prior to release to Construction work. On top of that, as-built drawings are also required approval by
PEPC.

5.0 PROCUREMENT SCOPE OF WORK

CONTRACTOR shall provide all procurement services necessary to control, purchase, expedite,
inspect, test, transport, ship and deliver to site - all equipment materials, spare parts and any other
items required for the WORKS through to the final location at plant.

CONTRACTOR shall provide requisitions and underlying engineering documents, specifications,


drawings and term and conditions to obtain realistic quotations. The content for requisitions shall be
checked by CONTRACTOR for compliance with project requirement.

The costs of all CONTRACTOR procured goods and services shall include packing costs, export
permits, shipping costs, handling costs, port duties, sales tax and surtax of the country of origin,
import licenses, permits, custom clearances and costs of all kinds associated with procurement of
this Contract goods and services.

The delivery term of all equipment shall be per INCOTERM 2020 Delivery Duty Paid (DDP).

CONTRACTOR shall maximize the utilization of all equipment, supplies, materials, and services
available in Malaysia.

CONTRACTOR shall warrant that all equipment, supplies, materials and spare parts supplied shall
be new, unused, fit for purpose intended and shall conform to the requirement of the Contract.

CONTRACTOR shall comply with project requirement. If non-compliance is unavoidable,


CONTRACTOR shall communicate with PETRONAS for approval.

CONTRACTOR shall provide PETRONAS with copies of all unpriced purchase orders and purchase
order amendments.

CONTRACTOR shall pay all correct Vendor’s invoices by the due date. CONTRACTOR shall
maintain a register showing the status of all invoices issued by Vendors, in accordance with the
purchase order.

CONTRACTOR shall expedite all activities from the issue of a requisition until material or
equipment’s arrival on site, ensure that the order terms and delivery times are maintained.

CONTRACTOR shall perform quality control, inspection and testing for materials, equipment and
supplies delivered. CONTRACTOR shall issue Request for Material Receiving Inspection to
PETRONAS.

Manufacturer’s Data Records (MDRs), assembly drawings, operating manuals, packing slips, import
documents and etc. shall be compiled and submit to PETRONAS.

CONTRACTOR shall ensure appropriate protection for all materials, supplies, equipment, and spare
parts shipped to prevent damage and contamination by the elements.

6.0 CONSTRUCTION SCOPE OF WORK

CONTRACTOR shall execute all construction activities with high standards of quality assurance,
quality control and safety. Additionally, CONTRACTOR shall provide all necessary construction
equipment, the mobilization and control of the required labour force and the efficient and safe
construction management of the WORKS.

CONTRACTOR shall develop Inspection Test Plant (ITP) and submit to PETRONAS for approval
for the relevant Non-Destructive Testing (NDT) & Destructive Tests required such as Radiography
Test (RT), Hydrotest, Dye Penetrant Test, Ultrasonic Test (UT) and perform the tests after approval
from PETRONAS.

CONTRACTOR shall prepare Job Method Statement (JMS) and Job Hazard Analysis (JHA) for

CONTRACTOR shall prepare lifting plan for approval by PETRONAS.

Prior to execution of work at site, CONTRACTOR shall ensure all equipment, power supply such as
mobile genset, and construction materials are available, inspected by CONTRACTOR personnel
and PETRONAS, and stored properly at site.

CONTRACTOR shall ensure proper storage, and preservation of materials prior to construction.

CONTRACTOR shall perform cleaning, blasting, painting and coating work as per approved ITP.

CONTRACTOR shall perform tie-in work for the newly constructed FACILITY to existing facility
within the shutdown time allocated (to be confirmed by PETRONAS prior to tie in).

CONTRACTOR shall ensure best practice related to safe workplace, and proper housekeeping at
all times.

The CONTRACTOR is encouraged to appoint PETRONAS Vendor Development Program


(VDP) Scaffolding Contractor, i.e. Protection Rigging Access Services Sdn Bhd (PRASSB) as
subcontractor for the scope of scaffolding for the CONTRACT.

7.0 COMMISSIONING SCOPE OF WORK

CONTRACTOR shall be responsible for the execution of pre-commissioning, commissioning, and


start-up.

