A method statement is a document prepared by an organisation that describes in a logical sequence exactly
how a work activity is to be carried out in a manner which is safe and without risk to health.

To a large extent, the way that the job will be undertaken, and therefore detailed in the method statement,
will reflect the findings of the risk assessment(s) for the same job.

The means of controlling risk will influence the method of carrying out the job. In most cases, extensive
reference to risk assessment(s) will be necessary during the drafting of a method statement.

Well written method statements provide an ideal means of communicating vital health and safety
information to those who will be doing the works and others, such as project managers, or contractors,
who have an interest in how the job will be carried out.

This document is to be written using the input of the person who is safety trained and the person
responsible for carrying out the work. It should also be reviewed by the contractor’s project leader and
endorsed by the operatives.

Overview
In this Occupational Health and Safety Report the following points quoted hereunder from Legal Notice 36 of 2003,
General Provisions for Health and Safety at Workplaces Regulations are taken into consideration:
4. (1) It shall be the duty of the employer to ensure the health and safety of workers at all times in every aspect
related to the work.
4. (2) An employer shall make such appropriate arrangements for the effective planning, organization, control,
monitoring and review of the preventive and protective measures, taking into consideration the nature of the
activities and the size of the undertaking.
5. “… an employer shall after taking into account the nature of activities of the enterprise and establishment:
(i) Identify hazards at the place of work and shall avoid risks to the occupational health and safety.
(ii) Evaluate those risks to the health and safety of workers, which cannot be avoided and shall combat them at
source.
(iii) Adapt the work to the individual, especially with regards to the design of workplaces, the choice of work
equipment and the choice of working and production methods, with a view in particular to alleviate
monotonous work and work at a predetermined work rate and to reduce their effect on health”.
In order to ensure cooperation between employer and employee regarding health and safety at the workplace, the
same legal notice deals, also, with the employees’ share of responsibility:
15. (1) “It shall be the duty of each worker to care as far as possible of his own safety and health as well as that over
persons who can be affected by his acts or commissions at work in accordance with the training and the
instructions given by the employer.
The risk assessment carried out shall pinpoint hazards that may arise during works so that preventive action is
continuously in place to ensure the safety and health of workers and other personnel.
The following Legal Notices taken from the Occupational Health and Safety Authority Act- Act XXVII of 2000 Chapter
424 were consulted considering all phases of the project:

PROPOSED CONTROL MEASURES TO REDUCE THE RISK

Slips, trips and falls:

Workers must follow instructions delivered during induction meetings and SWMS
briefing.
Workers must walk through designated pathways.
Any steps or changes in levels must be suitably marked and rendered visible
Contractor is to ensure that the site is kept clean and clear of loose material. Good

housekeeping is essential.
Trailing cables are to be raised from the ground or covered with soft material accordingly
in order to avoid trips and damage to same wires.
The working areas, access routes and site surroundings must be illuminated at all times in
dark hours
Suitable non-slip safety shoes must be used at all times when working on site

Moving vehicles and machines:

Workers must follow instructions delivered during induction meetings and SWMS
briefing.
The manoeuvring/ danger zone area must be segregated.
Workers and/or machinery shall at all times keep away from exposed edges.
All plant and equipment must be inspected by a competent person and operated by
trained operators.
2 Moving vehicles/machinery must be equipped with warning lights and reverse beepers. X
Moving vehicles/machinery must be assisted by Banksman.
Moving vehicles/machinery must not exceed 10km/h.
Once the vehicles/machinery is in place, the engines must be switched off and key
removed from the ignition slot.
Workers are to wear the necessary PPE at all times.
Use of mobile phones or any other electronic equipment shall be prohibited while
operating machinery.

Moving parts of machinery:

All machinery shall be operated by competent personnel only


Workers must be familiar with the moving parts of the machinery, with special
consideration to pinch points during the manoeuvring and operation of equipment.
Moving parts of machinery must be equipped with appropriate guards.

Guards are not to be removed or tampered with.
If guards of machinery are not physically possible, the machinery must be secured with a
physical barrier
All machinery must have emergency stop buttons affixed and operational

Objects and parts with dangerous surfaces (sharp, rough etc.):

Hot or cold surfaces, material, etc

Hot surfaces;

Any surfaces which can cause serious injuries must be sign posted as “hot surfaces”
Before handling material that has been subjected to heat, a heat gun shall be used to
determine if that surface have cooled down to safe levels.
Resulting hot surfaces due to works must be handled by heat resistant gloves.

Hand & power tools

Workers must follow instructions delivered during induction meetings & SWMS briefing.
All hand tools must be in a good condition.
Damaged or worn out tools must be disposed of and replaced immediately.
All tools must be CE marked and used for their intended purpose
Hand tools must have adequate ergonomic handles.
Hand tools must be fit for purpose.
All power tools must be PAT tested.

All power tools must be equipped with all the necesary guards.
Loose clothing must not be worn when using power tools such as a driller.
All power tools must be equipped with 16A industrial sockets.
Electrical cables must be in a good condition. Damaged and/or taped cables must be
disposed of.
Trailing cables must be raised off the ground to avoid damage.
Workers should wear the necessary PPE at all times, such as safety gloves, safety glasses,
safety goggles, etc.

High pressure tools/machinery

Workers must follow instructions delivered during induction meetings & SWMS briefing.
High pressure tools and machinery must be inspected and certified by a competent
person before use

The links of pressure hoses and pressure pipes must be secured with appropriate fittings
The links of pressure hoses and pressure pipes must be secured with hose whips
High pressure equipment must be de-energised after use
Before doing any maintenance works on high pressure equipment, the necessary PTW
must be issued.

High pressure equipment must be de-energised before doing maintenance works, even if
routine. This must be done through the LOTO procedure.
Workers must use the necessary PPE when using high pressure tools or machinery.

Whole-body vibration

Workers & sub-contractors must follow all instructions delivered during the SWMS
briefing.
9 To minimise the exposure to HAV, workers must adopt job rotation and use vibration X
resistant gloves.
Tools and equipment used must be well maintained and lubricated frequently to
minimise vibrations

Adverse weather conditions:

Works in hot weather:


Works in high heat stress conditions/weather must be avoided. If not, workers must be
provided with suitable resting time.
Fresh drinking water must be available at all times.
Where possible, the working area must be shaded from direct sunlight.
Proper ventilation must be provided where possible.
Workers must be provided with sun cream (min 30SPF)

10 Works in cold weather: X


Works in rainy conditions must be avoided. If not, workers must be provided with rain
coats and boots to avoid getting soaked.
Workers must have facilities to shelter from the rain.

Works in windy conditions:


Works at heights must be avoided in winding conditions exceeding Force 6
Lifting of any material with mobile cranes or any other lifting equipment must be avoided
in windy conditions.
Loose material must be secured when high winds are forecasted.
After high winds, the stability scaffolds (if any) must be checked by a competent person.

Work involving poor posture

Workers & sub-contractors must follow all instructions delivered during the SWMS
briefing.

All workers must be trained in manual handling.
Mechanical aids must be used to lift heavy objects.
When lifting manually, the use of legs rather than the back should be encouraged.
Workers should wear the necessary PPE.

When works involve areas in awkward positions, workers must be allowed time to rest
and use job rotation as necessary.

Environment

Workers must follow instructions delivered during induction meetings & SWMS briefing.
Dust emitting activities must have dust extraction or water suppression systems in place.
Fine material stored anywhere on site must be covered to avoid dust dispersion in windy
conditions.
Equipment must be well maintained. If any machinery leaks any fluids, it shall be taken
12 out of service and repaired accordingly. X
Drip trays must be used as a last resort
Spill kits must be located on site and used if need be.
When vehicles and machinery are not in use, they must be switched off.
The site must have suitable waste management systems and waste separation
procedures in place during the works. Resulting waste must be recycled as much as
possible.

