AFFIDAVIT OF UNDERTAKING
I, REBOMAPIL L. SUMOSA, Filipino, of legal age, single, and a resident of Tagum
Sur, Trinidad, Bohol, after having been duly sworn to in accordance with law, do hereby depose
and say:

That I applied for renewal of my Professional Driver’s Licsnese with the LTO-Tagbilaran
City;

That when they processed the renewal of my first driver’s licesme, they dicoevere that I
was still a minor at that time 2002;

That I really did not know that I was still a minor at that time because I believe that I was
born on July 15, 1982. I only dicosvered my true date of birth when I secured my records of birth
showing that I was born on July 15, 1985;

That I request and apologize to this error/mistake;

That I execute this affidavit to prove the truth of the above facts to support my renewal of
professional driver’s license and for whatever legal purpose/s this may serve.