CONTRACTOR shall provide all necessary labour, tools/equipment, materials, consumables and
services during pre-commissioning, commissioning, and start-up.

CONTRACTOR shall prepare commissioning activity schedule as part of overall project schedule.

PETRONAS shall conduct Pre-Activity Safety Review (PASR) as per PETRONAS’s Process Safety
Management requirement. CONTRACTOR shall perform necessary action to resolve/close findings
raised from the PASR session.

CONTRACTOR shall prepare test pack for PETRONAS approval prior to pressure testing.

CONTRACTOR shall perform pressure testing of piping, supply testing equipment and consumables
inclusive of test blind and temporary gaskets.

CONTRACTOR shall perform blowing and drying of pipe upon completion of pressure testing.

CONTRACTOR shall perform reinstatement or preservation work upon completion of pressure


testing.

8.0 INSPECTION AND TEST REQUIREMENTS

CONTRACTOR shall develop detailed Inspection and Testing Plan for all piping activities, which
includes acceptance criteria for each activity and submit for COMPANY’s approval. CONTRACTOR
approved ITP shall form a basis for the WORK requirements.

Piping to be installed and workmanship shall be subjected to an inspection conducted at the


CONTRACTOR premises by the COMPANY’s representative(s) as per ITP. The COMPANY
inspector or authorized representative reserves the right to witness the shop tests and inspect in the
CONTRACTOR vendor’s plant. Any deficiencies or faults due to the negligence on the
CONTRACTOR behalf will be rectified at CONTRACTOR costs.

The test reports shall include the following as a minimum:


a) Visual and Dimensional Check
b) Hydrostatic Test
c) Post Weld Heat Treatment (PWHT)
d) Material Test and Traceability Reports
e) NDE reports

NDE test, if applicable, based on piping class shall include Magnetic Particle Examination, Liquid
Penetration Examination, Radiography Examination and Ultrasonic Examination.

CONTRACTOR shall have a good system to monitor and update the RFI Status. All the
RFI status shall be recorded/updated in daily basis and ready at anytime when
requested by OWNER.

The CONTRACTOR shall submit the Inspection reports within 24hours after inspection have
been accepted. Any delay event of submission reports will cause reinspection with
CONTRACTOR cost.

9.0 DOCUMENTATION

CONTRACTOR shall update drawings and related documents based on the works executed
such as P&ID, Isometric and System Description. The original documents will be provided by
PETRONAS to CONTRACTOR after Kick Off Meeting.

Document review cycle to be limited up to 3-cycle reviewing.

CONTRACTOR to track the comment via Comment Response Sheet (CRS).

CONTRACTOR shall submit the final documents to COMPANY in accordance with Appendix
6 – DOCUMENTATION PROCEDURE.

10.0 SCHEDULE

10.1 CONTRACTOR shall complete the overall work up to Final Documentation within stipulated
timeline in CONTRACT after Letter of Award (LOA) / Purchase Order (PO).

10.2 CONTRACTOR require to submit as per mentioned document below within one (1) week after
LOA / PO. CONTRACTOR to propose schedule based on normal working hours and based on
24hours, PETRONAS have the right to choose which working hours preferred. The documents
consist of:

i. Level 5 project schedule using software Primavera P6 (Mandatory)

ii. Propose Project Calendar inclusive public holiday in Primavera P6

iii. Master Deliverables List (MDL),

iv. Overall Project Schedule,

v. Project Execution Plan,

vi. Organization Chart and Manpower Plan,

vii. Progress Measurement, and Plan S-Curve

10.3 CONTRACTOR shall plan, monitor, and control the schedule and deliverables throughout the
execution to meet the CONTRACT duration. CONTRACTOR to perform mitigation plan in the
event there is potential delay more than two (2) weeks for the deliverables to complete within
stipulated timeline in CONTRACT

10.4 CONTRACTOR shall produce Project Progress Reporting daily and weekly for COMPANY.
Deliverables and Timeline of submission will be as table below or as per agreed with COMPANY.