Falls from Height


Workers must follow instructions delivered during induction meetings & SWMS briefing.
Any exposed edge which includes but not limited to a working area, an access route, etc,
with a risk of fall from heights must be secured with suitable edge protection.
Edge protection must be 50cm & 100cm high and include a toe board.
Edge protection must sustain sufficient lateral forces.
Where there is no edge protection affixed, or where it is not possible to have adequate
edge protection in place, a full body safety harness shall be compulsory in all areas
presenting risk of falls from height.
Training in donning and correct use of safety harness shall be given to workers.
Only trained workers and workers who are medically fit shall be allowed to work with
safety harnesses at height.
Harness shall be attached to a safe anchor point by means of a lanyard or fall arrestor as
the case requires. Lanyard shall be sized according to task.

The anchor point must be certified by a competent person and must have a safety factor
of at least 5. The anchor point must be located as high as possible.
Safety harnesses must be checked for any wear & tear before each use and must not be
used if damaged.
Safety harness must be inspected by a competent person every 6 months.
Safety harnesses must only be used as a last resort and where no other reasonable means
is possible.

Working platforms:
Working platforms must be at least 60cm wide (full flooring), protected with guardrails
50cm & 100cm high (including a toe board), laid on stable footings and access with a
ladder 1 meter taller than the place of landing.
Working platforms shall be certified as correctly assembled and aligned by a competent
person (such as an architect or structural engineer) prior and material being placed or

personnel access on top of it.
Workers assembling the working platforms shall wear full body safety harness at all times
until all exposed edges are adequately guarded.
Safety harness shall be certified and attached to a secure anchor point via an adequate
lanyard or fall arrestor.
The work platforms must be designed and constructed according to the required SWL,
and must be checked by competent person before use.
The SWL must never be exceeded
If the work platforms cannot be fully boarded and there is a risk of falls, workers must use
a full body safety harness and affix it to a safe anchor point.
Work platforms in shafts must be fully boarded, only allowing a gap of 30cm from the
platform to the adjacent walls. If the gap is more than 30cm, a full body safety harness
shall be used at all times.
Elevated platforms around shutter components must be fully enclosed with edge
protection. Should any exposed edge remain due to works, workers must use fall arrest
equipment at all times.

Scaffolds:
Scaffold shall be erected by competent and certified personnel only.
Scaffold shall be erected on solid, stable and level ground and fitted with base plates.
Scaffold shall be equipped with, full flooring, toe boards, mid rails at 50 cm and top rails
at 100 cm from ground level and an adequate internal means of access, all secured /
anchored to scaffold structure to avoid any possible movement.
Scaffold shall only be assembled / composed of scaffold grade materials and components.
Scaffold`s Safe Working Load (SWL) shall never be exceeded.
No one shall be allowed to improvise heights (using ladders etc.) on top of scaffold.
No one shall be allowed to over reach beyond guard railing.
Scaffold shall only be accessed by means of access ladders fixed on scaffold internally and
secured to scaffold firmly.
Climbing on re-bar, outer parts of scaffold is strictly prohibited.
Access areas to the various scaffold levels shall be fitted with trap doors.
In cases were ratio of 3:1 (Height:width) is exceeded scaffold shall be fitted with out-
riggers or be adequately anchored.
Workers shall wear minimum PPE required (Safety shoes, safety helmet, high visibility
vest) at all times.
Workers assembling the scaffold shall wear full body safety harness at all times while on
scaffold.
Safety harness shall be certified and attached to a secure anchor point via an adequate
lanyard or fall arrestor.
While being assembled scaffold shall be tagged not safe for use and shall only be
accessed by competent personnel performing the assembly.
Upon completion of assembly, scaffold shall be checked and certified by a competent
person and will be tagged safe for use if deemed so.
If the scaffold is located along the pavement, sharp edges must be capped to avoid
injuries from passers by
If the scaffolding is located on the pavement or road, suitable lighting or flashers must be

affixed.
Suitable protection from the possibility of being struck by moving vehicles must be in
place.
Should scaffold need to be relocated, this shall not be dragged or jerked as it might
compromise components and / or structural integrity. Scaffold to either be fitted with
appropriate wheels fitted with brakes or be dismantled and re-assembled at required
location
Dismantling of scaffold shall also only be carried out by competent/certified persons
starting from the top and with use of full body safety harness anchored to a solid anchor
point.
Any anchorage / out riggers shall be removed starting from the top and only when ratio
of 3:1 (Height : width) is met.
During dismantling scaffold shall be tagged not safe for use and shall only be accessed by
competent personnel performing the disassembly.

Ladders:
Ladders must be in a good condition and not altered or temporarily repaired.
Ladders must only be used for works where 3 points of contact can be held, and at which
height does not exceed 2 meters from the rung that the worker is standing on.
Ladders must only be used for access purposes, or for works which require to stand on
the rungs for a long period of time. For work purposes, works must not last more than a
few seconds.
Workers must not work on the top 3 rungs of a ladder.
Ladders must be placed at a ratio of 1:4, secured from top and secured from the bottom.
If ladders cannot be secured in place, a buddy system must be used, where as a second
worker must hold onto the ladder until the task is completed.
Ladders must be used on solid and stable ground, and the footings must have the
required padding to avoid slipping
Ladders must be CE marked and safe for use. Makeshift ladders are not allowed to be
used.

Works on non-solid slabs;


Non-solid slabs must be identified before being accessed. These include steel slabs,
corrugated sheetings, etc. non-solid slabs must be inspected from underneath for
stability or defects before being accessed.
Non solid slabs must not be accessed prior to being inspected by a structural engineer
and the SWL determined accordingly.
If works are required on non-solid slabs, a lifeline must be set up, certified by a
competent person, and a safety harness used at all times.

Manholes and floor openings;


Manholes and floor openings must be covered with a robust piece of material, secured in
place and sign posted.

Use of Mobile Elevated Work Platform (MEWP);
The type of MEWP to be use must be selected according to the works at hand.
The MEWP must be certified every 6 months
The MEWP must be operated by a competent person, having undergone training to
operate a cherry picker
MEWPs must be setup on solid and level ground which must be checked beforehand.
MEWPs must not be driven with workers in the man basket, unless the MEWP is designed
for such purpose.
The outriggers of MEWPs (if any) must be fully extended before use
Workers inside the man basket must use a safety harness at all times. The harness must
be attached to the man-basket.
Overhead hazards must be considered such as slabs of balconies and overhead cables
In case of overhead cables, such cables must be sleeved or otherwise operator must keep
a safe distance at all times.

Use of man-cage with lifting equipment;


Any lifting equipment used for the purpose of man-lifts must be certified every 6 months.
All lifting accessories such as chains, slings & the man cage must be certified every 6
months
The crane must be operated by a trained and competent operator
A full body safety harness must be used when working in a suspended man cage.
The harness must be attached to a safe line affixed directly to the main hook of the crane.
The man cage must have the SWL clearly indicated and ensure that this is never exceeded
Crane operator must stay clear of any overhead cables at all times
Communication between the operator and the workers in the man cage must be
established prior to works. 2-way radios, hand signals and/or verbal communication
means must be established beforehand.
Works in suspended man-cage must only be carried out as a last resort and where not
other practical means are possible.

Important notice on working at height;


WORKS AT HEIGHT SHALL ONLY BE CARRIED OUT BEHIND THE SAFETY OF EDGE PROTECTION
OR AS A LAST RESORT, WITH FALL ARREST OR FALL RESTRAINT SYSTEMS.

Falling objects:

Workers must follow instructions delivered during induction meetings & SWMS briefing.
Where and when necessary, areas prone to falling objects must be hoarded accordingly.
Areas that are being concreted in situ, and also areas where precast elements are being
placed must be physically hoarded during such operations. Workers are only to be

allowed inside these areas in case of urgent propping adjustments and when the concrete
has cured to a safe level. Propping shall not be removed before the stipulated time
frames as enlisted in the method statement.
Temporary support systems must be inspected by the responsible eningeers before
casting is done, and a written declaration is issued in that regards. Such declaration must
cover the entire areas that are to be cast.

