AFFIDAVIT OF LOSS
I, GIOVANNI TAMBAHOYOT, of legal age, married, Filipino, and a resident of
Balugo, Dumaguete City, after having been sworn to in accordance with law, do hereby depose
and state THAT:

1. I am the registered holder of the following Identification Cards:

a. Driver’s License – issued by the Land Transportation Office (LTO) – Dumaguete

City Field Office;
b. Unified Multipurpose Identification Card (UMID) – issued by the Social
Security Services;
c. Voter’s Identification Card – issued by the Commission on Elections
(COMELEC);

2. The abovementioned Identification Cards were all placed in my wallet;

3. On September 03, 2020, while I was driving my motorcycle, I must have dropped the
said wallet on the road because when I reached home, it was not inside my pocket
anymore;

4. I, together with my wife, exerted all efforts to locate the said wallet but they all proved
futile, such that we now honestly believe that my wallet, and consequently all its contents
including the abovementioned Identification Cards, are already lost and are beyond
recovery;

5. The abovementioned Identification Cards were not confiscated or surrendered to any

government institution;

6. I am executing this Affidavit of Loss to attest to the truth of the foregoing and for
whatever legal purpose this may serve best.