10.4.1 Report shall consist of progress status actual against plan, highlights of the daily/week/month,
updated MDL, updated S-Curve, manhours plan against actual, productivity measurement, look
ahead activity, issues, and concerns.
10.4.2 CONTRACTOR shall complete, compile, and submit Project Closeout Report and Final Report
which include all the drawing, native file, and PDF file for all the deliverables upon completion
of the work within agreed duration by COMPANY.

10.4.3 CONTRACTOR shall propose the working hours required to meet PETRONAS duration of the

11.0 OBLIGATION OF CONTRACTOR

11.1 The CONTRACTOR shall execute all WORKS as stipulated in this CONTRACT
specification.

11.1.1 The CONTRACTOR shall submit resume or CV of the above key personnel in the
Unpriced Technical proposal and the COMPANY, or its authorized representative(s) will
interview the candidate, whenever it is technically required.
11.1.2 The Work Supervisor shall assume total responsibility of work performance during
execution and shall be the CONTRACTOR representative and focal person between the
COMPANY and the CONTRACTOR
11.1.3 The CONTRACTOR shall schedule, control, and monitor the WORKS closely to
accommodate the work schedule of the other COMPANY’s contractors and shall inform
both the COMPANY's and COMPANY’s contractor should there be any delay in the
WORKS that may and/or will impact their schedule. The CONTRACTOR shall be equally
liable and responsible for any delay in the WORKS. The CONTRACTOR shall provide
liaison and co-ordinate closely this portion of the WORKS with the other contractor as
per COMPANY instruction.
11.1.4 CONTRACTOR shall assist COMPANY during DOSH inspection activities. The activities
involve explaining to DOSH officer on maintenance service done.

11.2 Manpower, Equipment, Tools, Material and Consumables

11.2.1 The CONTRACTOR shall provide adequate numbers of skilled and competent
manpower according to agreed resource loading, complete with PPE requirement as per
COMPANY HSE Policy as per Exhibit V.
11.2.2 CONTRACTOR to manage and ensure manpower and equipment mobility to and from
origin to FACILITY. Any cost for associated management of RAPID gate access shall be
borne by CONTRACTOR.
11.2.3 The CONTRACTOR shall screen and provide resume to the COMPANY for all key
personnel. Basic requirement of CIDB shall apply to all manpower. Applicable
Competency certificate issued by regulatory bodies (e.g.: DOSH, NIOSH) is required for
the following position but not limited to:
 Engineer
 Work Supervisors
 Technicians

11.2.4 COMPANY shall reserve the right to perform mock-up test and/or conduct interview to all
personnel and manpower proposed by the CONTRACTOR. Additionally, COMPANY
shall also reserve the right to reject the said personnel and manpower in the event of
failing the mock-up test or interview conducted.
11.2.5 COMPANY may request any of the CONTRACTOR personnel to be replaced if their
performance is not satisfactory to the extent that the jobs’ safety, quality and schedule

could be affected. The CONTRACTOR shall bear any cost arises due to this
replacement.
11.2.6 The CONTRACTOR to provide equivalent competent manpower as replacement for
any event of resignation or termination. COMPANY shall reserve the right to impose
Penalties for any event of key personnel changes.
11.2.7 The CONTRACTOR shall propose the number of manpower to execute the service
WORKS for COMPANY evaluation.
11.2.8 Equipment for use at site shall be inspected and approved by the COMPANY personnel
as per the COMPANY’s equipment inspection procedure. Defective and/or unsafe
equipment shall not be used, and the CONTRACTOR shall arrange immediate
replacements at the CONTRACTOR’s cost.
11.2.9 In the event, power supply to be used for the execution of WORKS, CONTRACTOR
shall ensure a COMPANY’s approved portable Circuit Breaker device is to be used.
11.2.10 CONTRACTOR shall be responsible for all performance of the WORKS, observing and
exercising all engineering standard and practices, safety and health of all personnel
directly or indirectly involved in the WORKS.
11.2.11 The CONTRACTOR shall take immediate action to repair and replace for all damages
to any of the COMPANY General Facilities such as but not limited to fences, road,
drainage, drain cover, lamp post and etc. subjected to negligence, carelessness,
improper handling and misused by the CONTRACTOR’s personnel OR shall be back
charged to the CONTRACTOR at cost of equivalent to the original purchased price of
the said material.
11.2.12 CONTRACTOR shall be responsible for all performance of the WORKS, observing and
exercising all engineering standard and practices, safety and health of all personnel
directly or indirectly involved in the WORKS.
11.2.13 The CONTRACTOR must safeguard materials, equipment and/or other devices
provided or on loan by the COMPANY against loss or damage prior to, during and after
usage. The CONTRACTOR shall take immediate action to replace for any lost or
damage to the items provided by the COMPANY subjected to negligence,
carelessness, improper handling and misused by the CONTRACTOR personnel OR
shall be back charged to the CONTRACTOR at cost of equivalent to the original
purchased price of the said material.