Lifting zones must be hoarded with a physical barrier.
Working platforms and scaffolds must be equipped with toe boards to avoid objects from
being struck overboard, resulting in damage to third party property or injury to other
workers.
Loading platforms must be inspected by a competent person. Such an inspection must
determine the SWL of any platform, which SWL must be clearly indicated on the
structure.
Any tools being used above other workers must be secured with hand straps.
High risk areas due to the risk of falling objects must be affixed with safety nets
Workers should wear the necessary PPE at all times.

Fire
Workers must follow instructions delivered during induction meetings & SWMS briefing.
A suitable amount of fire extinguishers must be kept in the working areas at all times
Fire extinguishers must be sign posted and serviced regularly
A number of workers must be trained in basic fire fighting
The stores and working areas must be kept in a good level of housekeeping, avoiding the
accumulation of material which can increase the fire load
Smoking must be limited to external designated areas only.

Hot works;

Hot works must be carried out following a permit to work system
The necessary hot work permit must be issued prior to works
Before carrying out hot works, workers must check the surrounding area for any potential
fire source that can be caused with such works
Workers must clear any source of fire before works
Hot works must be carried out by competent persons only and with the presence of a fire
watch team.
Working areas must be checked 1 hour after the hot works have been completed to
ensure that no sources of fire have been left behind.

Explosion
Workers must follow instructions delivered during TBTs & SWMS briefing.
Gas bottles must be used and transported vertically secured at all times
Gas bottles must be labelled with their content
16 Gas pipes must be inspected frequently for any cracks and possible leaks X
Gas bottles must be affixed with the necessary gauges
Flashback arrestors must be affixed as close as possible to the source of the flame
Gas bottles must be kept at a safe distance from the cutting area

Chemical Substance (incl Dust) in the air

Dust;

Dust emitting activities must be carried out with dust extraction or wetting process.
Workers must use dust masks as collective measure during such works.

Chemicals;
Workers must be trained in the handling, storage and use of chemicals.
Chemicals must be stored in suitable lockable containers, in a well-ventilated area and
according to their data sheets.
Chemicals that may react with each other must not be stored in the same area or cabinet.
All the necessary safety data sheets (SDS) must be available on location and must be
consulted prior to use.
Workers must use chemicals in well ventilated areas, away from ignition sources, and by
using the necessary PPE at all times. The selection of PPE must be according to the data
sheets.
Workers shall not mix any chemicals

Concrete;
Workers must avoid at all times direct contact with wet concrete
Workers must wear suitable clothing that covers all the extents of the body
Workers must use safety glasses in case of concrete splashes in the eyes

Electrical installations

Electrical connections must be obtained through an appropriate DB.


Electrical works must only be carried out by licensed electricians.
Electrical installations must be certified by a licensed electrician or electrical engineer at
least once every 3 months, or when additions or alterations to the existing system has
been carried out.
Electrical installations must be inspected and checked on a weekly basis to ensure good
functionality of all safety devices and circuit breakers.
All power tools must be equipped with 16A industrial sockets.
Electrical cables must be in a good condition with no parts of the insulation being

Water borne pathogens;


Any stale water on site must be immediately removed. Stale water can attract a number
of pathogens such as mosquitos, which may carry certain diseases.

Stress / Fatigue due to Night Shifts

Workers & sub-contractors must follow all instructions delivered during the SWMS
briefing. Educate your managers and workers about the importance of sleep
Ensure all employees are aware of the dangers coming to and from work due to fatigue –
advise on carpools/public transport/taxis
Ensure the site is well lit at night as workers should not be allowed to work in the dark
Allow workers to have regular rests throughout their shift
Ensure all workers are wearing the correct PPE i.e. Hard hats, High Visibility etc.

Workers should also be conscious of what they need to do to prepare themselves for a
night shift.
• Take a nap of 1-4 hours before the first night shift
• Keep your sleeping pattern regular
• Have your largest meal after your day-time sleep, before starting the night shift

Managers and supervisors should learn to recognize signs and symptoms of the potential
health effects associated with night shifts. Workers who are being asked to work night
shifts should be diligently monitored for the signs and symptoms of fatigue. Any
employee showing such signs should be evaluated and possibly directed to leave the
active area and seek rest.

Plan to have an adequate number of personnel available to enable workers to take


breaks, eat meals, relax, and sleep. If at remote sites, ensure, as far as possible, that
there is a quiet, secluded area designated for rest and recuperation.

Method Statement for Buildings Works
As per Sixth Schedule (regulations 7 & 8)

Please note that additional documents such as drawings and photographs are to be referred to as numbered appendices and
uploaded using the relevant document type following this form.

1. Commencement Date of Works

Upon issuance of the clearance to start works on site from the Building Construction Agency (BCA)

2. Checks showing that any existing floors are capable of sustaining the additional load being imposed by the
new construction. (Include relevant appendix number/s)

No additional loads are going to be transferred to the other third-party structure since this development consists of a stand-
alone building.

3. Checks showing that the foundations of the building are capable of sustaining any additional loads placed
over the existing storeys. (Include relevant appendix number/s)

This is not applicable since there will be no construction works carried out on the existing foundations and
the building is going to be totally demolished.

5. Proposed Methodology

MATERIAL

All the materials specified herein will be in accordance with the approved specification for reinforced concrete works.
Material submittal will be done in accordance with the approved specification.

FORMWORK

- The formwork shall comply with EN, and Malta Local Standards as specified in Project Specification.
- The formwork will be so constructed as to remain sufficiently rigid during the concrete placing and should be
sufficiently tight to prevent loss of cement slurry from the concrete.
- Execution of form work will take into account climatic conditions, the expected heat of hydration and finishing of
concrete surfaces.
- Form will be sufficiently tight to prevent leakage of grout or cement paste.
- Plywood and other wood surfaces will be sealed against absorption of moisture from the concrete by either a field
applied, approved form oil or sealer, a factory applied non-absorptive liner.
- All surfaces of forms and embedded materials will be cleaned of any accumulated mortar or grout from previous
concreting and of all matter foreign material before concrete is placed in them.

REBAR

- Bars will be transported to the Site for placing by trailer or truck.


- Bars will be cut and bended as per drawings at workshop, and will be fix at workshop, when necessary.
- Bending machine with low steady pressure shall be used.
- Welding of reinforcement shall only be done in accordance with Project Specification.
- All reinforcement will be placed and maintained in the position shown on the drawings, in which the dimensions
and scheduling of bars should be in accordance with EN ISO 15630-1:2002
- Bars will be tied together using annealed iron wires of 1.5mm at intersection points, to form rigid cages that will
maintain their position without distortion or twisting during the placing and compaction of the concrete.
- Prefabricated spacers of concrete or plastic type shall be inserted between the bars and internal surfaces of the
formwork, so that the required concrete cover thickness is maintained during the placing and compaction of the
concrete.
- All metal for reinforcement should be free from loose mill scale, loose rust, oil and grease, or harmful matter,
immediately before placing the concrete.
- The tips of the exposed rebars will be protected with mushroom caps.
- Exposed rebars will be protected by cement-based material which will be submitted for approval.

CONCRETE

- The measuring, mixing, production and transporting of concrete will be in accordance with EN and Malta Local
Standards.
- The placing and compaction of concrete shall be in accordance with the appropriate section of EN and Malta Local
Standards
- All concrete will be consolidated by vibration, spading, rodding, or forking so that the concrete is thoroughly worked
around the reinforcement and around the embedded items.

- Concrete Materials:
Cement:
Portland cement conforming with EN 197-1:2000 will be used. Stacked cement will be stored in a dry, well-
ventilated place with boarded floors.
Aggregates
Coarse aggregates will consist of first quality crushed hard stone passing a 19.05mm mesh sieve, graded
according to EN 12620-2002. Sand or fine aggregates will be clean, sharp, and gritted and free from loamy
matter and other deleterious substance. They will be graded according to EN 12620-2002.
Water
Clean, fresh water, entirely free from organic matter will be used.

HOLLOW CONCRETE BLOCK WALLS (HCB WALLS)

- The production and transporting of HCBs will be in accordance with EN and Malta Local Standards.
- The blocks will be unloaded from the truck by tower crane, mobile crane and/or forklift.
- All blocks will be lifted to the requested floors by tower cranes.

