1 Location : Chiller Room

Observed that aluminum ladder kept near electric panel at chiller room,
aluminium is an good conductor for electricity and may leads to passing of
electric shock incase if electric shock occurs.

As Per OSHAD CoP 8.0 General Workplace Amenities

As per OSHAD CoP-15.0 Electrical safety

Risk : High

Preventive Action Required:

Aluminium ladder should be removed from the electrical room and
QHSE team recommends to use fibre ladder for electrical related activity
and maintain good housekeeping practices.

2 Location: Generator Room.

Observation:
Observed that fire extinguisher placed on the ground, which may cause
damage to the fire extinguisher and may not be able to access during any
fire related emergencies.

As UAE Fire and Life Safety Code of Practice- 2018

Risk: Moderate.

Preventive action required:

Hang the fire extinguisher in the wall in a easily accessible
position so that it does not get damaged due to mishandling and easy to
access in fire related emergencies.

Observation:
Observed that battery terminals do not have protecting cover
which may reduce the lifetime of the battery and may sparks and lead to
fire.

As per OSHAD CoP-15.0 Electrical safety

Risk Rating: HIGH.

Preventive action required:

Appropriate battery terminal cover should be installed for the battery
terminal and maintain in good condition so that it will increase the battery
lifetime.

4 Location : Adeeb Office- Ground

Observation:
An Expired first-aid kit was found in Ateeb's office, which could have
delayed getting first-aid during an emergency.

As Per OSHAD CoP-4.0 - First Aid and Medical

Risk : HIGH.

Preventive Action Required:

QHSE recommends replacing expired first aid so that it leads to immediate
first aid in an emergency. And the first aid kit is available in our adeebs
store.

5 Location : Chilled Pump Room- Ground

Observation:
Observed that hazardous substances stored without SDS, Safety Signage
and Spill tray at chilled pump room indicate poor housekeeping. Hazardous
substances may spill and get injurerd by slip, trip and fall hazard. Which may
escalate minor fire into major fire accident.

Preventive Action Required:

Hazardous substances should be removed from chilled pump room and
stored in designated place along with safety signage, SDS and spill tray.

General Recommendation:

1. Site Engineer Should arranged required PPE for the site people and shall monitor
for compliance.
2. Eng./Sup shall instruct workers for compliance of PPE properly for their assigned
task.
3. Supervisor shall ensure that all welfare facilities are available on site.
4. Consultation shall continue with the workers to maintain a good standard of
7 health & safety in the workplace.
5. Site Engineer should ensure that the revised incident report format is used for
reporting incidents.
6. Prevent falling object.
7. Prevent slip, trip and fall hazards in the workplace.
8. Keep your work area clean.
9. Keep all tools in good condition with regular maintenance.

All the issues raised above should be closed immediately with effects.