11.3 Quality Assurance and Quality Control (QAQC)

11.3.1 The CONTRACTOR to comply with the COMPANY Quality Manual and PTS 19.60.01.
The procedure shall ensure that all stages of work are controlled and executed within the
agreed procedures, standards and good engineering practice in order to achieve
equipment reliability and to allow the plant to be operated reliably.
11.3.2 The WORKS or services to be performed shall meet the Job Package requirements. The
following shall serve as minimum guidelines for work execution:
 All equipment isolation points and equipment to be worked on will be identified
by the COMPANY. Lines to be cut out and/or modified must be positively
identified and confirmed in writing by the COMPANY.
 In order to ensure continuity in the WORKS, the CONTRACTOR personnel
shall be organized in teams assigned to a specific unit or project. The core

members of each team will remain in the assigned tasks until completion of
the WORKS. The assignment of the same member to more than one team at
any time is subject to the COMPANY or its representative approval.
 Personnel working in the plant must exercise caution in handling electrically
heat-traced lines and equipment to avoid damage to the system. Please note
heat tracing lines may be energized during the duration of the WORK unless
switched off and explicitly mentioned in the Permit To Work.

11.3.3 CONTRACTOR is not allowed to ride on his SUB-CONTRACTOR personnel


qualification i.e., Painting Inspector, Radiographic Interpreter and etc. CONTRACTOR
shall employ Qualified personnel accordance for their jobs.

11.3.4 CONTRACTOR shall develop:


a) Quality Objectives

b) Quality Communication Management i.e. Site Quality report

c) Site Quality Program

d) Develop or Validate Site Quality Manpower Plan

e) Develop Job Descriptions for Quality Positions

f) Identify Inspection Tools Required for Inspection

g) Site Quality Audit

11.3.5 CONTRACTOR shall actively initiate with own cost for the below activities:
a) Quality Promotion & Campaign (align with PETRONAS’s Campaign)

b) Quality Moment (for every meeting with Petronas)

c) Site Quality Program

11.4 HSE

11.4.1 The CONTRACTOR shall comply with all applicable with the host country's laws and
regulatory requirements, standards, industrial code of practices and guidelines which
are applicable to where the WORK is being carried out.

11.4.2 The CONTRACTOR shall always comply with the requirements as set forth by the
COMPANY in its PETRONAS Technical Standards (PTS), Manual and Guidelines on
Health, Safety and Environment Management System (HSEMS), policies, and its
operating procedures:

i. COMPANY HSE Management System (HSEMS) and its procedures.

ii. PETRONAS HSE Mandatory Control Framework.

iv. PETRONAS Zero Tolerance (ZeTo) Rules Guidelines.

11.4.3 In addition, CONTRACTOR to abide by any Security Rules and other Special Rules
which may be implemented and issued by COMPANY from time to time.

11.4.4 The CONTRACTOR shall develop, implement and maintain a Project HSE Plan
containing detailed procedures for the safe performance, monitoring and reporting from
engineering to demobilization activities as per required by PTS 18.03.03
(CONTRACTOR HSE MANAGEMENT).