WATERPROOFING (MEMBRANE)

- The production, transporting, and storage of waterproofing membrane will be in accordance with EN and Malta
Local Standards.
- Weather conditions will be checked before installation.
- Waterproofing works will be applied on hardened blinding concrete surface which was polished with power floater
during concrete casting process and HCB walls Top of blinding concrete and surface of HCBs shall be clean,
sound and dry, free from oil and grease, dust, loose or friable particles, protruding ribs, nails, etc.
- Additionally, all works which belongs to other disciplines (for example Mechanical works, pipe installation, etc) will
be finished before execution.
- All holes on the concrete / HCB surfaces will be filled by required materials to provide a suitable surface. Detail
points as pipes and cables around them shall be filled by repair mortar.
- All cracks, expansion and construction joints shall be raked out, swept, washed, cleaned before the application of
the waterproofing treatment.

INSPECTION & TEST PLAN

- Notices to check shall be provided at least twenty-four (24) hours in advance of any inspection that HAS
Construction has planned to make. If the twentyfour (24) hour period would expire on a Saturday, Sunday and/or
Public Holiday, the notice period for the inspection shall be forty-eight (48) hours.
- Testing to be executed will be done according to the contract specifications. Also, one more test will be issued 3
days after the casted concrete to get permission to start removing the formwork. This is also stated in the method
statement of removal formwork.
- The Notice to check is a request of site inspection/check issued by the HAS Construction to the Document
Controller, Engineer the Relevant Employer’s Consultants, and other relevant stakeholders to inform that some
work will be inspected.
- HAS Construction will collect all delivery notes and will issue a LOG to track the concrete. All necessary
information like time, date, location, quantity, concrete grade, etc. will be stated in the LOG.
- HAS Construction will issue the plan of the construction joints for all concrete elements.
- These plans will be submitted separately for approval.

The SQAQC Responsible shall verify that:

- Mix Designs have been properly submitted by the main contractor and approved by the Engineer.
- These can be made and crushed in ready mixed concrete suppliers laboratory to check that the cured concrete
has obtained the required design strength. A standard 150 x 150 x 150 mm steel test cube mould is used,
thinly coated inside with formwork oil. A concrete sample is taken from the discharge outlet of mixer or from the
point of placing on site.
- The sample cube is covered with a damp sack or other covering and left for 24 hours at a temperature of 4.4-
21°C. The sample is then removed from the mould and stored in water at a temperature 10-21°C until required
for testing.
- Testing of Concrete is carried out in accordance with technical specifications.
- However, an accredited laboratory will be assigned to witness the samples and tests.
- Specified Formwork and Concrete Mock-ups are completed and approved
- Reinforcing steel quantity, size, location, and support conforms with project requirements
- Processes are in place to ensure that all pours are properly scheduled and coordinated with all project disciplines
and testing agencies
- Curing and Sealing Processes comply with specifications.
- All inspections and tests noted in the specifications and Special Inspections Schedule are completed and
approved as specified
- Waterproofing and Termite Treatment comply with specifications
- Finishers are aware of the finish tolerances specified
- Post-Tensioned Tendon test logs are maintained as specified
- Proper calibration and certification for all Stressing and Testing Equipment
- Hot & Cold Weather processes are followed when required.
- Ensure that all necessary control joints, expansion joints, embeds, block-outs and pipe sleeves noted on the plans
and in the specifications for all crafts are installed prior to concrete placement.

APPLICATION

Formwork & Scaffolding

- Formwork installations will be done according to Project drawings. In accordance with approved formwork
systems, the vertical / horizontal scaffolding arms will be installed according to the determined intervals.
- Push-Pull props will be used for the vertical alignment and supporting wall and column formwork according to
technical specification and design.
- Slab probs will be used for supporting slab formwork according to technical specification and design.
- The scaffold will be erected with educated employees.

Foundation

- Foundation reinforcement will be prepared at an appropriate place on ground.


- All steel reinforcement will be free from scales and any appreciable rust, and steel will conform in all respects to
the requirements specified in EN ISO 15630-1:2002
- The prepared foundation reinforcement will be carried to its location with tower/mobile Crane and will be installed
according to design drawings.
- Formwork will be prepared with appropriate dimensions. Cleaning and greasing will be made for easy removal.
Approved material will be used for greasing.
- Adequate and effective precautions will be taken to leave all reinforcement undisturbed during concreting.
- The surfaces will be cleaned, and air-pressured compressor will be used when necessary.
- Pour concrete following the steps:
- Sample will be taken from truck mixer for slump test
- Concrete pump or tower Crane will be used for carrying concrete
- Vibrators will be used for consolidation after pouring

Columns

- Column reinforcement will be prepared - longitudinal bars and stirrups will be tied together with binding wires
according to design drawings – at an appropriate place on ground.
- All steel reinforcement will be free from scales and any appreciable rust, and steel will conform in all respects to
the requirements specified in EN ISO 15630-1:2002.
- The prepared column reinforcement will be carried to its location with tower/mobile crane and the reinforcement
will be fixed to starter bars coming from bottom level.
- Adequate and effective precautions will be taken to leave all reinforcement undisturbed during concreting.
- Formwork will be prepared with appropriate dimensions. Cleaning and greasing will be made for easy removal.
Approved material will be used for greasing.
- Half of the column formwork will be installed (two consecutive sides forming a corner) with tower crane.
- The column base will be cleaned
- The other half of the formwork will be installed to close the frame with tower crane.
- Final topographic checks will be made on column formwork location.
- Shuttering or centering will be true to line and sufficiently strong to carry the dead weight of wet concrete plus any
incidental loading without noticeable deflection. It will be sufficiently rigid to guard against movement or vibration
whilst concrete is being placed and tamped into position. Shuttering will be close together to prevent leakage of
the fine stuff and will be ‘treated’ with the minimum of formwork oil to facilitate striking.
- Cantilever scaffolding will be prepared at two sides of column formwork top as a safely working platform. The
platform will has side rails on three sides.
- Harness will be applied to fixed points when climbing to high level areas in general before the formwork platform
is erected.
- Ladder will be used to safely reach the top of column formwork.
- Concrete pouring will follow the steps below;
- Sample will be taken from truck mixer for slump test
- Concrete pump or tower Crane will be used for carrying concrete on top of column.
- A hose pipe will be laid down into formwork.
- Internal vibrators will be used for consolidation after pouring each layer.

Shear Walls

- Necessary actions to reserve MEP holes will be taken on the walls as they are designated on design drawings.
- The wall reinforcement will be prepared and installed – horizontal and vertical bars tied together with binding wires
according to design drawings – at the place. Starter bars coming from bottom level will be used.
- All steel reinforcement will be free from scales and any appreciable rust, and steel will conform in all respects to
the requirements specified in EN ISO 15630-1:2002
- Adequate and effective precautions will be taken to leave all reinforcement undisturbed during concreting,
- Formwork will be prepared with appropriate dimensions. Cleaning and greasing will be made for easy removal.
For greasing, approved material will be used.
- One long and one short side of the wall formwork will be installed with tower crane.
- The shear wall base will be cleaned
- The other half of the formwork will be installed to close the frame with tower crane.
- Final topographic checks will be made on wall formwork location.
- Shuttering or centering will be true to line and sufficiently strong to carry the dead weight of wet concrete plus any
incidental loading without noticeable deflection. It will be sufficiently rigid to guard against movement or vibration
whilst concrete is being placed and tamped into position. Shuttering will be close together to prevent leakage of
the fine stuff and will be ‘treated’ with the minimum of formwork oil to facilitate striking.
- Cantilever scaffolding will be prepared at two sides of column 12 formwork top as a safely working platform. The
platform will have side rails on three sides.

- Any holes left which was necessary to tie the formwork, will be plugged and covered with an approved cement
mortar/material.

Slabs

- Scaffolding supports will be fixed securely at the bottom.


