Introductory Message
Welcome to TVL – ICT (Animation)!

The Learning Activity Sheet is a product of the collaborative efforts of the Schools
Division of Iloilo and DepEd Regional Office VI - Western Visayas through the Curriculum and
Learning Management Division (CLMD). This is developed to guide the learning facilitators
(teachers, parents, and responsible adults) in helping the learners meet the standards set by the
K to 12 Basic Education Curriculum.

The Learning Activity Sheet is self-directed instructional materials aimed to guide the
learners in accomplishing activities at their own pace and time using the contextualized
resources in the community. This will also assist the learners in acquiring the lifelong learning
skills, knowledge and attitudes for productivity and employment.

For learning facilitator:

The TVL - ICT (Animation) Activity Sheet will help you facilitate the teaching-learning
activities specified in each Most Essential Learning Competency (MELC) with minimal or no
face-to-face encounter between you and learner. This will be made available to the learners with
the references/links to ease the independent learning.

For the learner:

The TVL - ICT (Animation) Activity Sheet is developed to help you continue learning
even if you are not in school. This learning material provides you with meaningful and engaging
activities for independent learning. Being an active learner, carefully read and understand the
instructions then perform the activities and answer the assessments. This will be returned to
your facilitator on the agreed schedule.

II. Background Information for Learners
Animation tools and equipment must be properly inspected and assessed.
Through these, it will help to ensure the condition of the tools and equipment before using it
and to ensure the safety of the users.
In animation, different forms are accomplished to do a task:

● Job order slip is a written instruction that indicates performance of work according
to a specified requirements within specified time frame.

Requisition is made through a required form known as requisition slip or in the form of
a letter, if the previous is not available. Most government offices use uniform requisition slip,
while others have their own. In school shops, teachers are creative enough to devise their
own form to suit their local need. Such practice of using requisition slip promotes proper
monitoring of supplies, materials, tools, and performance and accomplishment of required
service or services among others

● Tools and Materials requisition form is a document that the production department
uses to request tool/s or material/s for production process. It is used to record the
movement of tool/s or material/s throughout the production process and allocate
production costs to goods in process.

The production manager usually fills out the materials requisition form and delivers it
to the materials or storage department where all the material/s or tools are stored.
Once the materials manager signs off on the request, the tool/s, material/s or are
moved from storage and placed on the production floor.

● Borrower’s Slip is a form used to borrow something such as materials, tools, or
equipment checkouts, for monetary means, etc. Its use promotes proper monitoring of
inventories. It has numerous purposes and outweighs disadvantages. This form is
devised according to the need of the office or accountable officer.

Properly completed Requisition Form is an important step in efficiently securing your


order; otherwise expect delays in the processing and approval of your requisition. The
requisitioning process is done by an authorized person.
1. Accomplish the slip with the required data.
2. Have it approved or signed by your supply officer or immediate supervisor.
3. Forward the slip to the section unit officer in charge of purchases, procurement, or
preparation and release of requisitioned items

Tips in Assessing Tools and Equipment for Proper Operation and Safety
1. Check if the materials and tools are complete in terms of quantity and specifications.
2. Check the physical condition of tools. Ensure that the parts are complete and free from
damage.
3. Check accessories of each tool and materials.
4. See to it that nothing is missing.
5. If any of the materials or tools is found to be missing or defective inform immediately
the officer in charge.

6. Secure a replacement for the material or tool found defective.
7. One must sign the requisition form upon receiving the materials and tools to ensure
that you have receive it.
8. Always handle the materials and tools carefully.
