This is to confirm that the LSA equipment, as specified in below table, have been subjected to a periodic inspection and serviced by a person appropriately
trained and certified in accordance with MSC.402(96) as amended, and that the equipment is fit for purpose at the time of completion of the inspection.
Mandatory periodic servicing and overhaul of launching appliances and on-load release gear in compliance with SOLAS Chapter III regulation 20, including
latest amendments and as required by MSC.402(96) as amended. The ship-owner is responsible for servicing and maintenance on board its ships in
accordance with SOLAS regulation III/20 and for the establishment and implementation of health, safety and environment (HSE) procedures covering all
activities during servicing and maintenance. Weekly and monthly inspections, and routine maintenance as defined by the manufacturer, should be conducted
under the direct supervision of a senior ship's officer in accordance with the instructions provided by the manufacturer.