- Scaffolding will be prepared and installed on the line of beam axes first, then continue with slab areas.
- Safe working conditions will be established to work on upper level.
- For the columns with column caps, bottom/side forms of column caps will be prepared and installed with site cut
plywood.
- Bottom forms of beams will be prepared and installed with site cut plywood.
- Side forms of beams will be prepared and installed with site cut plywood.
- Bottom forms of slabs will be prepared and installed with site cut plywood.
- Shuttering or centering will be true to line and sufficiently strong to carry the dead weight of wet concrete plus any
incidental loading without noticeable deflection. It will be sufficiently rigid to guard against movement or vibration
whilst concrete is being placed and tamped into position. Shuttering will be close together to prevent leakage of
the fine stuff and will be ‘treated’ with the minimum of formwork oil to facilitate striking.
- Final topographic checks will be made on formwork elevations.
- Necessary actions will be taken to reserve MEP holes on the slab as they are designated on design drawings.
- The surfaces will be cleaned, and air-pressured compressor will be used when necessary. For easy removal,
grease will be applied to all surfaces and approved material will be used.
- All steel reinforcement will be free from scales and any appreciable rust, and steel will conform in all respects to
the requirements specified in EN ISO 15630-1:2002.
- Mesh reinforcement will comply with EN ISO 15630-1:2002
- Adequate and effective precautions will be taken to leave all reinforcement undisturbed during concreting.
- Column cap reinforcement will be prepared and installed if any exist.
- Beam reinforcement will be prepared and installed.
- Bottom slab reinforcement followed by top slab reinforcement will be prepared and installed.
- Additional slab reinforcement will be prepared and installed if any exist.
- The formwork will be cleaned apart from dust
- Concrete pouring will follow the steps below Sample will be taken from truck mixer for slump test
- Concrete pump or tower Crane will be used for carrying concrete
- Vibrators will be used for consolidation after pouring
- Special edge protection precaution will be taken for slabs to be erected in more than one pour at a time.
- At the edge of the part poured at first, edge protection will be prepared to prevent worker/material fall from upper
level to the level below.
- This temporary edge protection will extend from one end of peripheral edge protection to the other end.
- Sufficient edge protection will be provided at all sides of the slab where a falling potential exists.

Curing and Protection of Concrete

- Curing for foundation will be done with water.


- Curing of Slabs, columns and walls will be done by chemical compound.

Methods to Control Vertically of Structure

- The structure points will be given by the survey team according to the project. Methods will be used to check or
control verticality works include;
Plumb-bob technique
- A weight with pointed tip will be attached on the bottom end of a string.
- The heavy weight will hang under gravity and offer a precise vertical line which is called plumb line.
- This method will be applied for checking or controlling vertical line of structural elements especially
indoors such as lift shaft. Added to that, it will be used to control verticality of foundation, walls, and
columns.
• Theodolite
- Digital Theodolite centered on a peg that installed 500 mm from the column grid.
- After Theodolite set up accurately the laser beam will be turned on and focused it to the steel tape which
is held to the formwork.
- The reading of the steel tape will be taken with the telescope at the same level on both top and bottom
levels of the formwork.
- By taking two readings at the same level any curvature on the surface will be identified.

(Refer to relevant figures on pages 14- 15 in Annex 1)

Hollow Concrete Block Walls (HCB Walls)

- The measuring, mixing, production and transporting of HCBs will be in accordance with EN and Malta Local
Standards.
- Guide blocks shall be placed onto cement mortar bed; leveled and plumbed by the help of rubber hammer.
Location and level of guide blocks shall be accurate to wall layout.
- Cement mortar will be applied in between block surfaces with notched trowel. Horizontal or vertical joint gaps
will be approximately 1 - 1,5 cm.
- Wall corners will be constructed by staggered installation (Refer to Annex 1 p.16). Staggered installation will
also be provided at partition wall intersections.
- End of walls will be connected by Hilti nail gun to columns by 2 mm galvanized type-L clips in every 2nd course.
Connection points will not overlap with the opposite end.
- The gap between wall and ceiling slab will be filled with rockwool. Fire resistant foam will be applied only one
side.
- Blocks will be cut to required dimension with concrete block cutting machine by the help of set square.
- Door subframes will be connected to 6’’ solid concrete blocks. Solid blocks will only be used at bottom, mid and
top anchorage points.
- Concrete precast / metal lintels will be provided over door openings. Precast production will be done at job site.

Waterproofing

For Below Grade; (Refer to Annex 1 pg. 18)

- Simproof is loose laid on the lean concrete, self adhesive overlaps are adhered. Overlaps are 10 cm.
- The joints are aligned.
- Remove the protective tape
- Joints are adhered and passed over with a printing roller.
- Final checks are made on the membrane.
- It is a SBS modified waterproofing membrane (4.5mm) for below grade which adheres permanently to the
structural concrete to be poured.

For Single Sided Shearwall;

- The waterproofing membrane is adhered to the drainage board which has been previously applied on the shoring
wall, which is designed to be rigid and able to withstand the concrete pressure. Self-adhesive overlap parts are
adhered. Overlaps are 10 cm.
- It is a SBS modified self-adhesive waterproofing membrane which is used for the waterproofing of blank mould
shear walls, which is applied before concrete is poured on it and can provide a permanent adhesion to the
structural concrete.

Formwork Removal

Reinforced concrete slabs

- The slab formwork system shall consist of various scaffolding modules which can be independently dismantled
without the need to dismantle the whole formwork system.
- In this regard, the undersigned is hereby proposing the removal of concrete slab formwork subject to the following
conditions:
- Formwork can be removed after 4 days from time and date of casting as long as a 3-day cube crushing
test result proves that concrete has reached a minimum of 70% of its ultimate strength, or a minimum of
25MPa.
- Formwork shall be removed progressively and a nominal amount of props shall be immediately reinstated
under the concrete slab, with an average distance of approximately +/- 2.5m. Additional props in the form
of scaffolding might be required at midspan depending on the type of slabs being cast. The amount of
props to be provided after the 4th day from concrete casting shall be at the discretion of the undersigned.

Reinforced concrete beams

- Similarly to slabs, beam formwork system shall also consist of various scaffolding modules which can be
independently dismantled without the need to dismantle the whole formwork system.
- In this regard, the undersigned is hereby proposing the removal of concrete beams formwork subject to the
following conditions:
- Formwork can be removed after 7 days from time and date of casting as long as a 7-day cube crushing
test result proves that concrete has reached a minimum of 70% of its ultimate strength, or aminimum of
25MPa.
- Formwork shall be removed progressively and a nominal amount of props shall be immediately reinstated
under the concrete beams, withan average distance of approximately +/- 2.5m. Additional props in the
form of scaffolding might be required at beam midspans depending on the type of beams being cast. The
amount of props to be providedafter the 8th day from concrete casting shall be at the discretion of the
undersigned.

Housekeeping

- Housekeeping will be executed daily wise. Additionally, waterjet or compressor will be used for cleaning the areas
where concrete pouring will take place.

The above-mentioned works shall commence with the installation of all the necessary Health and Safety signs on the
façade and preparation of all the necessary measures for safety.

Additionally, All activities shall be carried out by taking the following precautions:

- Work area shall be cleared for safe access


- Application area must be clean.
- The shafts, stairs and any openings on the slab shall be covered or surrounded with safety net and-or temporary
railings
- Temporary electrical power shall be provided where necessary for equipment and temporary illumination

All reinforcement works to be done as per structural drawings. If any issues which may be of concern at any point, the
contractor will ask the Perit in charge and should stop from any progress of works. Prior to the casting of any concrete,
the Contractor will check and confirm that the reinforcement is laid out as per approved structural drawings. Adequate
spacing should be left beneath the steel reinforcement as per structural drawings.

