1. This document is to be used by all vessels entering, working or departing an installation or field.
2. Any checks marked ‘No’ shall be discussed with the Installation prior to 500m zone entry.
3. Reference to “Installation” means field, installation or contracted MODU
4. All personnel responsible for 500m zone operations will be familiar with, and work to, the guidance
contained in The NWEA Guidelines for the Safe Management of Offshore Supply and Rig Move
Operations.
A.1 Vessel confirms all waypoints used are outside the 500 m safety zone.
Confirm with installation their intent to receive vessel, intended time alongside and

activity or cargo plans and/or priority.
Vessel confirms that it is aware of and understands the procedures and requirements
A.6 to safely complete the operation as agreed and understands the associated tasks and
has the proper resources / equipment onboard.
All crew are adequately rested; as per ILO Convention N.180 and IMO’s STCW

The Master & Bridge Officers are fully aware of the procedures for vessel entry.
B.1 (Steering offset course, speed, maneuvering, communication points, physical layout
of the installation etc)
Sea and weather conditions including weather forecast assessed and acceptable for

operational period within 500 m safety zone.
Safe direction of approach towards the installation evaluated, (including prevailing and
B.3 predicted current/tidal conditions) checked and confirmed as suitable to carry out
expected operations.
Masters & Bridge Officers of vessels operating on DP are to comply with the

B.6 Guidelines for the Safe Operation of Dynamically Positioned Offshore Supply Vessels
& the
Guidance to Vessel Masters manual. (see separate DP checklist)
Whilst within the 500m zone the bridge will be manned with a minimum of two

competent personnel and the engine room with a minimum of one competent person.
Main Engines / Thrusters: Fully tested & confirmed to be functional and in satisfactory

operating condition.
Steering Gear: Function tested (main and emergency) and confirmed to be fully

operational.
Confirm auto pilot is off after testing and vessel to proceed within the 500 metre zone

at minimum steerage speed.
Communications: VHF sets and two-way comms. Tested and established with Control

Room, Deck Foreman, Cranes and Vessel Deck Crew.

All involved crew informed on planned operation and tool box safety meeting has

B.17 Wearing of PPE by vessel deck crew, including ‘hi-vis’ vests whilst working alongside.
Installation has confirmed readiness for vessel arrival and that their checks as listed

on Data Card have been completed.
Master to record completion of checks in Vessel Deck Log Book and formally request

permission to enter.
Confirm with installation any delays or wait time. If vessel will be standing by,

move a safe distance from the installation and wait for instructions.
Departure route has taken into account wind and current effects in case of

equipment failure so that vessel doesn’t impact installation.
D.4 Vessel has notified installation of intention to depart the 500 m safety zone.