11.4.5 The CONTRACTOR shall prepare and submit within thirty (30) days after EFFECTIVE
DATE; a PROJECT specific HSE Plan and a list of Project HSE Deliverables to
COMPANY which, as minimum describes the following (refer to PTS 18.00.01
(HEALTH, SAFETY AND ENVIRONMENT MANAGEMENT SYSTEM (HSEMS))

11.4.6 CONTRACTOR shall conduct Job Hazard Analysis (JHA) to identify the possible safety
hazards and ensure that every corrective action is taken prior to commencing work.
Draft JHA to be submitted in proposal submission.

11.4.7 CONTRACTOR shall produce and/or update the following deliverables, for
COMPANY’s information, review and/or approval as a minimum:

i. Project HSE Plan


ii. Week HSE Reporting

11.4.8 CONTRACTOR shall complete below requirement & training to be allowed to work at
SITE. The cost shall be borne by CONRACTOR:

a) NIOSH Oil & Gas Safety Passport OGSP (all workers)


b) PTW Training by MOCA STS (for Work leaders)
c) PRPC UF PTW Training (Work leaders)
d) PIC & PRPC UF Safety Induction (all workers)
a. CONTRACTOR shall submit to COMPANY following document with

ii. Copy Passport with valid working Permit
iii. Medical Certificate approved by PETRONAS Authorized Medical

11.4.9 The CONTRACTOR shall be responsible for maintaining and enhancing the HSE
communication & awareness among its personnel and SUB-CONTRACTOR’s
personnel, including arranging safety induction, toolbox meetings, regular HSE
meetings and emergency exercises and drills.

11.4.10 The CONTRACTOR shall provide adequate training to all its personnel to ensure that
they have sufficient knowledge and skill to undertake assigned tasks. The
CONTRACTOR’s training records shall be maintained and controlled

11.4.11 Prior to performance of WORK, the CONTRACTOR shall implement JHA as a safety
precaution to eliminate or guard against hazards on a specified job.

11.4.12 The CONTRACTOR shall comply with PTS 18.06.04 (HSE PERFORMANCE,
MONITORING AND REPORTING), and the COMPANY’s requirement. The HSE target
shall be communicated to all level of personnel throughout the project.

11.5 Housekeeping

11.5.1 The CONTRACTOR shall always ensure proper and timely housekeeping of the
workplace / area. General guidelines for housekeeping are and not limited to:
11.5.2 Housekeeping must be performed regularly at end of the working day or completion of
Work. Should the condition be unsatisfactory, the COMPANY has the right to stop all
on-going work to facilitate total housekeeping work.
11.5.3 Materials, parts of equipment removed or disassembled for inspection/repair must be
stored and stacked neatly and safely.
11.5.4 CONTRACTOR is required to segregate waste into the following categories and placed
into separate containers provided by the COMPANY’s appointed CONTRACTOR:
 Industrial waste
 Domestic waste
 Scheduled waste

11.5.5 All types of waste shall be disposed of in compliance with the COMPANY Waste
Management requirement or procedures.
11.5.6 Surplus materials are to be returned to the COMPANY

12.0 OBLIGATION OF COMPANY

12.1 The following are the responsibilities of the COMPANY, unless otherwise specified later:

12.1.1 Safety briefing will be conducted by COMPANY


12.1.2 DOSH payment for CF renewal will be borne by COMPANY.
12.1.3 Issuance of complete and approved Job Package according to specific work requirement
such as WORK during outage etc.
12.1.4 Supply of permanent spare parts (gasket), bolts and nuts, when necessary.
12.1.5 Supply of scaffolding for working platform in timely manner in enduring CONTRACTOR
to perform the services within the allocated number of days.
12.1.6 Identify and allocate a dedicated area for the execution of WORKS offsite such as in
workshop, laboratory etc. Under no obligation that the COMPANY to provide any office
accessories or other administrative services to the CONTRACTOR.

14.0 WARRANTY

CONTRACTOR shall be fully responsible for the manufacture in respect of proper design,
quality, workmanship and operation of the equipment, accessories etc. including items supplied
by sub-vendors.

CONTRACTOR to refer to ARTICLE 2.1.52 in General Terms and Conditions (GTC) for further
explanation on the Warranty Period.