The existing or new structures’ ceilings should not be loaded with building materials / debris or any other objects
exceeding the permissible loading. If additional loading is required then, the ceilings should be temporarily propped.
Services passing through the site, whether overhead or buried, must be identified, carefully diverted, and protected.
The site must be carefully hoarded and cordoned off and kept always closed. Safety signage will be installed as
necessary, including warnings to pedestrian traffic or motor vehicles, stationary or in motion. Access to and from the
site must be strictly controlled. Adequate protection will be taken against the accumulation of rainwater within the site

Procedure for the construction of the pool/s:

1. Temporary formwork will be erected for the casting of the concrete, ensuring that the formwork is rigid and stable.
2. Once the formwork is set up, the reinforcement is to be placed. This should be placed out with the spacing and
quantity specified in the construction drawings.
3. The Contractor will then check and confirm that the reinforcement is laid out as per approved structural drawings.
4. The Perit will be notified if at this point any water or electrical services need to be passed alongside the
reinforcement.
5. The Perit in charge will also be given adequate time to carry out an inspection of the reinforcement before the
casting of concrete.
c) Measures to safeguard the stability of the works being undertaken and the stability of the contiguous
structures:
Same as those listed in (h)

d) Machinery equipment to be used on site during construction works include;


i. Towe Crane – upon installation a certificate will be provided by a qualified engineer.
ii. Concrete Mixer Trucks
iii. Mobile Concrete Pump
iv. Vibrators
v. Hand Tools

e) The type of machinery equipment that is not allowed to be used;


No other machinery should be used unless first consulting with the project architect and obtaining written consent for
the use of additional machinery.

f) Specific interventions required during the course of the works;


None, however, should the contractor see the need arise, he will contact the Perit in charge immediately and wait for
further instruction.

g) The type of crane to be used;

The development in question can be sufficiently managed and operated with the use of a tower crane. Materials will
be brought at a pre-appointed time, in accordance with the Local Council, Police Force and any other competent
authority, unloaded with care, tightly always securing all the components.

DELIVERY, STORAGE, HANDLING

- Required concrete will be provided from approved suppliers with concrete trucks for all concrete works.
- If necessary, packaged materials will be delivered to the site in the original packages and/or containers with
labels intact and seals unbroken.
- Cementous materials will be kept dry and stored off the ground under cover away from damp surfaces until
ready to be used.
- Metal accessories and miscellaneous materials will be kept in closed storage area, where recommended by the
manufacturer.
- Store materials will be under cover and kept dry and protected against any damage from weather, direct
sunlight, surface contamination, corrosion, construction traffic, and other causes during application.
- No materials will be stored on newly casted concrete.

h) Precautions and safeguards to be adopted on site include;


i. Against instability of the structure;
Proper structural design and the use of any bracing deemed necessary.

The Contractor shall be taking care of:

- Supervision of all superstructure works.


- Carrying out superstructure in accordance with the project requirements.
- Verifying that the works is carried out as per the quality standards.
- Ensuring full compliance with HSE requirements and procedures.

QUALITY CONTROL
Critical stages for inspection to follow are;

- Delivery, storage, and handling of the materials shall be as specified.


- Drawings to be reviewed and checked indicating all locations and including necessary plans, elevations,
sections, details of components and necessary attachments to the other works.
- Surface preparation shall be checked.
- Materials and equipment shall be clean and free of damage and shall be checked before use.

ii. For parts thereof being demolished;


The contractor will ensure that construction works are carefully carried out and that the whole site is safe and
secure to work in, special attention will be given to any falling debris. Temporary props, warning signs, gangways,
scaffolding, hoarding and lighting will be installed as required by law.

iii. For any contiguous structures;


Constant monitoring of the situation as the works progress. The contractor will report anything that might appear
to be of danger or potential danger immediately.

iv. For the safety of persons;


Primarily, a Health and Safety officer for the safety of persons as well as;

other protection measures for the safety of pedestrians & vehicles which should be used on site:

i. Adequate lighting, safety signage and traffic control measures are to be provided (as necessary).
ii. A 1.0m footpath must be kept clear in front of the building alignment. If the site fencing restricts such a
space, then a temporary new footpath is to be constructed before commencement of the works on site.
iii. Signage specifying key contractors and a 24-hour contact name and phone number must be fixed on
site. The Contractor is also responsible, during such operations, for providing an alternate safe
pedestrian route is to passers-by.

precautions must include the following measures:

i. Use of signage throughout.


ii. Restriction on the hours of operation for such activities (non-peak hours).
iii. When any type of lifting operation is carried out on site, the procedures to be adopted shall strictly
adhere to the Risk Assessment Method Statement provided to the Contractor by the H&S officer.
6. Procedure to be adopted for the loading and carting away of the resulting debris

Waste Management

The Construction Industry is one of the greatest generators of waste in the local scenario. Thus efforts should be put in place
to primarily MINIMIZE waste generation (e.g. clipping of Rebar, and steel reinforcement, cutting of masonry and concrete
blocks, disposal of formwork and shuttering). Works shall be carried out in strict adherence with the provisions of the
Environmental Protection Act, 1990. Environmentally sustainable procedures shall be adopted throughout the construction
sequence. Care shall be taken not to cause damage to any areas located within close proximity to the site, particularly areas
of ecological and/or historical importance. No construction works, storage or parking of construction vehicles shall be
permitted outside the site.

Only registered and approved dumping sites shall be used for the disposal of construction or other waste. Waste separation
at source shall be practiced throughout the duration of works and regular waste collection and disposal shall be ensured.
The Contractor is committed to minimizing construction waste by adopting best practices including a site wide waste
minimization scheme that maximizes material use and keeps waste to a minimum. The locations of waste management
facilities will be carefully planned to minimize disturbance to the public and to allow easy access for workmen, along with the
following practicalities:

i. Reuse of suitable excavated materials from site works.


ii. Giving details to key site personnel of what and when can material taken off the site be recycled, typically to include timber,
concrete, bricks, site metals and drink cans/bottles.
iii. Reduction of materials wastage through good storage and handling.

The following steps shall be taken to prevent disruption to the public area: • Skips or rubbish bins are to be place away from
public pedestrian access areas

● Times and methods for loading and unloading of bins and skips are to be specified
● Pavements and streets are to be protected

Any hazardous material shall be notified to the Environmental Protection Department and shall be transported in accordance
with the relevant regulations.

Air and dust management

Air quality (airborne dust and pollutants) in and around the construction site is to be maintained at acceptable levels throughout
the construction period. Adequate measures for preventing dust and other airborne matter impacting on the surrounding area
are to be ensured such as using water in proper proportions to avoid any damages which may result due to water ingress in third
party quickly as possible. All trucks transporting material off the site need to be covered. Dumping of the debris into the truck
must not be done from a height and the bucket is to be kept as low as possible to the edge of the truck to minimize the possibility
of debris spilling over the side and to reduce the generation of dust.

8. MATERIAL

All the materials specified herein will be in accordance with the approved specification
for reinforced concrete works. Material submittal will be done in accordance with the
approved specification.

• FORMWORK
o The formwork shall comply with EN, and Malta Local Standards as
specified in Project Specification.
o The formwork will be so constructed as to remain sufficiently rigid during the
concrete placing and should be sufficiently tight to prevent loss of cement
slurry from the concrete.
o Execution of form work will take into account climatic conditions, the
expected heat of hydration and finishing of concrete surfaces.
o Form will be sufficiently tight to prevent leakage of grout or cement paste.
o Plywood and other wood surfaces will be sealed against absorption of
moisture from the concrete by either a field applied, approved form oil or
sealer, a factory applied non-absorptive liner.
o All surfaces of forms and embedded materials will be cleaned of any
accumulated mortar or grout from previous concreting and of all matter
foreign material before concrete is placed in them.

• REBAR
o Bars will be transported to the Site for placing by trailer or truck.
o Bars will be cut and bended as per drawings at workshop, and will be fix at
workshop, when necessary.
o Bending machine with low steady pressure shall be used.
o Welding of reinforcement shall only be done in accordance with Project
Specification.
o All reinforcement will be placed and maintained in the position shown on the
drawings, in which the dimensions and scheduling of bars should be in
accordance with EN ISO 15630-1:2002
o Bars will be tied together using annealed iron wires of 1.5mm at intersection
points, to form rigid cages that will maintain their position without distortion
or twisting during the placing and compaction of the concrete.
o Prefabricated spacers of concrete or plastic type shall be inserted between
the bars and internal surfaces of the formwork, so that the required concrete
cover thickness is maintained during the placing and compaction of the
concrete.

o All metal for reinforcement should be free from loose mill scale, loose rust,
oil and grease, or harmful matter, immediately before placing the concrete.
o The tips of the exposed rebars will be protected with mushroom caps.
o Exposed rebars will be protected by cement-based material which will
besubmitted for approval.

