Violation: Person engaged in gas cutting job, on shuttering structure made
from wooden members, about 2 meter height without full body harness.
Consequence : Chances of fall

For height jobs lifeline must be provided and person should anchor the lanyard
of full body harness to the lifeline. Ladder must be extended one meter above
the landing area. Landing area must be free from projected nails & other

Ladders were not properly aligned and upper segment of the ladder was not
properly fixed (unstable) at 5m height.

Ladder must be aligned or in continuation so that change over will be safe. Fall

Violation: Life line wire rope has been tied with 12 mm reinforcement bar
separated by about 10 meter and two persons were found anchored their
lanyard with the same life line. As per standard the anchorage point should be
capable of supporting at least 2000 kilograms per person attached.

Violation:
1. One person was found standing on the ”CHAJJA” at about 5 meter
height without using full body harness.
2. Approach chajja /work place was also unsafe.
Safe Method:
Person working or standing on ”CHAJJA” like platform at height
Should wear full body harness. Access should be direct and safe.

Section of ladder hanging dangerously at more then 10 meter height, it could
fall & cause serious injury.
Consequence: Chances of fall of materials.
Safe Method:
Ladder should not be in dangerously hanging condition, it could fall.

1.Five persons were working on the corrugated roof, anchoring the lanyard of
the full body harness to extremely sagged life line.
2.The vertical post of the life line was about 10 meter apart.
Consequence: Chances of injury in case of fall

Safe Method:
While working on roof working at height standard has to be followed. Distance
between two post should not be more than 6m, wire rope to be used as a
lifeline, only two person can anchor their lanyard with 6m section of lifeline.
Edge of the roof must also be protected.
