AFFIDAVIT OF ACKNOWLEDGEMENT OF PATERNITY

I, FELIX S. DAGAN, of legal age, married and a resident of San Roque, Cortes, Bohol,
after having been sworn in accordance with law, hereby depose and say:

1. That I am the natural father of JERRY GUNO who was born on March 14, 1999, at
Loreto, Cortes, Bohol, to Leonor S. Guno, whom I eventually married on April 27, 1999, a copy
of the Certificate of Marriage is hereto attached and made an integral part of this document;

2. That Jerry Guno is a natural child under Rule 105 of the Rules of Court, having been
born before my wedding with his mother, Leonor S. Guno, and at the time when our child was
born, both f us were bent to marry and we were not in any way disqualified by any legal
impediment to marry each other;

3. That I hereby acknowledge and recognize that I am the true and natural father of Jerry
Guno, for all legal purposes and that he was born out of my intimate relationship with his
mother, Leonor S. Guno, whom I eventually married on April 27, 1999;

4. That I am executing this affidavit on my own free and voluntary will and do hereby
attest and affirm the fact and truth of the foregoing statements;

5. This affidavit is being executed to comply with the requirements of acknowledgement

and/or recognition of paternity, and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 7 th day, of June, 2005, at
Tagbilaran City, Bohol, Philippines.