• CONCRETE
o The measuring, mixing, production and transporting of concrete will be in
accordance with EN and Malta Local Standards.
o The placing and compaction of concrete shall be in accordance with the
appropriate section of EN and Malta Local Standards
o All concrete will be consolidated by vibration, spading, rodding, or forking
so that the concrete is thoroughly worked around the reinforcement and
aroundthe embedded items.
o Concrete Materials:
 Cement
Portland cement conforming with EN 197-1:2000 will be used.
Stacked cement will be stored in a dry, well-ventilated place with
boarded floors.
 Aggregates
Coarse aggregates will consist of first quality crushed hard stone
passing a 19.05mm mesh sieve, graded according to EN 12620-
2002. Sand or fine aggregates will be clean, sharp, and gritted and
free from loamy matter and other deleterious substance. They will be
graded according to EN 12620-2002.
 Water
Clean, fresh water, entirely free from organic matter will be used.

• HOLLOW CONCRETE BLOCK WALLS (HCB WALLS)


o The production and transporting of HCBs will be in accordance with EN
and Malta Local Standards.

o The blocks will be unloaded from the truck by tower crane, mobile crane
and/or forklift.

o All blocks will be lifted to the requested floors by tower cranes.

• WATERPROOFING (MEMBRANE)
o The production, transporting, and storage of waterproofing membrane will
be in accordance with EN and Malta Local Standards.

o Weather conditions will be checked before installation.

o Waterproofing works will be applied on hardened blinding concrete surface


which was polished with power floater during concrete casting process and
HCB walls Top of blinding concrete and surface of HCBs shall be clean,
sound and dry, free from oil and grease, dust, loose or friable particles,
protruding ribs, nails, etc.

o Additionally, all works which belongs to other disciplines (for example


Mechanical works, pipe installation, etc) will be finished before execution.

o All holes on the concrete / HCB surfaces will be filled by required materials
to provide a suitable surface. Detail points as pipes and cables around
them shall be filled by repair mortar.

o All cracks, expansion and construction joints shall be raked out, swept,
washed, cleaned before the application of the waterproofing treatment.

9. DELIVERY, STORAGE, HANDLING

• Required concrete will be provided from approved suppliers with concrete


trucks for all concrete works.
• If necessary, packaged materials will be delivered to the site in the original
packages and/or containers with labels intact and seals unbroken.
• Cementous materials will be kept dry and stored off the ground under cover
away from damp surfaces until ready to be used.
• Metal accessories and miscellaneous materials will be kept in closed storage
area, where recommended by the manufacturer.
• Store materials will be under cover and kept dry and protected against any
damage from weather, direct sunlight, surface contamination, corrosion,
construction traffic, and other causes during application.
• No materials will be stored on newly casted concrete.

10. INSPECTION & TEST PLAN

• Notices to check shall be provided at least twenty-four (24) hours in advance


of any inspection that HAS Construction has planned to make. If the twenty-
four (24) hour period would expire on a Saturday, Sunday and/or Public
Holiday, the notice period for the inspection shall be forty-eight (48) hours.
• Testing to be executed will be done according to the contract specifications.
Also, one more test will be issued 3 days after the casted concrete to get
permission to start removing the formwork. This is also stated in the method
statement of removal formwork.
• The Notice to check is a request of site inspection/check issued by the HAS
Construction to the Document Controller, Engineer the Relevant Employer’s
Consultants, and other relevant stakeholders to inform that some work will be
inspected.
• HAS Construction will collect all delivery notes and will issue a LOG to track
the concrete. All necessary information like time, date, location, quantity,
concrete grade, etc. will be stated in the LOG.
• HAS Construction will issue the plan of the construction joints for all concrete
elements.

These plans will be submitted separately for approval.

The SQAQC Responsible shall verify that:

• Mix Designs have been properly submitted by the main contractor and
approved by the Engineer.
• These can be made and crushed in ready mixed concrete suppliers
laboratory to check that the cured concrete has obtained the required design
strength. A standard 150 x 150 x 150 mm steel test cube mould is used,
thinly coated inside with formwork oil. A concrete sample is taken from the
discharge outlet of mixer or from the point of placing on site.
• The sample cube is covered with a damp sack or other covering and left for 24
hours at a temperature of 4.4-21°C. The sample is then removed from
the mould and stored in water at a temperature 10-21°C until required
for testing.
• Testing of Concrete is carried out in accordance with technical specifications.

• However, an accredited laboratory will be assigned to witness the samples and
tests.
• Specified Formwork and Concrete Mock-ups are completed and approved
• Reinforcing steel quantity, size, location, and support conforms with project
requirements
• Processes are in place to ensure that all pours are properly scheduled and
coordinated with all project disciplines and testing agencies
• Curing and Sealing Processes comply with specifications.
• All inspections and tests noted in the specifications and Special Inspections
Schedule are completed and approved as specified
• Waterproofing and Termite Treatment comply with specifications
• Finishers are aware of the finish tolerances specified
• Post-Tensioned Tendon test logs are maintained as specified
• Proper calibration and certification for all Stressing and Testing Equipment
• Hot & Cold Weather processes are followed when required.
• Ensure that all necessary control joints, expansion joints, embeds, block-outs
and pipe sleeves noted on the plans and in the specifications for all crafts are
installed prior to concrete placement.

11. APPLICATION
• Formwork & Scaffolding
o Formwork installations will be done according to Project drawings. In
accordance with approved formwork systems, the vertical / horizontal
scaffolding arms will be installed according to the determined
intervals.
o Push-Pull props will be used for the vertical alignment and supporting
wall and column formwork according to technical specification and
design.
o Slab probs will be used for supporting slab formwork according to
technical specification and design.
o The scaffold will be erected with educated employees.

• Foundation
o Foundation reinforcement will be prepared at an appropriate place
on ground.
o All steel reinforcement will be free from scales and any appreciable
rust, and steel will conform in all respects to the requirements
specified in EN ISO 15630-1:2002
o The prepared foundation reinforcement will be carried to its
location with tower/mobile Crane and will be installed according to
design drawings.
o Formwork will be prepared with appropriate dimensions. Cleaning
and greasing will be made for easy removal. Approved material will
be used for greasing.

o Adequate and effective precautions will be taken to leave all


reinforcement undisturbed during concreting,
o The surfaces will be cleaned, and air-pressured compressor will
be used when necessary.
o Pour concrete following the steps
 Sample will be taken from truck mixer for slump test
 Concrete pump or tower Crane will be used for carrying
concrete

• Columns
o Column reinforcement will be prepared – longitudinal bars and
stirrups will be tied together with binding wires according to
design drawings – at an appropriate place on ground.

o All steel reinforcement will be free from scales and any
appreciable rust, and steel will conform in all respects to the
requirements specified in EN ISO 15630-1:2002
o The prepared column reinforcement will be carried to its location
with tower/mobile crane and the reinforcement will be fixed to
starter bars coming from bottom level.
o Adequate and effective precautions will be taken to leave all
reinforcement undisturbed during concreting,
o Formwork will be prepared with appropriate dimensions. Cleaning
and greasing will be made for easy removal. Approved material
will be used for greasing.
o Half of the column formwork will be installed (two consecutive
sides forming a corner) with tower crane.
o The column base will be cleaned
o The other half of the formwork will be installed to close the frame
with tower crane.
o Final topographic checks will be made on column formwork
location.

o Shuttering or centering will be true to line and sufficiently strong to


carry the dead weight of wet concrete plus any incidental loading
without noticeable deflection. It will be sufficiently rigid to guard
against movement or vibration whilst concrete is being placed and
tamped into position. Shuttering will be close together to prevent
leakage of the fine stuff and will be ‘treated’ with the minimum of
formwork oil to facilitate striking.
o Cantilever scaffolding will be prepared at two sides of column
formwork top as a safely working platform. The platform will has
side rails on three sides.
o Harness will be applied to fixed points when climbing to high level
areas in general before the formwork platform is erected.
o Ladder will be used to safely reach the top of column formwork.
o Concrete pouring will follow the steps below;
 Sample will be taken from truck mixer for slump test

