service: Pre-authorization procedures:
If you know in advance that you are planning to be admitted to hospital on either an in-patient or day-
+44 (0) 1475 788 182 care basis, you must provide a complete Treatment Guarantee Form at least 2 working days before
incurring any such expenses.
In a medical emergency you will need to contact Cigna within 48 hours of admission on the number
on your insurance membership card. With the following information:
1. Your full name and date of birth, and
2. Your membership number, which can be found on the front of your membership card.
Cigna will liaise with the hospital for a cost estimate and details of what medical treatment is to
be carried out. Where eligible, an agreement will be put in place with the hospital to pay the bill
on your behalf.

Important:
• Failure to obtain pre-authorization will result in 20% loss of claim if the treatment was received
outside USA; if the treatment was in USA, the loss will be 50%.
