    PENALTY AGAINST SAFETY VIOLATIONS/ MISCONDUCT

    Date: 23/04/2019

    Dear Sir,

    Here we would like to state our deep concern about the safety measures implemented at our
    Birth site. Despite of repeated instruction & information, some workers of M/S- S (sup-
    GPL)Attn. found without safety shoe inside working premises at G.Floor B#02.

    We are requesting you to take necessary actions to prevent any mishaps in near future.

    So keeping in view to stopping this kind of unsafe act we oblige penalty clause against M/S-
    for the violation of safety rules at site.

    This is your information that a penalty of Rs 1,000/- is imposed against M/S- Ariful Islam
    Contractor for the following safety violation at site.
