Focal Person of San Pascual, North District, as our true and lawful
representative, for us and in our names, place and stead, to accept/pick-up rice
subsidy of DepEd intended for us.

Hereby giving and granting unto our said representative full power and
authority to do and perform every act and thing whatsoever requisite and necessary
to be done in and about the premises as fully to all intents and purposes as we
might or could lawfully do if personally present hereby ratifying and confirming all