 Concrete pump or tower Crane will be used for carrying

o Necessary actions to reserve MEP holes will be taken on the walls
as they are designated on design drawings.
o The wall reinforcement will be prepared and installed – horizontal
and vertical bars tied together with binding wires according to

level will be used.
o All steel reinforcement will be free from scales and any appreciable
rust, and steel will conform in all respects to the requirements
specified in EN ISO 15630-1:2002

o Adequate and effective precautions will be taken to leave all


reinforcement undisturbed during concreting,
o Formwork will be prepared with appropriate dimensions. Cleaning

o One long and one short side of the wall formwork will be installed

caps will be prepared and installed with site cut plywood.
o Bottom forms of beams will be prepared and installed with site cut
plywood.
o Side forms of beams will be prepared and installed with site cut
plywood.
o Bottom forms of slabs will be prepared and installed with site cut
plywood.
o Shuttering or centering will be true to line and sufficiently strong to
carry the dead weight of wet concrete plus any incidental loading
without noticeable deflection. It will be sufficiently rigid to guard
against movement or vibration whilst concrete is being placed and

tamped into position. Shuttering will be close together to prevent

be used when necessary. For easy removal, grease will be
applied to all surfaces and approved material will be used.

requirements specified in EN ISO 15630-1:2002
o Mesh reinforcement will comply with EN ISO 15630-1:2002
o Adequate and effective precautions will be taken to leave all
reinforcement undisturbed during concreting,
o Column cap reinforcement will be prepared and installed if any
exist.
o Beam reinforcement will be prepared and installed.
o Bottom slab reinforcement followed by top slab reinforcement will
be prepared and installed.
o Additional slab reinforcement will be prepared and installed if any
exist.
o The formwork will be cleaned apart from dust
o Concrete pouring will follow the steps below Sample will be taken

o Concrete pump or tower Crane will be used for carrying concrete
o Vibrators will be used for consolidation after pouring
o Special edge protection precaution will be taken for slabs to be
erected in more than one pour at a time:
o At the edge of the part poured at first, edge protection will be
prepared to prevent worker/material fall from upper level to the
level below.
o This temporary edge protection will extend from one end of

peripheral edge protection to the other end.
o Sufficient edge protection will be provided at all sides of the slab
where a falling potential exists.

• Curing and Protection of Concrete


o Curing for foundation will be done with water.
o Curing of Slabs, columns and walls will be done by chemical
compound.

• Methods to Control Vertically of Structure

The structure points will be given by the survey team according to the
project. Methods will be used to check or control verticality works include;

o Plumb-bob technique

 A weight with pointed tip will be attached on the bottom end of a


string.
 The heavy weight will hang under gravity and offer a precise
vertical line which is called plumb line.
 This method will be applied for checking or controlling vertical line
of structural elements especially indoors such as lift shaft. Added
to that, it will be used to control verticality of foundation, walls, and
columns.

o Theodolite

 Digital Theodolite centered on a peg that installed 500 mm from


the column grid.
 After Theodolite set up accurately the laser beam will be turned on
and focused it to the steel tape which is held to the formwork.

 the reading of the steel tape will be taken with the telescope at the
same level on both top and bottom levels of the formwork.
 By taking two readings at the same level any curvature on the
surface will be identified.

• Hollow Concrete Block Walls (HCB Walls)


o The measuring, mixing, production and transporting of HCBs will be in
accordance with EN and Malta Local Standards.
o Guide blocks shall be placed onto cement mortar bed; leveled and
plumbed by the help of rubber hammer. Location and level of guide blocks
shall be accurate to wall layout.

o Cement mortar will be applied in between block surfaces with notched
trowel. Horizontal or vertical joint gaps will be approximately 1 – 1,5 cm.

o Wall corners will be constructed by staggered installation as shown on the


figure. Staggered installation will also be provided at partition wall
intersections.

o End of walls will be connected by Hilti nail gun to columns by 2 mm

galvanized type-L clips in every 2nd course. Connection points will not
overlap with the opposite end.

o The gap between wall and ceiling slab will be filled with rockwool. Fire
resistant foam will be applied only one side.

o Blocks will be cut to required dimension with concrete block cutting


machine by the help of set square.

o Door subframes will be connected to 6’’ solid concrete blocks. Solid blocks
will only be used at bottom, mid and top anchorage points.

o Concrete precast / metal lintels will be provided over door openings.


Precast production will be done at job site.

• Waterproofing

For Below Grade;

• Simproof is loose laid on the lean concrete, self adhesive overlaps are
adhered.Overlaps are 10 cm.
• The joints are aligned.
• Remove the protective tape
• Joints are adhered and passed over with a printing roller.
• Final checks are made on the membrane.
• It is a SBS modified waterproofing membrane (4.5mm) for below
grade whichadheres permanently to the structural concrete to be
poured.

For Single Sided Shearwall


• The waterproofing membrane is adhered to the drainage board which
has been previously applied on the shoring wall, which is designed to
be rigid and able to withstand the concrete pressure. Self-adhesive
overlap parts are adhered. Overlaps are 10 cm.

• It is a SBS modified self-adhesive waterproofing membrane which is


used for the waterproofing of blank mould shear walls, which is applied
before concrete is poured on it and can provide a permanent adhesion
to the structural concrete.

• Formwork Removal

a) Reinforced concrete slabs


1. The slab formwork system shall consist of various scaffolding
modules which can be independently dismantled without the need
to dismantle the whole formwork system.

2. In this regard, the undersigned is hereby proposing the removal of


concreteslab formwork subject to the following conditions:

o Formwork can be removed after 4 days from time and date


of casting as long as a 3-day cube crushing test result
proves that concrete has reached a minimum of 70% of its
ultimate strength, or a minimum of 25MPa.
o Formwork shall be removed progressively and a nominal
amount of props shall be immediately reinstated under the
concrete slab, with an average distance of approximately +/-
2.5m. Additional props in the form of scaffolding might be
required at midspan depending on the type of slabs being
cast. The amount of props to be provided after the 4th day
from concrete casting shall be at the discretion of the
undersigned.

b) Reinforced concrete beams


1. Similarly to slabs, beam formwork system shall also consist of
various scaffolding modules which can be independently
dismantled without the need to dismantle the whole formwork
system.
2. In this regard, the undersigned is hereby proposing the removal of
concretebeams formwork subject to the following conditions:

o Formwork can be removed after 7 days from time and date


of casting as long as a 7-day cube crushing test result
proves that concrete has reached a minimum of 70% of its

ultimate strength, or aminimum of 25MPa.
o Formwork shall be removed progressively and a nominal
amount of props shall be immediately reinstated under the
concrete beams, with an average distance of approximately +/-
2.5m. Additional props in the form of scaffolding might be
required at beam midspans depending on the type of beams
being cast. The amount of props to be provided after the 8th
day from concrete casting shall be at the discretion of the
undersigned.

• Housekeeping

Housekeeping will be executed daily wise. Additionally, waterjet or


compressor will be used for cleaning the areas where concrete pouring
will take place.

12. HEALTH AND SAFETY

All activities shall be carried out by taking the following precautions.

• Work area shall be cleared for safe access


• Application area must be clean.
• The shafts, stairs and any openings on the slab shall be covered or
surrounded with safety net and-or temporary railings
• Temporary electrical power shall be provided where necessary for equipment
and temporary illumination.

Also, separate risk assessment will be issued for this method statement.

13. QUALITY CONTROL

Critical stages for inspection to follow are;

• Delivery, storage, and handling of the materials shall be as specified.


• Drawings to be reviewed and checked indicating all locations and including
necessary plans, elevations, sections, details of components and necessary
attachments to the other works.
• Surface preparation shall be checked.
• Materials and equipment shall be clean and free of damage and shall be
checked before us.

Also, separate quality control plan will be issued for this method statement.
