On dated 13th Aug 2021 it was observed that, at your area forklift operator Mr. LALAN KUMAR
PASWAN operated the “Hyva truck” ( is a non-transport motor vehicle under Rule 2 (cab)of CMV
Rules, 1989 and comes under Construction Equipment Vehicles, as defined in rule 2(ca) are being
considered to be motor vehicles and under the Central Motor Vehicles Act, 1988, R.2(ca)) without a
valid CNEQP TRANSPORT(HMV) l without a HMV(TRANS) / CNEQP License and Operator Pass from
VAL/RRL HSE team. As per the investigation and statement from the Shift In-charge and Forklift
Operator you and Mr. Joseph (RRL) insisted him to do the unsafe practice. As operator does not have a
valid HMV / CNEQP License and Hyva Operator Pass. It is a Gross violation of CMV Act and VAL
Safety Standard(Vehicle And Driving). If any incident occurs with the vehicle without valid license as per
Motor vehicle act 1988 insurances will not be considered in any manner.

Since you have violated the rules and VAL Safety guidelines a warning letter has been issued to you for
the above violation.This type of activity is not at all tolerable in plant premises and it’s a violation in our
term. This type of behavior will invite negative impact on your carrier and this is highly objectionable,
